Immune checkpoint inhibitors can cause endocrine toxicities, although involvement of multiple hormonal axes is uncommon. Hypophysitis associated with programmed death-1 inhibitors such as pembrolizumab may lead to life-threatening central adrenal insufficiency and can present diagnostic challenges. We present the case of a 58-year-old woman with metastatic non-small cell lung cancer who developed pembrolizumab-induced hypophysitis resulting in central adrenal insufficiency with delayed recognition, and concurrent immune-related thyroiditis.

Autoantibodies to human pituitary cytosol proteins were determined by immunoblotting in sera from patients with hypopituitarism and their relatives. Inflammatory and infectious diseases of the pituitary gland are rare, and imaging diagnosis may be difficult. They encompass a wide spectrum of pathology including autoimmune (lymphocytic) hypophysitis, granulomatous hypophysitis, local manifestations of systemic disease, and a multitude of infectious processes. Based on extensive review of the literature, this article will present a classification scheme, description of pathology, and imaging findings of the various inflammatory and infectious entities, along with selected images from our case material.

Ipilimumab, a monoclonal antibody against CTLA-4, is used in the treatment of melanoma and renal cell cancer. Hypophysitis is one of the more common adverse events, usually presenting with headache, pituitary enlargement and hypopituitarism, mostly ACTH deficiency, which is usually permanent. We describe a series of 3 cases developing pituitary enlargement in keeping with hypophysitis after ipilimumab without any long-term pituitary hormone deficiencies. This illustrates that a comprehensive endocrine assessment is required even when pituitary enlargement is present.

A patient with diabetes mellitus and hypopituitarism developed the Somogyi effect that was characterized by insulin-induced hypoglycemia and rebound insulin-resistant hypoglycemia. Sarcoidosis is a granulomatous disease of unknown etiology that can affect all organs and systems, including the central nervous system in approximately 5% of patients. Although the hypothalamo-hypophyseal system may be one of the areas affected by neurosarcoidosis, pseudotumoral lesions are very rare.
