Association between traumatic bone marrow abnormalities of the knee, the trauma mechanism and associated soft-tissue knee injuries.

OBJECTIVES: To determine the association between traumatic bone marrow abnormalities, the knee injury mechanism, and associated soft tissue injuries in a larger cohort than those in the published literature. METHOD: Retrospective study including 220 patients with traumatic knee injuries. Knee MRIs were evaluated for trauma mechanism, soft tissue injury, and the location of bone marrow abnormalities. The locations of the abnormalities were correlated with trauma mechanisms and soft tissue injuries using the chi-square test with Bonferroni correction. RESULTS: One hundred and forty-four valgus

ACR Appropriateness Criteria Acute Trauma to the Knee.

More than 500,000 visits to the emergency room occur annually in the United States, for acute knee trauma. Many of these are twisting injuri … ted meniscus or ligament tear, or the injuries from a reduced patellar dislocation. Patients with a knee dislocation should undergo radiographs and an MRI, as well as an angiographic study such as a fluoroscopic, CT, or MR angiogram. The ACR Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed every three years …

Effect of Vascular Injury on Functional Outcome in Knees with Multi-Ligament Injury: A Matched-Cohort Analysis.

BACKGROUND: Multi-ligament knee injury (MLKI) associated with knee dislocation can result in vascular injury. The purpose of this study was to compare knee function after MLKI between patients with a vascular injury requiring popliteal artery bypass grafting and patients without vascular involvement. Additionally, factors associated with poor knee function in patients who had MLKI with vascular injury were evaluated. METHODS: This retrospective study identified patients with an MLKI between 1992 and 2014. Each patient who had a concomitant vascular injury requiring bypass grafting (vascular c…

Prognostic Factors of First-Time Athletic Patellar Dislocation That Favor Surgical Intervention: A Systematic Review.

First-time athletic patellar dislocations are either treated surgically with medial patellofemoral ligament (MPFL) reconstruction or conservatively with bracing and physical therapy. Currently, there is no consensus on the best treatment for this condition. While outcomes are similar between treatment groups, operative treatment has been shown to yield lower redislocation rates and higher Kujala scores. Therefore, the aim of this study was to identify prognostic factors that favor operative treatment of first-time patellar dislocations. Data sources include PubMed, Cochrane, and Embase. PRISMA

Presumed Larsen syndrome in a child: a case with a 12-year follow-up.

Larsen syndrome (OMIM 150250) was first described in 1950 as an entity characterized by distinct facial features and dislocations of the multiple large joint, and cleft palate, hearing loss, and spinal abnormalities were occasionally observed. The prevalence of Larsen syndrome is estimated to be one in 100,000 live births . Management of multiple large-joint dislocations often proves difficult with a tendency toward recurrence, particularly if a patient has complete dislocation of the knee .We treated a boy with the clinical phenotype of Larsen syndrome using 10 orthopedic procedures, but fail

Limb length change after total knee arthroplasty for valgus deformity does not affect the clinical score and is not affected by fixed flexion deformity.

BACKGROUND: The limb length change (LLC) after total knee arthroplasty (TKA) is especially significant in valgus deformity. The higher LLC could cause higher incidences of lower limb length discrepancy (LLD) and low clinical score. However, studies about LLC after TKA for valgus deformity are limited, and there are none on the relationship between LLC and fixed flexion deformity (FFD) in valgus deformity. HYPOTHESIS: (1) The amount of LLC would affect the postoperative LLD, (2) the improvement of fixed flexion deformity (FFD) would affect the amount of LLC, (3) The amount of LLC would affect

Early or delayed reconstruction in multi-ligament knee injuries: A systematic review and meta-analysis.

BACKGROUND: Whether early or delayed surgical intervention in patients with multi-ligament injuries results in better outcomes, is of current and controversial debate. PURPOSE: The purpose of this study was to perform a meta-analysis comparing early versus late surgical treatment of multi-ligament knee injuries. METHODS: We performed a systematic review of Medline, Embase, Scopus, and Google Scholar to identify relevant studies in the English and German literature. Eligibility criteria included studies comparing early or delayed surgical interventions for multi-ligament knee injuries, with a

Quadriceps pull test: an outcome predictor for lateral retinacular release in recurrent patellar dislocation.

