Frequency and characteristics associated with opportunistic salpingectomy at cesarean delivery: A retrospective chart review.

OBJECTIVES: To identify the frequency and characteristics associated with total salpingectomy (TS) versus occlusion or partial salpingectomy (PS) at the time of cesarean delivery. STUDY DESIGN: We performed a retrospective chart review of cesarean deliveries with a concurrent permanent contraception procedure, from July 1, 2014 to June 30, 2019 at 2 hospitals (community hospital and tertiary care academic center) within a single healthcare system. We assessed the proportion of TS versus PS at cesarean, and secondarily compared operative times between the 2 procedures. RESULTS: We identified

Hospital level of service, rural-urban location, and neonatal resuscitation interventions: A population study in Alberta Canada from 2000 to 2020.

BACKGROUND AND OBJECTIVES: Advanced neonatal resuscitation interventions (ANRIs) are rarely performed for late preterm and term infants. However, healthcare providers in community hospitals may need to perform ANRIs, while having limited experience and resources. Understanding practice differences between hospitals of different levels of service (LoS) and rural/urban location may inform quality improvement. Our objective are to a) examine how hospital LoS and rural/urban location relate to ANRI rates in Alberta, Canada, a public health system with standardized Neonatal Resuscitation Program® t

Development of a routine bedside CYP2C19 genotype assessment program for antiplatelet therapy guidance in a community hospital catheterization laboratory.

Genotype based personalized antiplatelet therapy in the setting of percutaneous coronary intervention (PCI) has been studied in clinical trials. Despite the demonstrated risk associated with CYP2C19 loss-of-function (LoF) carriage in clopidogrel-treated PCI patients, real-world implementation of genotyping for PCI has been low. The goal of the current study was to provide CYP2C19 genotype information to the interventionalist prior to the completion of the catheterization to facilitate immediate personalized antiplatelet therapy. Routine personalization of P2Y12 inhibitor therapy for PCI in a c

Revolutionising healthcare access: student-led clinics as catalysts for change in an underserved urban community in Karachi, Pakistan.

BACKGROUND: Karachi, Pakistan's largest metropolitan city, faces profound healthcare disparities, with an estimated 45-49% of the population residing in underserved informal settlements. Within these communities, the primary healthcare system is not merely under-resourced but structurally fractured, leaving patients to navigate prohibitively expensive private providers or overcrowded public hospitals. To address these systemic gaps, the Humanity Initiative, a student-led non-governmental organisation, implemented student-led clinics (SLCs) as a community-embedded, cost-effective model to bridg

Delayed discharges in older adults with mental illness and/or dementia in hospital: a scoping review.

BACKGROUND: Delayed discharge among older adults is rising in hospitals across many countries, placing increased strain on healthcare systems and negatively affecting patients' emotional and physical well-being due to prolonged hospital stays and uncertainty. Despite this growing issue, limited research has explored the specific impact of delayed discharge on older adults living with a mental illness and/or dementia. This scoping review aims to examine what is currently known about older adults (aged 65 and over) with a diagnosis of dementia and/or mental illness who experience delayed dischar

From Personal Readiness to Systems Change: Progressive Clinician Cross-Competency Development in the Robert Wood Johnson Foundation Clinical Scholars Program.

PURPOSE: Clinicians are essential to transforming community health; however, traditional medical training does not provide needed multilevel, sequential, and equity-based leadership skills. The Robert Wood Johnson Foundation (RWJF) Clinical Scholars (CS) program was designed to fill this gap through interdisciplinary and applied competency training alongside an intensive community-based project. However, how clinicians operationalize and implement these competencies temporally and in their own terms has yet to be documented. PATIENTS AND METHODS: We conducted a qualitative, retrospective dedu

Navigating complex care pathways-healthcare workers' perspectives on health system barriers for children with tuberculous meningitis in Cape Town, South Africa.

