Evaluation of brain SPECT in the diagnosis and prognosis of the normal pressure hydrocephalus syndrome.

From January 1989 to September 1990 fourteen patients with the Normal Pressure Hydrocephalus (NPH) syndrome were diagnosed in our Service. All patients were studied by CT-Scan, Radionuclide Cisternography and brain Single Photon Emission Computed Tomography (SPECT). In this series, the predictive value of the brain SPECT was assessed. A statistically significant relationship between SPECT patterns and clinical outcome was found. The patients who showed a Dementia Alzheimer Type (DAT) pattern on SPECT had worse results. The results are discussed and compared with those reported by other authors

Quantitative Analysis of Cerebrospinal Fluid Pressure Gradients in Healthy Volunteers and Patients with Normal Pressure Hydrocephalus.

Magnetic resonance imaging (MRI) can depict not only anatomical information, but also physiological factors such as velocity and pressure gradient. Measurement of these physiological factors is necessary to understand the cerebrospinal fluid (CSF) environment. In this study we quantified CSF motion in various parts of the CSF space, determined changes in the CSF environment with aging, and compared CSF pressure gradient between patients with idiopathic normal pressure hydrocephalus (iNPH) and healthy elderly volunteers. Fifty-seven healthy volunteers and six iNPH patients underwent four-dimens

Link between vasogenic waves of intracranial pressure and cerebrospinal fluid outflow resistance in normal pressure hydrocephalus.

Recent studies on normal pressure hydrocephalus (NPH) have pointed to a possible link between the disturbance in CSF circulation and cerebrovascular factors. We investigated the quantitative relationship between the resistance to CSF outflow (Rcsf) and vasogenic waves of ICP in patients with normal pressure hydrocephalus. Forty-five patients with NPH were investigated by an infusion study. The magnitudes of vasogenic ICP components: pulse, respiratory and slow vasogenic waves were assessed, and compared with Rcsf. Both baseline respiratory and slow waves of ICP were positively correlated with

Predictability of intracranial pressure oscillations in patients with suspected normal pressure hydrocephalus by transcranial Doppler ultrasound.

Intracranial pressure (ICP) was monitored continuously for one night in 36 patients with suspected symptomatic normal pressure hydrocephalus (NPH) to identify patients who might benefit from subsequent shunting. In 33 of these patients middle cerebral artery (MCA) blood flow velocity by means of transcranial Doppler sonography (TCD) and ICP were recorded simultaneously. ICP B-waves always paralleled changes in the TCD signal (TCD B-wave equivalents). The relative frequency of ICP B-waves was predictable by TCD, albeit slightly underestimated due to a generally lower relative amplitude of the T

Magnetisation transfer ratio is low in normal-appearing cerebral white matter in patients with normal pressure hydrocephalus.

We measured the magnetisation transfer ratios (MTR) of normal-appearing rostral (PR) and caudal (PC) periventricular white matter, the genu (CG) and the splenium (CS) of the corpus callosum and the thalamus (TH) in 12 patients with normal-pressure hydrocephalus (NPH) and compared them with 16 healthy control subjects. We found a significantly lower MTR in the NPH group than in the normal group for PR, PC, CG, and CS but not for TH. MT measurements give additional information which cannot be gained by conventional MRI, suggesting that NPH is associated with diffuse white matter damage, even in

[Comparison of quantitative image indexes of brain MRI between differentiates idiopathic normal pressure hydrocephalus and Alzheimer disease, predict positive response of the CSF drainage in possible idiopathic normal pressure hydrocephalus].

AIM: The clinical guidelines for idiopathic normal pressure hydrocephalus (iNPH) in Japan recommend cerebrospinal fluid (CSF) drainage. The positive response rate of the diagnostic CSF drainage is not very high because brain MRI findings of Alzheimer disease (AD) are similar to those of iNPH. Therefore, we sought to determine simple, quantitative indexes of head MRI to differentiate iNPH from AD and to predict positive response of the CSF drainage in possible iNPH. METHODS: Eighteen patients with the clinical criteria of possible iNPH who had undergone diagnostic CSF drainage were evaluated.

