A case of severe hyperaldosteronism caused by a de novo mutation affecting a critical salt bridge Kir3.4 residue.

CONTEXT: Familial hyperaldosteronism type III (FH-III) is a rare and clinically heterogeneous condition, that can display mild as well as severe phenotypes. Point mutations in the KCNJ5 gene, affecting the ion selectivity of the inward rectifier K(+) channel 4 (Kir3.4), underlie the molecular basis of FH-III. OBJECTIVE: The objective of the study was to investigate the effects of a de novo germline KCNJ5 mutation. PATIENTS AND METHODS: We describe the case of a girl who came to medical attention at the age of 2 years because of polydipsia, polyuria, and failure to thrive. The patient, affect…

[Determining aldosterone in the adrenal veins in order to establish uni- or bilateral aldosterone production in patients with primary hyperaldosteronism].

In 3 patients, men aged 60, 55 and 60, respectively, with hypertension due to primary hyperaldosteronism, the aldosterone level in the adrenal veins was determined for the purpose of further diagnosis. In two patients, unilateral adrenal enlargement on the CT-scan was accompanied by overproduction ofaldosterone, in one case in a non-enlarged adrenal gland and in the other case in both adrenals. The first patient underwent adrenalectomy of the non-enlarged adrenal gland, while in the second patient surgery was decided against. The third patient had bilateral adrenal gland enlargement on the CT-…

The utility of three different methods for measuring urinary 18-hydroxycortisol in the differential diagnosis of suspected primary hyperaldosteronism.

OBJECTIVE: Urine 18-hydroxycortisol (18-OHF) measurements are claimed to discriminate between primary hyperaldosteronism due to Conn's syndrome/adrenal adenoma or idiopathic bilateral adrenal hyperplasia (BAH), and also to identify cases of glucocorticoid-suppressible hyperaldosteronism (GSH). We have evaluated three urine 18-OHF methods using a panel of urine samples from patients with hypertension. DESIGN: Clinical methods comparative study. METHODS: Urine samples from patients with primary hyperaldosteronism due to either adenoma (n = 6), BAH (n = 6), GSH (n = 9), or essential hypertensio…

Comparison of active renin concentration and plasma renin activity for the diagnosis of primary hyperaldosteronism in patients with an adrenal mass.

OBJECTIVE: Plasma aldosterone concentration (PAC) to plasma renin activity (PRA) ratio is an established screening test for primary hyperaldosteronism. Due to the increased recognition of adrenal incidentalomas, reliable parameters are required. Determination of active renin concentration (ARC) in contrast to PRA offers advantages with regard to processing and standardization. The present study compared PRA and ARC under random conditions to establish thresholds for the diagnosis of primary hyperaldosteronism. DESIGN AND METHODS: Fifty patients with various adrenal tumors, including ten patie…

The value of adrenal venous sampling in unveiling the conundrum of double unilateral adrenal adenomas in a case of primary hyperaldosteronism.

Primary hyperaldosteronism (PHA) is a cause of secondary hypertension, often due to adrenal adenoma. The diagnosis and management of multiple unilateral adrenal adenomas in PHA represent a unique challenge. Adrenal venous sampling (AVS) has become a pivotal diagnostic tool in these cases, providing critical information for accurate localization of aldosterone production. Here, we describe a rare case of a 52-year-old female with a history of resistant essential hypertension who presented with multiple unilateral adrenal adenomas concomitant with adrenal hyperplasia in a case of primary hyperal…

Integrative bioinformatics approach identifies novel drug targets for hyperaldosteronism, with a focus on SHMT1 as a promising therapeutic candidate.

Primary aldosteronism (PA), characterized by autonomous aldosterone overproduction, is a major cause of secondary hypertension with signific … e bioinformatics methods and experimental validation successfully identified novel drug targets for hyperaldosteronism. SHMT1, in particular, represents a promising candidate for future therapeutic development. These findings provide new opportunities for developing causative treatments for PA, though further clinical validation is warranted.

Efficacy, Quality of Life, and Cost-Effectiveness of Superselective Adrenal Arterial Embolization in Idiopathic Hyperaldosteronism: A Comparative Study.

