Targeting host factors: a novel rationale for the management of hepatitis C virus.

Hepatitis C is recognized as a major threat to global public health. The current treatment of patients with chronic hepatitis C is the addition of ribavirin to interferon-based therapy which has limited efficacy, poor tolerability, and significant expense. New treatment options that are more potent and less toxic are much needed. Moreover, more effective treatment is an urgent priority for those who relapse or do not respond to current regimens. A major obstacle in combating hepatitis C virus (HCV) infection is that the fidelity of the viral replication machinery is notoriously low, thus enabl

Repurposing ALK inhibitors as influenza and corona virus antivirals targeting lymphocyte tyrosine kinase (LTK).

The development of effective antiviral drugs for pandemic scenarios is a significant challenge, particularly due to the rapid emergence of drug-resistant viral strains and a paucity of drugs that have broad efficacy across viral families. A promising strategy is so called host-targeted therapeutics which inhibit host-mechanisms that viruses rely on instead of viruses themselves. LTK, an endoplasmic reticulum-resident kinase, plays a crucial role in ER-to-Golgi trafficking, a process exploited by many viruses during their life cycle, and is therefore a potential candidate for host-targeted anti

New understandings on the pathophysiology of bacterial meningitis.

PURPOSE OF REVIEW: Currently, dexamethasone is the only adjuvant of proven benefit in bacterial meningitis. Dexamethasone halves the risk of poor outcome, but only in selected patient groups. New therapies based upon an understanding of the pathophysiology are needed. This article summarizes our knowledge on the pathophysiology of bacterial meningitis with special emphasis on pneumococcal meningitis, the experimentally best characterized subtype. RECENT FINDINGS: Experimental studies made clear that the harmful inflammatory reaction is initiated by the interaction of bacterial products with h

Flagellin pretreatment enhances host early innate immune responses against mycobacterial infection.

Host-directed strategies that enhance early innate immune defense are increasingly recognized as important adjunct approaches for controlling mycobacterial infections. Flagellin, a classical ligand of Toll-like receptor 5 (TLR5), has been widely studied as a vaccine adjuvant. However, whether it can transiently modulate innate immunity to enhance host resistance against mycobacterial infection without inducing sustained inflammation remains unclear in vivo. In this study, we systematically evaluated this question using a zebrafish-Mycobacterium marinum infection model. Our results demonstrate

[Sepsis phenotypes and individualized fluid therapy in patients with chronic diseases].

Sepsis is a clinical syndrome caused by a dysregulated host response to infection, often resulting in life-threatening organ dysfunction. Patients with chronic diseases are more susceptible to sepsis and have a higher risk of mortality due to impaired organ reserve and immune imbalance. Fluid resuscitation remains a cornerstone of sepsis management. However, a "one-size-fits-all" approach may not be suitable for patients with chronic diseases. In recent years, with the development of machine learning, researchers have proposed phenotype-based classification to identify the heterogeneity of sep

Efficacy and Mechanisms of Alkaloids Against Enterovirus A71: A Systematic Review and Meta-Analysis of Preclinical Evidence Integrated with Network Pharmacology and Molecular Docking.

Enterovirus A71 (EV-A71) is a major causative agent of hand, foot, and mouth disease, yet no specific antiviral therapy has been approved. This study systematically evaluated the efficacy and mechanisms of alkaloids against EV-A71 infection by integrating meta-analysis, network pharmacology, and molecular docking. Nine animal studies were included. Meta-analysis suggested that alkaloid intervention significantly improved survival (OR = 30.62, 95% CI: 10.44-89.82), reduced clinical severity, attenuated body weight loss, and decreased viral loads in infected tissues. Subgroup analyses preliminar

Functionalized nanoparticles for the precise treatment and prevention of bacterial infections.

Multidrug-resistant (MDR) bacterial infections are an escalating health issue, and their rise poses a major threat worldwide. Inappropriate antibiotic use, including unnecessary prophylaxis, and nonpathogen-directed therapy, accelerates resistance, diminishing the effectiveness of conventional antibiotics. Consequently, novel antimicrobial strategies are urgently required to achieve precise prevention and treatment of bacterial infections while minimizing selective pressure for resistance. Nanotechnology-based antimicrobials are promising candidates because of their unique advantages: payload

Two Worlds, One Battle: How Bacteria and Malignancies Converge on Drug Resistance.

