[Intestinal desinvagination in children].

168 cases of conservative and 52 cases of surgical desinvagination are reported. The push-like insuflation of oxygen (under the pressure of 130 mm Hg) in large intestine after gaining the pressure of 20-30 mm Hg was preformed in case of conservative desinvagination. The electrometric control and management of the procedure has been performed. The following 3 parameters of biological beed-back were regarded: the acoustic noise, the fall of oxygen pressure in the rectum and changes of the abdomen volume. The method was effective in 91% of cases.

[Carcinoids of the large intestine].

An analysis of 25 patients with carcinoids of the colon (1,94% of the amount of patients with cancer of the colon) was made. In the recent decade this disease has become 2 times more frequent. Sixteen patients had carcinoids of the rectum, 9 patients had carcinoids of the right half of the colon. Malignant carcinoids were diagnosed in 16 cases, 8 of them being located in the right half. Radical operations are recommended for all carcinoids of the right half of the colon and for tumors of the rectum if more than 2 cm.

Intestinal obstruction in Khartoum.

The pattern of intestinal obstruction at Khartoum Teaching Hospital was reviewed in this study which included 239 patients. 170 of them were males and 68 were females. Their ages ranged from two days to 95 years (mean 31.4 +/- 5.3 years). The commonest causes of intestinal obstruction were strangulated external hernias (27.7%), intestinal adhesions (21%), intussusception (12%) and sigmoid volvulus (11%). Less frequent causes were paralytic ileus, large bowel tumours, peritoneal bands and Hirschsprung's disease. Of the strangulated hernias, inguinal hernia (70%) was the most frequent type of he

Intestinal bypass of the esophagus.

Esophageal replacement by a segment of isoperistaltic ileum with cecum or by transverse or left colon will allow near-normal swallowing for many years. The authors reviewed the course of 59 children who had bypass of their entire esophagus and of four whose distal esophagus was resected and replaced. The follow-up period ranges from 1 to 37 years; in 36 cases, it exceeds 5 years. Thirty children had caustic strictures and 25 had either isolated esophageal atresia or atresia with fistula. Two children with esophageal injury caused by foreign body ingestion and two with congenital strictures als

[Diverticulosis of the small intestine].

Small bowel diverticulosis is a rare disease of gastrointestinal tract that occurs most frequently in older patients. Since this disease can be asymptomatic, for a long time, the diagnosis rarely is made in the preoperative period. In 40% of cases there are acute or chronic complications, most frequently diverticulitis either with or without perforation, bowel obstruction and massive haemorrhage clinically manifested as bleeding from the lower part of the gastrointestinal tract. Abdominal pain, pseudoobstructive and malabsorption syndrome, expressive weight loss, steatorrhea and anemia are com

Neuronal intestinal dysplasia.

Neuronal intestinal dysplasia (NID) is one of the disorders of gastrointestinal motility with defined pathological changes. Although first described more than 20 years ago, the disease remains controversial. The histological findings do not correlate with the degree of symptoms. The etiology of NID is obscure, but an immunologic process has been suggested. The diagnosis can be made on biopsy specimens performed to identify Hirschsprung's disease, but treatment must be individualized. The clinical course is variable, with spontaneous improvement in some, relentless constipation or severe dysmot

[Vesico-intestinal fistulas].

The authors report the various locations (ileal, appendicular, colic, rectal and through Meckel diverticulum) of the fistulas between the bladder and the digestive tract and specify the mainly inflammatory etiologies and specifically those due to the Crohn disease or those caused by tumors aggravated by irradiation and repeated surgery.

Intestinal permeability: an overview.

The noninvasive assessment of intestinal permeability in humans has a 20-year history. Because the tests are increasingly used in clinical practice and research and because there is much controversy, we reviewed the literature and outlined the potential and possible shortcomings of these procedures. Data was obtained from personal files and from a systemic search through MEDLINE and EMBASE. The principle of the differential urinary excretion of orally administered test markers is explained with reference to the desired physicochemical properties of the markers and how the principle can be expl

[Sarcoidosis of the large intestine].

A 68-years old female patient suffering over a period of several weeks from fatigue, weight loss, recurrent diarrheas and mild fever was admitted in our clinic. A few months ago, sarcoidosis of a cervical lymph node had been diagnosed and had been treated with steroids. During the present admission, the extensive study of the gastrointestinal system revealed a tumor of the ascending colon, obstructing the lumen. The patient was subjected to laparotomy due to persistent symptomatology of intestinal obstruction. A right hemicolectomy was performed. The pathological findings were persistent with

[Nosography of intestinal lymphomas].

Lymphomas can be localized to the gastrointestinal tract and can be primitive or secondary to a systemic lymphoma. Lymphomas can be classified as Hodgkin's and non-Hodgkin's. The latter include IPSID (all of the B-cell) and non-IPSID lymphomas. The involvement of the small intestine in its entire length is a feature of IPSID lymphomas. Non-IPSID lymphomas include MALT-type, which is a B-cell lymphoma, and EATCL, a T-cell lymphoma which is a not infrequent complication of coeliac disease. There are several classifications of lymphomas: the one by Isaacson is the most widely used at present.

[Intestinal pseudo-obstruction].

Intestinal pseudo-obstruction is a rare and heterogeneous syndrome caused by severe disorders of gastrointestinal motility. It affects the entire gastrointestinal tract or only segments, giving rise to symptoms and physical signs of a mechanical obstruction of the gut despite negative results of all imaging procedures. The disease may occur in an acute or chronic form. The acute and some of the chronic forms develop as complication of other gastrointestinal or extragastrointestinal diseases, e.g. pancreatitis or systemic sclerosis. The primary forms of chronic intestinal pseudo-obstruction are

Intestinal obstruction and bezoars.

BACKGROUND: Bezoars are large conglomerates of vegetable fibers, hairs, or concretions of various substances located in the stomach or small intestine of humans and certain animals, mainly ruminants. Gastrointestinal bezoars have constituted a relatively common clinical reality ever since the introduction of truncal vagotomy associated with drainage or gastric resection in the treatment of gastroduodenal peptic ulcer. STUDY DESIGN: This study presents a series of 87 cases of intestinal bezoar treated in our department of general surgery. Analysis was made of data obtained retrospectively from

An intestinal natriuretic factor.

In 1975, reports were published that suggested that the gastrointestinal tract can "taste" the intake of sodium and in some unknown way influence the kidneys to increase sodium excretion. To test whether the intestine contained a natriuretic factor, intestinal tissue from cats was homogenized and fractionated by ultrafiltration to a molecular range of approximately 500-10,000 Da and separated by gel chromatography (Sephadex G25). The fractions were pooled into four large fractions that were assayed for "natriuretic" activity on anesthetized rats. The fraction containing the material with an ap