Idiopathic hyperaldosteronism (IHA) is the most common subtype of primary aldosteronism, typically managed with mineralocorticoid receptor antagonists (MRAs). However, long-term MRA therapy is associated with suboptimal cardiovascular outcomes and adverse effects. Superselective adrenal arterial embolization (SAAE) is a novel minimally invasive alternative, but its long-term efficacy, particularly regarding quality of life and cost-effectiveness, remains underexplored. In this study, 62 patients with bilateral IHA were prospectively enrolled and assigned to two groups: SAAE (n = 42) and MRA th…

Diagnostic Challenges of Adrenal Venous Sampling for Primary Hyperaldosteronism in a Patient With Subclinical Cushing's Syndrome: A Case Report.

Hypertension is a significant comorbidity in the New World, and its prevalence is rising. Around 5%-10% of cases with hypertension have secondary hypertension. Adrenal gland disease is among the common causes of secondary hypertension. We present a 57-year-old male with uncontrolled hypertension and a history of intracranial hemorrhage. The lab evaluation of the patient revealed a K+ of 3.8 mEq/L, Na + of 138 mEq/L, Renin of 2.6 mIU/L, Aldosterone of 47.3 ng/dL, consistent with primary hyperaldosteronism (PHA). The patient's abdominal computed tomography (CT) revealed an adrenal mass measuring…

Rising Prevalence of Obesity and Primary Hyperaldosteronism: Co-incidence or Connected Circumstances Leading to Hypertension? A Narrative Review.

While obesity and its associated complications, mainly diabetes and hypertension, have been the largest public health problems of modern world, the emerging data suggests an increasing prevalence of primary hyperaldosteronism (PA) as one of the most common undiagnosed causes of hypertension. We believe that rising prevalence of PA in the era of high rates of obesity is likely not a chance finding but is deeply intersected with the rising rates of obesity. Higher serum aldosterone concentrations and urinary aldosterone excretion have been observed in patients with increased body mass index or l…

Diagnosis and management of primary hyperaldosteronism in patients with hypertension: a practical approach endorsed by the British and Irish Hypertension Society.

Alongside the lack of homogeneity among international guidelines and consensus documents on primary hyperaldosteronism, the National UK guidelines on hypertension do not provide extensive recommendations regarding the diagnosis and management of this condition. Local guidelines vary from area to area, and this is reflected in the current clinical practice in the UK. In an attempt to provide support to the clinicians involved in the screening of subjects with hypertension and clinical management of suspected cases of primary hyperaldosteronism the following document has been prepared on the beh…

Development of [18F]AldoView as the First Highly Selective Aldosterone Synthase PET Tracer for Imaging of Primary Hyperaldosteronism.

The purpose of this study was to synthesize a fluorine-18 labeled, highly selective aldosterone synthase (hCYP11B2) inhibitor, [18F]AldoView, and to assess its potential for the detection of aldosterone-producing adenomas (APAs) with positron emission tomography in patients with primary hyperaldosteronism (PHA). Using dibenzothiophene sulfonium salt chemistry, [18F]AldoView was obtained in high radiochemical yield in one step from [18F]fluoride. In mice, the tracer showed a favorable pharmacokinetic profile, including rapid distribution and clearance. Imaging in the adrenal tissue from patient…

A potential role for CPY17 as a parameter in differentiation between aldosterone-producing adenoma and nodular hyperplasia in patients with hyperaldosteronism.

OBJECTIVE: To investigate CYP17 mRNA and protein expressions in aldosterone-producing adenoma (APA), nodular hyperplasia (NH) and normal adrenal gland (NAG) and if CPY17 might be used as a potential marker to differentiate between APA and NH in patients with hyperaldosteronism. METHODS: Total RNA and protein were extracted from APA, 12 NH, and 15 NAG tissues. mRNA and protein expressions of CPY17 were examined by real-time polymerase chain reaction (PCR) and Western blot analysis. RESULTS: The relative expression levels of CPY17mRNA to glyceraldehyde 3-phosphate dehydrogenase in the APA, NH,…

[Subclinical endothelial inflammation markers in a family with type I familial hyperaldosteronism caused by a de novo mutation].

BACKGROUND: Type I familial hyperaldosteronism is caused by the presence of a chimaeric gene CYPl 1B1/CYP11BZ which encodes an enzyme with aldosterone synthetase activity regulated by adrenocorticotrophic hormone (ACTH). Therefore, in patients with FH I is possible to normalize the aldosterone levels with glucocorticoid treatment. Recently it has been shown that aldosterone plays a role in the production of endothelial oxidative stress and subclinical inflammation. AIM: To evaluate subclinical endothelial inflammation markers, like Metalloproteinase 9 (MMP-9) and ultrasensitive C reactive pro…
