Cronobacter sakazakii bacteremia in a heart transplant patient with polycystic kidney disease.

Infections with Cronobacter sakazakii are mainly described among neonates and infants, with contaminated powdered infant formulas most often incriminated as the cause. We describe here a case of C. sakazakii bacteremia secondary to a suspected cyst infection in a heart-and-kidney transplant patient with polycystic kidney disease.

Vitamin D and bone minerals in neonates.

Osteopenia and rickets remain a problem for high-risk infants, especially preterm infants <1500 g birthweight (very low birth weight, VLBW). The primary cause of osteopenia in VLBW infants is a low intake of calcium and phosphorus compared to requirements for the rapidly growing skeleton. Human milk is a relatively low mineral containing substance and cannot meet the bone mineral needs of very low birth weight infants. As such, most VLBW infants need additional bone minerals and in many neonatal care units these are provided as part of human milk fortificants or specialized infant formulas. In…

Exclusive breastfeeding protects against bacterial colonization and day care exposure to otitis media.

OBJECTIVE: We followed a cohort (N = 306) of infants at well-baby visits in two suburban pediatric practices to assess the relation of exclusive breastfeeding, and other environmental exposures, to episodes of acute otitis media (AOM) and otitis media with effusion (OME). METHODS: Detailed prospective information about the exclusiveness of breastfeeding, parental smoking, day care attendance, and family history was obtained at scheduled clinic visits. Tympanometric and otoscopic examinations were used in the diagnosis of otitis media (OM). Nasopharyngeal cultures were performed at 1-6 months,

[Breast feeding of infants with phenylketonuria].

Five infants with phenylketonuria who received their phenylalanine primarily from breast milk were compared with five other infants with PKU who received infant formula as their primary phenylalanine source. No significant differences between both groups were observed for weight gain, the daily phenylalanine intake, and mean plasma phenylalanine concentrations. This study shows that breast feeding can be continued in young infants with their difficult to predict weight gain and daily changing phenylalanine requirements.

[Unusual case of food protein-induced enterocolitis syndrome].

Food protein-induced enterocolitis syndrome is a nonimmunoglobulin E mediated gastrointestinal food hypersensitivity that manifests as profuse, repetitive vomiting, pallor and hypotonia, often with diarrhea leading to severe dehydration and lethargy (sepsis-like symptoms) in the acute setting. An oral food challenge is sometimes performed to confirm the diagnosis and treatment consists of elimination of the food trigger(s) from the diet. We report a case of a 3-monthsold infant with several episodes of food protein-induced enterocolitis syndrome after taking infant formula milk with tolerance …

Calcium and phosphorus supplementation after initial hospital discharge in breast-fed infants of less than 1800 grams birth weight.

This study evaluated whether calcium and phosphorus supplementation after initial hospital discharge was advisable in infants of < 1800 gm birth weight who were being breast fed. Twenty-seven infants (15 without any illness affecting nutritional intake and 12 with medical illness) received breast milk plus a liquid human milk fortifier mixed 1:1 and 400 IU vitamin D daily during initial hospitalization. At discharge, 12 infants (6 without and 6 with previous illness) were randomly assigned to receive calcium and phosphorus supplementation, and 15 infants (9 without illness and 6 with previous

Breastfeeding is not a risk factor for mother-to-child transmission of hepatitis B virus.

BACKGROUND: Many clinicians do not encourage breastfeeding in hepatitis B virus (HBV) carriers, since HBV DNA can be detected in breast milk and breast lesions may increase exposure of infants to HBV. The aim of this study was to determine whether breastfeeding may add risk for perinatal HBV transmission. METHODOLOGY/PRINCIPAL FINDINGS: Totally 546 children (1-7-year-old) of 544 HBV-infected mothers were investigated, with 397 breastfed and 149 formula-fed; 137 were born to HBeAg-positive mothers. All children had been vaccinated against hepatitis B but only 53.3% received hepatitis B immune

Profiling of microbiota in liquid baby formula consumed with an artificial nipple.

It is suspected that oral bacteria are transferred to the liquid baby formula through the artificial nipple and multiply in the bottle after feeding. In the present study, in order to understand the influence of bacteria on liquid baby formula after feeding, the transfer of oral bacteria through artificial nipples and their survival in liquid baby formula were examined immediately after drinking as well as after storage at 4°C for 3 h. Four healthy human subjects (20-23 years old) were asked to drink liquid baby formula (Aptamil®, ca. 50 mL) from baby bottles using artificial nipples. Samples

Effects of a dietary and environmental prevention programme on the incidence of allergic symptoms in high atopic risk infants: three years' follow-up.

A prospective case-control study is presented to assess an allergy prevention programme in children up to 36 months of age. Infants born at three maternity hospitals were followed from birth: 279 infants with high atopic risk (intervention group) were compared with 80 infants with similar atopic risk but no intervention (non-intervention group). The intervention programme included dietary measures (exclusive and prolonged milk feeding diet followed by a hypoantigenic weaning diet) and environmental measures (avoidance of parental smoking in the presence of the babies, day care > 2 years of lif

Feeding patterns and the risk of perianal abscess in early infancy: a case-control study.

