Validation of an Operational Definition to Identify Distal Radius Fractures in a National Health Insurance Database.

PURPOSE: To develop and validate identification criteria for distal radius fractures (DRFs) and their treatment using a national health insurance database. METHODS: Patients who had at least 1 wrist radiograph taken in 2018 were recruited from a single academic referral hospital. After excluding patients who lacked immobilization code for wrist pathology, we collected data on the overall population. Because some patients might have undergone wrist radiography at another institution or had DRFs without an immobilization code, we additionally included patients who had a DRF diagnosis code at ou

Do Racial/Ethnic and Economic Factors Affect the Rate of Complicated Appendicitis in Children?

INTRODUCTION: Appendicitis continues to be one of the most common surgical conditions in the pediatric population. We set out to determine demographic and practice variations among children admitted with appendicitis and highlight the racial/ethnic and healthcare access role in relation to the rate of complicated appendicitis using the 2012 Kids' Inpatient Database (KID). Methodology. A retrospective cross-sectional database study was performed using the 2012 KID. All children (age 1 months to 20 years) with appendicitis were identified using the ICD-9 diagnosis codes. Children with a diagnosi

Impact of weekend treatment on short-term and long-term survival after urgent repair of ruptured aortic aneurysms in Germany.

OBJECTIVE: There is some evidence that weekend admission to the hospital is associated with worse outcomes compared with weekday admission. However, only a few studies have focused on weekend vs weekday surgery outcomes. This study aimed to determine whether there is a weekend effect on outcomes in the treatment of ruptured aortic aneurysms in Germany. METHODS: Health insurance claims of Germany's third largest insurance provider, DAK-Gesundheit, were used to investigate short-term and long-term mortality after weekend vs weekday treatment of ruptured aortic aneurysm. Patients undergoing endo

Treatment of early-stage oral cavity squamous cell carcinoma at low-volume facilities impacts surgical margins and survival: an analysis of 22,258 patients from the NCDB.

BACKGROUND: Positive surgical margins (PSMs) persist in many patients, even in early-stage oral cavity squamous cell carcinoma (ES-OCSCC), impacting recurrence rates and overall survival (OS). While high-volume facilities (HVFs) enhance surgical outcomes, their survival benefit in ES-OCSCC has not been assessed in an updated large cohort. The role of social determinants of health (SDOH) in predicting survival in this context is not well described. METHODS: This retrospective cohort study analyzed stage I and II OCSCC patients from the National Cancer Database between 2004 and 2020. Facilities

Does patient education level affect 90-day outcomes and disposition following total joint arthroplasty?

BACKGROUND: Social determinants of health influence patient outcomes. In total joint arthroplasty (TJA), factors such as race, marital status, insurance type, and socioeconomic status affect outcomes and discharge disposition. One underexplored factor is patient education level. Although census data associate higher education with improved health outcomes, its impact following TJA has not been examined. This study evaluated differences in outcomes and discharge disposition between patients with higher versus lower education levels after TJA. METHODS: A retrospective review within an integrate

The Influence of Patient Characteristics and Social Determinants of Health on Postoperative Complications Following Achilles Tendon Rupture.

BACKGROUND: The influence of social determinants of health (SDH) on postoperative complications has been investigated in several studies, although correlation with Achilles tendon rupture (ATR) repair remains uninvestigated. SDH encompasses several factors, including insurance status and area-based measurements, including the Area Deprivation Index (ADI) and Social Vulnerability Index (SVI), which ranks neighborhoods by social disadvantage. This study investigated the correlation between patient demographics, SDH, and complications following ATR repair. METHODS: A retrospective cohort study w

Early complications after esophageal atresia repair: analysis of a German health insurance database covering a population of 8 million.

The treatment of esophageal atresia is not centralized in Germany. Therefore, high numbers of departments are involved. Data on the results of esophageal atresia repair from Germany are lacking. The aim of this study was to evaluate the early postoperative results after repair of esophageal atresia based on unbiased data of a German health insurance. We aimed to determine whether characteristics of the departments had an impact on outcome and compared the results from this study with the literature data from centers with a high caseload. Data of a German health insurance covering ∼10% of the p

Factors Affecting Length of Stay, Readmission, and Revision After Shoulder Arthroplasty: A Population-Based Study.

BACKGROUND: Increased length of hospital stay, hospital readmission, and revision surgery are adverse outcomes that increase the cost of elective orthopaedic procedures, such as shoulder arthroplasty. Awareness of the factors related to these adverse outcomes will help surgeons and medical centers design strategies for minimizing their occurrence and for managing their associated costs. METHODS: We analyzed data from the New York Statewide Planning and Research Cooperative System on 17,311 primary shoulder arthroplasties performed from 1998 to 2011 to identify factors associated with extended
