The parathyroid glands play a critical role in the maintenance of calcium homeostasis. It has been suggested that the set-point for calcium-regulated parathyroid hormone (PTH) release is higher in uremic patients than normal subjects. We describe a 69-year-old woman with a Milwaukee shoulder syndrome in the presence of primary hyperparathyroidism. After removal of a parathyroid adenoma, effusion of the shoulder did not recur. We believe that primary hyperparathyroidism and Milwaukee shoulder can be causally related.

Primary hyperparathyroidism is a chronic, multi-system disease due to an overproduction of PTH from adenoma, hyperplasia or carcinoma of the parathyroid. Classical disease type classification consisted of bone type, kidney type and latent or chemical type. Episodes of transient thyrotoxicosis after surgery for primary hyperparathyroidism have previously been described, and surgical trauma to the thyroid gland has been suggested as an etiologic factor. Hypercalcaemia was found in a 54-year-old woman suffering from dystrophia myotonica. The diagnosis of primary hyperparathyroidism was made. The patient was not considered to be a suitable candidate for surgery. Therefore she underwent ultrasonically guided infiltration of the causative adenoma with 2.8 ml alcohol 95% in three sessions. We delineate the case history of a 74-year-old female patient. On the occasion of acute physically established psychoses in this case, we were able to detect a primary hyperparathyroidism due to adenoids of the epithelial body. This critical psychopathological condition developed from a psychosyndrome, most likely seeming as senile schizophrenia, and which had previously existed for four years. It will be discussed whether these chronic schizophrenia-like symptoms and signs can be explained by hyperparathyroidism.

In a series of 416 parathyroidectomies for primary or secondary hyperparathyroidism, 19 were reoperations for persistence (17 cases) on recurrence (2 cases) of the disease. (1) Preoperative localisation studies were useless in half of the cases. (2) In re-explorations, 72% only of parathyroid glands were discovered, 46% of them in ectopic locations. In reoperations for primary hyperparathyroidism, 56% of cases had more than one pathological gland. (3) Long term results have been less satisfactory after re-explorations than after the first operations.

Patients with mild asymptomatic primary hyperparathyroidism who do not meet currently accepted guidelines for surgery may be followed medically. A patient on long term haemodialysis for analgesic nephropathy, with known teriary hyperparathyroidism developed a rapidly evolving, erosive, non-infective spondylo-arthropathy at two vertebral levels. In addition, erosions were noted adjacent to the right sacroiliac joint. Subsequent postmortem material from the affected vertebral levels demonstrated changes consistent with hyperparathyroidism alone and this is suggested as the possible aetiology underlying this recently recognised destructive spondyloarthropathy.