BACKGROUND: Breastfeeding is widely recognized as protective against infectious morbidity in infancy, largely through immunologic, anti-inflammatory, and microbiome-mediated mechanisms. Whether exclusive breastfeeding also influences the risk of perianal abscess, a relatively common condition in early infancy, remains unclear. This study aimed to evaluate the association between infant feeding patterns and the development of perianal abscess. METHODS: This retrospective case–control study with age-frequency matching was conducted at a tertiary pediatric surgery center and included male infants

Enteral feeding practices among infants with gastroschisis in Canada: A national cohort study.

PURPOSE: To describe enteral feeding practices and clinical outcomes among infants with gastroschisis at neonatal intensive care units (NICUs) in Canada. METHODS: Infants with gastroschisis were identified using data from the Canadian Neonatal Network and Canadian Pediatric Surgery Network. These included patients with (1) mild or no matting, (2) severe matting, and (3) complex gastroschisis (i.e., presence of intestinal necrosis, perforation, and/or atresia). We explored patterns of enteral feeding and associated clinical outcomes during the NICU admission. RESULTS: We identified 348 infant

Minimal Enteral Nutrition to Improve Adaptation After Intestinal Resection in Piglets and Infants.

BACKGROUND: Minimal enteral nutrition (MEN) may induce a diet-dependent stimulation of gut adaptation following intestinal resection. Bovine colostrum is rich in growth factors, and we hypothesized that MEN with colostrum would stimulate intestinal adaptation, compared with formula, and would be well tolerated in patients with short bowel syndrome. METHODS: In experiment 1, 3-day-old piglets with 50% distal small intestinal resection were fed parenteral nutrition (PN, n = 10) or PN plus MEN given as either colostrum (PN-COL, n = 5) or formula (PN-FORM, n = 9) for 7 days. Outbreak of necrotizing enterocolitis associated with Enterobacter sakazakii in powdered milk formula.

We describe an outbreak of necrotizing enterocolitis (NEC) that occurred in the neonatal intensive care unit of our hospital. A total of 12 neonates developed NEC in June-July 1998. For two of them, twin brothers, the NEC turned out to be fatal. Enterobacter sakazakii, a known contaminant of powdered milk formula, was isolated from a stomach aspirate, anal swab, and/or blood sample for 6 of the 12 neonates. A review of feeding procedures revealed that 10 of the 12 patients were fed orally with the same brand of powdered milk formula. E. sakazakii was isolated from the implicated prepared formu

Development of a consistent body mass index.

A new body mass index, termed Consistent Body Mass Index CBMI=[Formula: see text], is developed from first principles. Customarily used Body Mass Index (BMI[Formula: see text]) or its variant Tri-ponderal Mass Index (TMI[Formula: see text]) or other measures serving to the purpose of classifying the anthropometric status of a person have repeatedly been demonstrated to be unsatisfactory in various aspects. A new index inclusive of all ages, body sizes, and sexes is therefore needed. A non-dimensional hence scalable index CBMI, which correlates well with waist-to-height ratio, [Formula: see tex

American Academy of Pediatrics recommendations on the effects of early nutritional interventions on the development of atopic disease.

PURPOSE OF REVIEW: To review the American Academy of Pediatrics statement on the effects of early nutritional interventions on the development of atopic disease in infants and children. RECENT FINDINGS: Recent findings suggest that restriction of maternal diet during pregnancy and lactation does not play a major role in the development of allergic disease. In high-risk infants, exclusive breastfeeding for at least 4 months prevents or delays atopic dermatitis, cow milk allergy, and wheezing early in life. There is evidence that supplementing breastfeeding with a hydrolyzed formula protects ag

A Rare Case of Congenital Glucose Galactose Malabsorption Due to SLC5A1 Mutation.

Protracted neonatal diarrhea is severe and potentially life-threatening if not promptly diagnosed and treated. Causes include congenital defects in sodium, chloride, glucose/galactose, bile acid transport, enterokinase deficiency, and villous atrophy. Glucose-galactose malabsorption (GGM) is a rare autosomal recessive disorder caused by mutations in the SGLT1 gene, impairing glucose and galactose absorption and leading to osmotic diarrhea. This case report highlights the rarity of GGM and emphasizes the importance of early recognition to enable prompt dietary intervention, preventing failure t

The Effect of Additives for Reflux or Dysphagia Management on Osmolality in Ready-to-Feed Preterm Formula: Practice Implications.

BACKGROUND: A common osmolality threshold for feedings is to stay <450 mOsm/kg for normal infants. Preterm formulas are frequently modified to improve growth, modify nutrition, and manage gastroesophageal reflux (GER) or dysphagia. Relationships between osmolality and additives to ready-to-feed preterm formulas are unclear. Our aims were to evaluate and compare the effects of caloric density, thickening agent recipes, and supplements to ready-to-feed preterm formula on osmolality. METHODS: A freezing point osmometer was used to measure the osmolality of 47 preterm infant formula combinations …