Characterisation of the complexity of intracranial pressure signals measured from idiopathic and secondary normal pressure hydrocephalus patients.

Hydrocephalus is a condition characterised by enlarged cerebral ventricles, which in turn affects intracranial pressure (ICP); however, the mechanisms regulating ICP are not fully understood. A nonlinear signal processing approach was applied to ICP signals measured during infusion studies from patients with two forms of hydrocephalus, in a bid to compare the differences. This is the first study of its kind. The two forms of hydrocephalus were idiopathic normal pressure hydrocephalus (iNPH) and secondary normal pressure hydrocephalus (SH). Following infusion tests, the Lempel-Ziv (LZ) complexi

[Continuous lumbar subarachnoid pressure monitoring as an indicator of shunt operation for so-called normal pressure hydrocephalus].

Lumbar subarachnoid pressure (LSP) was continuously monitored via intrathecally introduced polyethylene catheter to select the patients for shunt operation. A total of seventy cases included so-called normal pressure hydrocephalus (NPH) secondary to subarachnoid hemorrhage (SAH; 34 cases), idiopathic NPH (17 cases), secondary NPH whose symptoms developed after operations for brain tumors, head injuries or meningitis (12 cases) and other intracranial diseases including pseudotumor cerebri or meningeal carcinomatosis, etc. (7 cases). Shunt operation was effective in 36 cases and not effective in

Spatially resolved periventricular ADC profiling in normal pressure hydrocephalus: an exploratory pilot study.

PURPOSE: To explore whether boundary-anchored apparent diffusion coefficient (ADC) profiling can characterize periventricular diffusion patterns in normal pressure hydrocephalus (NPH). MATERIALS AND METHODS: Seventeen patients with shunted NPH with baseline and postoperative follow-up ADC maps acquired at 1.5 T or 3 T and 17 age-, sex-, and scanner-matched controls were analyzed retrospectively. A short anchor was placed along the frontal horn ventricular boundary. ADC values were sampled along approximately orthogonal lines and summarized as amplitude- and shape-related profile metrics. Clinical profile of idiopathic normal pressure hydrocephalus with reduced myocardial sympathetic nerve function.

INTRODUCTION: Findings of reduced cardiac sympathetic nerve function and pareidolic illusions have been useful for identifying Lewy body disease (LBD). Therefore, we aimed to investigate whether these findings are associated with a high prevalence of LBD-related symptoms and poor shunt responsiveness in patients with idiopathic normal pressure hydrocephalus (iNPH). METHODS: We retrospectively analyzed 104 iNPH patients who underwent preoperative 123I-metaiodobenzylguanidine myocardial scintigraphy and the Pareidolia test. We divided the patients into the abnormal cardiac scan (AC) group and t

Targeted CSF metabolomics and conformal prediction improve diagnostic accuracy of normal pressure hydrocephalus.

BACKGROUND AND OBJECTIVES: Idiopathic normal pressure hydrocephalus (iNPH) is a progressive but treatable neurological disorder. Yet, diagnosis is often confounded by overlapping symptoms and biomarker profiles with Alzheimer’s disease (AD), mild cognitive impairment (MCI), and frontotemporal dementia (FTD). We aimed to determine whether cerebrospinal fluid (CSF) metabolomic profiling, combined with uncertainty-aware machine learning using conformal prediction (CP), could improve diagnostic differentiation of iNPH. METHODS: CSF samples were collected from 120 patients with iNPH, 44 healthy con

Impact of frailty on gait outcomes in idiopathic normal pressure hydrocephalus after shunting.

OBJECTIVES: Frailty, reflecting reduced physiological reserve and increased vulnerability to stressors, is common among patients with idiopathic normal pressure hydrocephalus (iNPH). The extent to which frailty influences gait outcomes following ventriculoperitoneal shunting (VPS) remains unclear. This study evaluated the association between frailty and objective gait changes after VPS in patients with iNPH. METHODS: A single-center database was reviewed for patients who underwent VPS for iNPH with preoperative gait analysis between October 2018 and May 2024. Gait parameters included cadence,

CSF protein clearance impairment revealed using stable isotope kinetics in normal pressure hydrocephalus.

