Drugs used in paediatric bone and calcium disorders.

Calcium and bone disorders in paediatrics are treated with a variety of drugs, many of which, although licensed for use in adults, are not so in children but are nevertheless used on the basis of accepted practice. The mainstay of drug treatment for osteoporosis is the bisphosphonates which alter the balance between bone accretion and reabsorption mainly by temporarily reducing the activity of osteoclasts. Vitamin D and its metabolites are used for the treatment of various forms of rickets and vitamin D deficiency and the active metabolites are also employed when hypoparathyroidism causes hypo

Small-molecule FGFR-targeted medicinal chemistry: Advances since 2020 and future perspectives.

FGFR alterations, including fusions, amplifications, rearrangements, and mutations, exist as pathogenic drivers or bypass mechanisms in numerous diseases and cancers. Thus, FGFRs represent crucial therapeutic targets, particularly in oncology. Various agents, especially selective FGFR inhibitors, have shown promising therapeutic potential in oncological diseases. However, off-target toxicities (e.g., hyperphosphatemia) and acquired drug resistance associated with FGFR inhibitors often result in disease progression and unfavorable outcomes for patients, constraining clinical utility. This drive…

Klinefelter's syndrome with seizure, pseudohypoparathyroidism type Ib and multiple endocrine dysfunctions.

Klinefelter's syndrome is rarely associated with hypocalcemia, especially pseudohypoparathyroidism (PHP) type Ib. We describe a case of Klinefelter's syndrome associated with seizure, PHP type Ib and multiple endocrine dysfunctions. A 19-year-old Taiwanese male was admitted due to seizures with loss of consciousness. He had been diagnosed with Klinefelter's syndrome with seizure disorder and hypocalcemia 3 months previously. Physical examination revealed eunuchoidism but no osteodystrophy, while laboratory data revealed severe hypocalcemia, hyperphosphatemia, and elevated parathyroid hormone. …

New strategies for the treatment of secondary hyperparathyroidism.

Classic pathogeneses of secondary hyperparathyroidism (2HPT), hyperphosphatemia, vitamin D deficiency, and hypocalcemia, have been treated by the administration of phosphorus binders and vitamin D derivatives. However, these therapies have not brought about a successful result. The main reason could be attributed to hypercalcemia resulting from the administration of calcium salts as a phosphorus binder and the calcemic action of vitamin D. To prevent hypercalcemia, non-calcium-containing phosphorus binders and vitamin D analogues, which suppress parathyroid hormone (PTH) secretion with minimum…

Isolated adrenocorticotropin deficiency presenting with impaired renin-angiotensin-aldosterone system and suppressed parathyroid hormone-vitamin D axis.

We report here a 47-year-old woman with isolated adrenocorticotropin (ACTH) deficiency (IAD). She presented impaired renin-angiotensin-aldosterone (R-A-A) system and suppressed parathyroid hormone (PTH)-vitamin D system. She showed severe hyponatremia due to secondary adrenocortical insufficiency, which was deteriorated by hypoaldosteronism. She also showed hyperphosphatemia and relative hypercalcemia with suppressed PTH-vitamin D axis. Moreover, she showed hypothyroidism, which was thought to be important to maintain normal Ca levels under secondary hypoadrenalism via decrease in bone resorpt…

Perineal herniorrhaphy: perioperative data from 100 dogs.

One hundred dogs (83 intact males, 15 castrated males, and two intact females) underwent 110 perineal herniorrhaphy procedures. Mixed-breed dogs (n = 32), miniature poodles (n = 14), Boston terriers (n = 11), and Pekingese (n = 9) were represented most frequently. Perineal swelling (n = 48) and a perineal defect on rectal palpation (n = 31) were common clinical signs. Twenty dogs had urinary bladder retroflexion and were significantly more likely to have elevated serum urea nitrogen and creatinine concentrations, hyperkalemia, hyperphosphatemia, and neutrophilic leukocytosis. Only five of 43 d…

[Metastatic pulmonary calcification in patients with chronic renal insufficiency].

Case reports on two patients with metastatic pulmonary calcification are presented. Both suffered from long standing chronic renal failure and received immunosuppressive therapy for a (non-functioning) renal transplant. Laboratory tests disclosed hyperphosphatemia and secondary hyperparathyroidism. In the first patient, who presented with "pulmonary edema", the course was rapidly fatal. Diffuse pulmonary calcification was diagnosed only post mortem. Transbronchial biopsy was diagnostic for calcification in the second patient, who had exertional dyspnea and bilateral, asymmetric, interstitial i…

Systemic Phosphate Elevations Induce Fibroblast Growth Factor 23 Production in Skeletal Muscle to Reduce Renal Phosphate Reabsorption in Mice.

KEY POINTS: Our study identified skeletal muscle as a novel source of fibroblast growth factor 23 (FGF23) in scenarios of hyperphosphatemia, including high dietary phosphate intake and CKD. Skeletal muscle-derived FGF23 had endocrine actions lowering serum phosphate levels, which may protect from hyperphosphatemia-induced tissue damages. Skeletal muscle-derived FGF23 might also have paracrine effects counteracting atrophy. BACKGROUND: Fibroblast growth factor 23 (FGF23) is a hormone that reduces the renal reabsorption of phosphate in response to systemic phosphate elevations. In CKD, serum le…

[A case report of severe hypocalcemia secondary to Fahr's syndrome].

Fahr's syndrome is a rare neurological disease characterized by bilateral intracranial calcifications associated with disturbances in calcium-phosphate metabolism, most commonly due to hypoparathyroidism. We report a 56-year-old man presenting with generalized seizures and behavioral changes. Laboratory evaluation revealed severe hypocalcemia, hyperphosphatemia, and low parathyroid hormone levels. Brain computed tomography demonstrated symmetrical calcifications involving the basal ganglia and other subcortical structures, consistent with Fahr's syndrome. Diagnosis was established through the …

Parathyroid Hormone: A Uremic Toxin.

Parathyroid hormone (PTH) has an important role in the maintenance of serum calcium levels. It activates renal 1α-hydroxylase and increases … ith a decline in its serum levels, decrease in intestinal calcium absorption, and, at later stages, hyperphosphatemia and high levels of PTH. In this paper, we aim to critically discuss severe CKD-related hyperparathyroidism, in which PTH, through calcium-dependent and -independent mechanisms, leads to harmful effects and manifestations of the uremic syndrome, such …

Autosomal dominant hypocalcemia due to a truncation in the C-tail of the calcium-sensing receptor.

Autosomal Dominant Hypocalcemia (ADH) is an endocrine disorder due to activating mutations of the calcium-sensing receptor (CASR) gene. We report on a young boy who presented low serum calcium with hypercalciuria, hyperphosphatemia and low serum concentration of parathyroid hormone, not accompanied by classic clinical signs of hypocalcemia. Treatment with calcitriol and calcium did not normalize serum calcium and renal calcium excretion. The use of thiazide diuretics slightly reduced calciuria. Despite high calcium excretion, no signs of nephrocalcinosis were detected. The patient had a prolon…