Drug resistance represents one of the most critical challenges in modern medicine, undermining the efficacy of therapies across both bacterial infections and cancer. Although these conditions arise in fundamentally distinct biological systems, they are governed by shared evolutionary pressures that drive the emergence and selection of resistant populations. This narrative review provides an integrative, cross-disciplinary perspective on drug resistance, focusing on bacteria and cancer and emphasizing the shared evolutionary and molecular mechanisms underlying treatment failure in both domains.

Dendritic cell targeting in lymph nodes with modular adapters boosts HAdV5 and HC-HAdV5 tumor vaccination by co-secretion of IL-2v and IL-21.

Adenoviral vectors demonstrate encouraging clinical outcomes for B and T cell vaccines. With such approaches, multiple payloads can be delivered, beyond the antigen itself. Nevertheless, the human adenoviral vector serotype C5 (HAdV5) exhibits limited transduction efficiency to dendritic cells (DCs), therefore necessitating very high viral loads. Targeting antigen-presenting cells (APCs) has remained challenging. To solve this problem, we developed a versatile platform that employs modular retargeting adapters to enhance transduction of specific cell types, including challenging host cells. By

ACOD1-mediated lysosomal membrane permeabilization contributes to Mycobacterium tuberculosis-induced macrophage death.

Mycobacterium tuberculosis (Mtb) primarily infects macrophages. In vitro without antibiotics, wild-type Mtb hastens death of the macrophages … ed cell death. Neither ACOD1 inhibitors nor cysteine protease inhibitors are suitable for potential host-directed therapy (HDT) of tuberculosis. Instead, this work directs attention to how ACOD1 acts nonenzymatically to promote the degradation of HSP70.

Promising effects of parasite-derived compounds on tumor regression: a systematic review of in vitro and in vivo studies.

The parasites are repeatedly confronting their host to take advantage of nutrients for multiplication and survival. In this sense, a wide spectrum of molecules is released from both sides, with immune-regulatory activity, accompanying this biological battle. Such parasites and their valuable molecules can be directed toward microbial-based cancer therapy. Herein, we contrived a systematic review to gather information on the antitumor activity of parasite-derived compounds. Following systematic search in Web of Science, ScienceDirect, Scopus, PubMed, ProQuest and Embase until 31 December 2019,

Clostridium difficile toxins induce VEGF-A and vascular permeability to promote disease pathogenesis.

Clostridium difficile infection (CDI) is mediated by two major exotoxins, toxin A (TcdA) and toxin B (TcdB), that damage the colonic epithelial barrier and induce inflammatory responses. The function of the colonic vascular barrier during CDI has been relatively understudied. Here we report increased colonic vascular permeability in CDI mice and elevated vascular endothelial growth factor A (VEGF-A), which was induced in vivo by infection with TcdA- and/or TcdB-producing C. difficile strains but not with a TcdA-TcdB- isogenic mutant. TcdA or TcdB also induced the expression of VEGF-A in human

Matrix metalloproteinase-9 activity and a downregulated Hedgehog pathway impair blood-brain barrier function in an in vitro model of CNS tuberculosis.

Central nervous system tuberculosis (CNS TB) has a high mortality and morbidity associated with severe inflammation. The blood-brain barrier (BBB) protects the brain from inflammation but the mechanisms causing BBB damage in CNS TB are uncharacterized. We demonstrate that Mycobacterium tuberculosis (Mtb) causes breakdown of type IV collagen and decreases tight junction protein (TJP) expression in a co-culture model of the BBB. This increases permeability, surface expression of endothelial adhesion molecules and leukocyte transmigration. TJP breakdown was driven by Mtb-dependent secretion of ma

Natural Products as Promising Therapeutics for Treatment of Influenza Disease.

The influenza virus represents a permanent global health threat because it circulates not only within but also between numerous host populations, thereby frequently causing unexpected outbreaks in animals and humans with a generally unpredictable course of disease and epidemiology. Conventional influenza therapy is directed against the viral neuraminidase protein, which promotes virus release from infected cells, and the viral ion channel M2, which facilitates viral uncoating. However, these drugs, albeit effective, have a major drawback: their targets are of a highly variable sequence. As a c