Tuberculous meningitis (TBM) occurs when tuberculosis (TB) bacilli disseminate and seed into the meninges, triggering a severe inflammatory response that often leads to brain infarction. It is the most severe and debilitating form of childhood TB with high mortality, and children who survive TBM often suffer lifelong physical and neuro-disability resulting in emotional, social, and economic burdens for families. In the early stages the symptoms may be non-specific and so the diagnosis is often made late when the patient already has significant brain injury. To facilitate earlier diagnosis, it

The institutional response to mental disorder in Ireland: censuses of Irish asylums, psychiatric hospitals and units 1844-2014.

Before the eighteenth century, there was limited response to the problem of psychiatric illness in Ireland as in many other countries. The asylums of the 1820s and 1830s were no sooner opened than they were overcrowded. A second wave of asylum building commenced in the second half of the nineteenth century continuing up to the early twentieth century. In 1966, the Report of the Commission on Mental Illness noted that the rate of psychiatric beds in Ireland per 1,000 was one of the highest in the world. The report called for a change in the policy of caring for the mentally ill in psychiatric h

Stakeholders' experiences with US hospital emergency preparedness--Part 2.

This study examined, via qualitative phenomenology, important stakeholders' experiences with US hospital emergency preparedness, finding that their experiences varied from positive and effective to frustrating, time-consuming and inefficient. The findings centred on four themes: the importance of funding; collaboration, communication and coordination; the role of government; and hospital leadership buy-in. Together, these findings suggest that issues such as a lack of performance metrics, insufficient funding, inadequate surge capacity, confusing federal guidelines and accreditation requiremen

Characterization of Toxin-Antitoxin (TA) Systems in Pseudomonas aeruginosa Clinical Isolates in Iran.

BACKGROUND: Pseudomonas aeruginosa is among the most problematic hospital and community-acquired pathogens. Toxin-antitoxin (TA) systems are maintenance regulatory systems in bacteria and have recently been considered new targets for antimicrobial therapy. The prevalence and transcription of these systems in clinical isolates are still unknown. OBJECTIVES: The aim of this study was to characterize three types of TA systems (parDE, relBE, and higBA) among P. aeruginosa clinical isolates. MATERIALS AND METHODS: We typed our clinical isolates by ERIC-PCR (enterobacterial repetitive intergenic c

Adult bacterial meningitis: aetiology, penicillin susceptibility, risk factors, prognostic factors and guidelines for empirical antibiotic treatment.

Episodes of adult bacterial meningitis (ABM) at a Danish hospital in 1991-2000 were identified from the databases of the Department of Clinical Microbiology, and compared with data from the Danish National Patient Register and the Danish National Notification System. Reduced penicillin susceptibility occurred in 21 (23%) of 92 cases of known aetiology, compared to an estimated 6% in nationally notified cases (p < 0.001). Ceftriaxone plus penicillin as empirical treatment was appropriate in 97% of ABM cases in the study population, and in 99.6% of nationally notified cases. The notification rat

Vancomycin-Resistant Enterococci in Patients Attending for Colonoscopy: An Estimate of Community Prevalence.

Aims Ireland has the highest vancomycin-resistant Enterococcus faecium (VRE) bloodstream infection prevalence in Europe. Two patterns of VRE carriage are recognised. European, with widespread community prevalence and North American, where carriage is predominantly nosocomial. It is unclear which pattern is dominant in Ireland. This uncertainty limits infection control measures. This study sought to explore this issue via a cross sectional point prevalence study. Methods Asymptomatic community volunteers, represented by patients undergoing elective outpatient colonoscopy testing, were opportuni

Who prefers to age in place? Cross-sectional survey of middle-aged people in Japan.

AIM: Preference for aging in place among middle-aged people is an important element in estimating the future needs for community services of disabled older adults. Drawing on Litwak and Longino's typologies of relocation, the present study investigated the characteristics of middle-aged adults who prefer to age in place and those who prefer to move, at two levels of disability: being unable to walk alone and being bedridden. METHODS: In the present cross-sectional study, an anonymous, self-administered questionnaire was sent to 2500 community-dwelling adults aged 40-64 years. RESULTS: If una