Recurrent dislocation of the patella is a disorder with a complex aetiology. Several surgical procedures have been recommended for its correction, but there are few clinical or radiological guidelines to facilitate the selection of a particular procedure. The results of treating a series of 29 knees are presented with a follow-up of at least 2 years. A significant correlation has been found between the horizontal movement of the centre of the patella on quadriceps contraction (quadriceps pull test) and surgical outcome when lateral release alone is selected for the correction of recurrent pate

The percutaneous lateral retinacular release.

A lateral retinacular release performed through a 2 ½-3 cm lateral parapatella incision can give rewarding results for patients with chondromalacia, degenerative joint disease, recurrent subluxation or recurrent dislocation of the patella. A retrospective review from August 1978 to December 1980 of 37 patients (42 knees) shows 83% excellent or satisfactory results, 17% unsatisfactory results with an average postoperative follow-up of 12.3 months. Preoperative pain was reduced in all but two patients; no patient was made worse by the procedure with respect to pain or function. In 17 knees with

Diagnosis of patellofemoral malalignment by computed tomography.

Conventional radiographic assessment of the patellofemoral joint (PFJ) is often unsatisfactory for a variety of technical and theoretical reasons. Using a simple device designed to allow easy and reproducible control of knee flexion, we used computed tomography (CT) to evaluate the PFJ in 10 normal volunteers and five patients with a history suggestive of recurrent subluxation or dislocation of the PFJ. At full extension of the knee the patients with a history of malalignment showed an abnormal patellar centralization and tilt compared with those of the normal volunteers. The abnormalities wer

[The measurement of the tibial tuberosity. Patella groove distanced technique and results (author's transl)].

The line between the summit of the tibial tuberosity and the bottom of the trochlea groove as seen on a radiograph taken with the knee in 30 degrees of flexion and neutral rotation is indicative of the valgus position of the quadriceps mechanism at the moment of engatement of the patella in the groove. The authors stress the technical requirements to obtain the radiograph which permits this measurement. Measurement of a group of normal knees and of two groups of knees presenting with known patello-femoral pathology (lateral patello-femoral arthrosis and current dislocation of the patella) are

Acute instability of the patella: is magnetic resonance imaging mandatory?

PURPOSE: Acute dislocations of the patella represent 2 to 3% of traumatic injuries of the knee. When patients are seen in the emergency department with a dislocated patella clinical diagnosis is often very evident. However, in cases of short-lived subluxation or dislocations that have reduced spontaneously, the diagnosis can be challenging. The aim of this study was to evaluate the utility of MRI in the evaluation of acute patellar instability. METHODS: This was a prospective study of 39 cases of acute patellar instability. The study protocol included a clinical examination by an emergency me

Congenital patellar dislocation with genu valgum and Klinefelter syndrome: a case report.

BACKGROUND: Congenital patellar dislocation (CPD) is a complex disease lacking consensus on the treatment. While genu valgum is a risk factor for patellar dislocation in general, it is less common in the congenital cases. In this report, a retrospective analysis of a child with bilateral CPD and genu valgum is presented, detailing the clinical characteristics, diagnostic approach, surgical intervention, and outcomes. CASE DESCRIPTION: A 3-year-old boy presented with an abnormal gait and Klinefelter syndrome. Comprehensive preoperative imaging evaluations were made to assess osseous and soft t

Popliteal Artery Stent Thrombosis After Multiligamentous Knee Injury Reconstruction: A Case Report.

CASE: A 44-year-old man sustained a thrombosed popliteal artery after a closed left knee dislocation after a high-energy motor vehicle collision. He underwent emergency open popliteal artery embolectomy, with insertion of endovascular stent. After arthroscopic multiligamentous knee injury reconstruction (MLKI-R) on day 18 after injury, he developed stent thrombosis, which resolved with urgent percutaneous thrombectomy. CONCLUSION: This is the first report of popliteal artery stent thrombosis after MLKI-R. Knee surgery in a revascularized limb has remained controversial, but studies are scarce…