Normal pressure hydrocephalus is a common cause of gait and cognitive impairment in older adults, marked by excessive CSF accumulation. Genetic studies suggest impaired fluid clearance, and clinical symptoms can improve after CSF diversion. However, no fluid biomarkers exist to explore CSF accumulation mechanisms, assist diagnosis, or predict response to treatment. Stable isotope labelling kinetics is a clinical research tool that uses non-radioactive isotopes to label newly translated proteins, enabling measurement of their appearance (synthesis) and disappearance (clearance) in compartments

Effect of Anti-Siphon Devices on Postoperative Outcomes in Idiopathic Normal Pressure Hydrocephalus.

OBJECTIVE: Ventriculoperitoneal shunts (VPS) integrated with purported anti-siphon devices (ASDs) were introduced with the goal of reducing the sequelae of overdrainage. We aim to compare the clinical benefit and complications of patients with idiopathic normal pressure hydrocephalus who underwent VPS surgery with or without an integrated ASD. METHODS: We retrospectively reviewed 140 patients who underwent a lumbar drain trial for possible idiopathic normal pressure hydrocephalus, 99 of whom underwent VPS surgery. Overdrainage complications included subdural hematoma, subdural hygroma, overdr

Pilot Evaluation of Rapid 2D Brain MR Elastography in Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Magnetic Resonance Elastography (MRE) of the brain provides quantitative biomarkers of mechanical properties of brain tissue. Prior studies have shown that an advanced 3D MRE can differentiate idiopathic normal pressure hydrocephalus (iNPH), from other neurodegenerative disorders. In this pilot study, we assessed whether a simplified 2D MRE protocol, that is more widely available, could detect stiffness differences between iNPH and non-iNPH cognitively impaired patients, thus providing preliminary diagnostic discrimination, while using a previously developed 3D MRE classifier as a

Reversible visual field loss in glaucoma following ventriculoperitoneal shunting for normal pressure hydrocephalus.

We report a case of visual field deterioration in previously stable open-angle glaucoma which resolved upon ventriculoperitoneal shunting (VPS) for coexisting normal pressure hydrocephalus (NPH). A woman in her 80s with bilateral open-angle glaucoma developed apparent progression of existing glaucomatous field defects, despite stable disease and consistent intraocular pressure control following bilateral trabeculectomies over a decade prior. She was concurrently diagnosed with NPH and underwent VPS. Following VPS, visual fields returned to a previously stable baseline.Visual field fluctuations

Molecular Signatures of Normal Pressure Hydrocephalus: A Largescale Proteomic Analysis of Cerebrospinal Fluid.

Given the persistent challenge of differentiating idiopathic Normal Pressure Hydrocephalus (iNPH) from similar clinical entities, we conducted an in-depth proteomic study of cerebrospinal fluid (CSF) in 28 shunt-responsive iNPH patients, 38 Mild Cognitive Impairment (MCI) due to Alzheimer's disease, and 49 healthy controls. Utilizing the Olink Explore 3072 panel, we identified distinct proteomic profiles in iNPH that highlight significant downregulation of synaptic markers and cell-cell adhesion proteins. Alongside vimentin and inflammatory markers upregulation, these results suggest ependymal

Evaluation of imaging indicators in differentiating idiopathic normal pressure hydrocephalus from Alzheimer's disease.

BACKGROUND: There are currently many imaging indicators for idiopathic normal pressure hydrocephalus (iNPH). However, their diagnostic performance has not been well compared, especially in differentiating iNPH from Alzheimer's disease (AD). This study aimed to evaluate the diagnostic performance of these imaging indicators in differentiating iNPH from AD. METHODS: We retrospectively collected patients with iNPH from the West China Hospital between June 2016 and December 2023. Age-sex-matched patients with AD and healthy controls (HCs) are included as controls (ChiCTR2300070078, March 2023). T
