Predicting Success: The Crucial Role of Preoperative Data in Refractive Surgery Outcomes.

Refractive surgery, which includes techniques such as Laser-Assisted In Situ Keratomileusis (LASIK), Photorefractive Keratectomy (PRK) and Small Incision Lenticule Extraction (SMILE), has revolutionized ophthalmology by offering advanced solutions for vision correction. However, the choice of the technique to be used in the individual patient is highly dependent on a thorough preoperative evaluation. This retrospective study aims to investigate how preoperative parameters, including corneal thickness, topography, and refraction, affect long-term post-operative clinical outcomes. Through a syst…

Advanced Surface Ablation in a Patient With Suspect Topography: A Case Report.

The purpose of this clinical report is to describe a 10-year clinical outcome of advanced surface ablation with photorefractive keratectomy (PRK) in a patient who had been previously incorrectly diagnosed with keratoconus (KC). Corneal ectasia is a rare but extremely relevant complication of laser vision correction, and KC represents a major contraindication for these procedures. Nonetheless, some surface ablation procedures, such as PRK, might be a valid option for particular patients with atypical corneal topography or subclinical or mild forms of KC. Patient education and complete preoperat…

Successful Medical Management of Pigmented Fungal Keratitis Following Vegetative Trauma.

Fungal keratitis remains a frequent cause of corneal morbidity in tropical regions, particularly after trivial trauma involving plant materi … and a brownish plaque. Direct microscopy confirmed septate pigmented hyphae. Following superficial keratectomy and intensive topical antifungal therapy, the infection resolved without any need for keratoplasty. This case illustrates that early suspicion and timely medical management can successfully control selected cases of dematiaceous fungal keratitis.

Implication of Corneal Refractive Surgery in Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy (DMD) is an X-linked disorder due to a dystrophin mutation and is the leading cause of muscular dystrophy. DMD presents with characteristic systemic effects, including severe muscular atrophy, cardiomyopathy, and ocular manifestations. Performing corneal refractive surgeries in patients with DMD raises concerns regarding patient positioning, risk of cataracts, and other comorbid conditions. Published reports of photorefractive keratectomy, laser-assisted in situ keratomileuses, and small incision lenticule extraction are lacking in this population. Here, we discuss …

The use of porcine small intestinal submucosa in ten cases of feline corneal disease.

Porcine small intestinal submucosa (SIS) was used as a novel graft material in the management of 10 cases of feline corneal disease. Five cases had stromal ulceration associated with trauma, ocular foreign body and/or suspected infection and required a grafting procedure. Five cases had feline sequestra that were managed by a keratectomy prior to placement of SIS as a graft material. Eight eyes healed with minimal corneal scarring with a very good cosmetic and visual result. One eye with continued aqueous leakage in the immediate postoperative period required a conjunctival pedicle graft to re…

Detection of bovine papillomavirus type 2 DNA in calf conjunctival myofibroblastoma.

An 8-month-old male Japanese Black calf was referred for the evaluation of a slow-growing conjunctival mass in the right eye. A superficial keratectomy was performed followed by recurrence on two occasions. No metastases were found in surrounding tissues. Histological, immunohistochemical, and ultrastructural investigation revealed that both the primary and the recurrent lesions were benign, conjunctival, myofibroblastomas. Interestingly, bovine papillomavirus type 2 (BPV-2) DNA was detected in both myofibroblastoma lesions. Archival bovine myofibroblastomas from the vulva and neck were also a…

Corneal topographic and pachymetric screening of keratorefractive patients.

PURPOSE: To review the incidence of corneal abnormalities detected in the preoperative examination, using videokeratography and pachymetry that excluded patients from laser in situ keratomileusis (LASIK) or photorefractive keratectomy (PRK). METHODS: We conducted a chart review of consecutive patients who had a preoperative examination and were excluded from refractive surgery. RESULTS: Eighteen patients from 1,392 refractive candidates (1.3%) were designated as poor candidates for LASIK or PRK based on corneal topography and/or pachymetry. Thirteen patients (0.9%) were identified as having …

Iatrogenic keratectasia after laser in situ keratomileusis for less than -4.0 to -7.0 diopters of myopia.

PURPOSE: To study 13 eyes with less than -4.0 to -7.0 diopters of myopia that developed central or inferior keratectasia after laser in situ keratomileusis (LASIK). METHODS: The progress of the ectasia was determined by a series of pre- and post-LASIK topographies and the case histories. RESULTS: Progressive ectasia developed from 1 week to 27 months after LASIK. This necessitated the wearing of hard contact lenses or penetrating keratoplasty. One eye improved for some months after photorefractive keratectomy retreatment. Two eyes in 1 patient and 1 eye in a second patient had latent keratoc…

Clinicopathologic study of satellite lesions in nontuberculous mycobacterial keratitis.

Multifocal stromal infiltrates or "satellite lesions" have been considered a characteristic feature of fungal keratitis. We examined two patients with nontuberculous mycobacterial keratitis who clinically presented with satellite lesions. The keratitis consisted of multifocal stromal infiltrates with indistinct white and fluffy margins. Both patients received topical fortified amikacin therapy with poor response. Lamellar keratectomy or penetrating keratoplasty was performed, respectively, in the two patients because of progressive stromal thinning and enlarging satellite lesions. Histopatholo…

A Comparison of LASIK Versus PRK Enhancement Outcomes in Eye With Prior Cataract Surgery.

INTRODUCTION: Cataract surgery continues to evolve with increasing levels of precision through the development and introduction of new technologies. Despite these advances, unexpected visual outcomes attributable to residual refractive error still occur. Additional "enhancement" procedures to the cornea can treat this residual refractive error to achieve a satisfactory outcome. Two frequently employed excimer laser enhancement procedures include laser in-situ keratomileusis (LASIK) and photorefractive keratectomy (PRK). While both of these procedures are widely accepted as safe and effective i…
