Obstetric brachial plexus injury: risk factors related to recovery.

OBJECTIVE: To investigate if multivariate risk calculation can discriminate those infants who do not recover after an obstetric brachial plexus injury (OBPI). STUDY DESIGN: All liveborn infants without lethal congenital abnormalities from 1988 through 1996 with a gestational age > or =30 weeks were included. Outcome variables were all OBPI and non-recovered OBPI. Risk calculation was performed by univariate analysis for all infants and by multivariate logistic analysis for all singleton infants delivered vaginally in cephalic presentation. RESULTS: A total of 62 of 13 366 liveborn infants su

Differences in perineal lacerations in black and white primiparas.

OBJECTIVE: To test the null hypothesis that there are no differences in incidence of perineal and vaginal lacerations in primiparous black and white women. METHODS: We reviewed University of Michigan Hospital delivery records, from July 1996 to December 1998, of black and white women 18 years and older and at least 35 weeks' gestation who had their first vaginal delivery. Birth weight, episiotomy, gestational age, laceration, length of second stage, oxytocin use, epidural use, and operative vaginal delivery were analyzed by univariable and multivariable tests. RESULTS: We analyzed 176 black

Retrospective Case-Control Study of Extended Birth Perineal Tears and Risk Factors.

BACKGROUND: A perineal tear is a rupture of the skin or muscle between the vagina and anus (perineum). A third-degree tear is one type of extended perineal tear (EPT), and it involves the penetration of the anal sphincter muscle. Another type of EPT is a fourth-degree laceration, which penetrates deeper into the lining of the anus or rectum. The stretching of the perineum during childbirth may result in perineal trauma. Invasive surgical interventions are required for the treatment of EPTs. For this reason, the reduction of their incidence can be achieved by fully comprehending the risk aspect

A randomized controlled trial comparing oxytocin administration before and after placental delivery in the prevention of postpartum hemorrhage.

OBJECTIVE: To determine if the timing of the administration of prophylactic oxytocin influences the incidence of postpartum hemorrhage caused by uterine atony, retained placenta, and third-stage duration. STUDY DESIGN: Parturients who presented for vaginal delivery were randomized in a double-blinded fashion to receive oxytocin, 20 units in a 500-mL crystalloid intravenous bolus, beginning upon delivery of either the fetal anterior shoulder or placenta. For all patients, the third stage of labor was managed with controlled cord traction until placental expulsion, followed by at least 15 secon

Prevalence of episiotomy and associated factors among women who gave birth at public health facilities in Jigjiga town, eastern Ethiopia: A cross-sectional study.

INTRODUCTION: Maternal morbidity and mortality has remained a major public health concern worldwide. Basic emergency obstetric care is the primary intervention to prevent obstetric complications and maternal death. Episiotomy is one of the basic obstetrical procedures used to facilitate vaginal delivery, shorten the second stage of labor and prevent complications. However, there is a paucity of evidence on the prevalence and factors associated with episiotomy among women who gave birth in eastern Ethiopia. OBJECTIVE: This study aimed to determine the prevalence of episiotomy and its associate

Mode of delivery at term and adverse neonatal outcomes.

OBJECTIVE: To determine the relationship between mode of delivery and serious adverse neonatal outcomes in term, singleton, cephalic neonates. METHODS: A 10-year study of 64,555 term neonates reaching the second stage of labor in a single tertiary obstetric unit from 2000 to 2009. Multiple pregnancies, preterm deliveries (before 37 weeks of gestation), and lethal congenital anomalies were excluded. The primary outcome was the rate of peripartum death by mode of delivery. Secondary outcomes were rates of neonatal encephalopathy, intracranial hemorrhage-related mortality, and the relationship b

Delivery outcomes of large-for-gestational-age newborns stratified by the presence or absence of gestational diabetes mellitus.

OBJECTIVE: To evaluate separate and combined contributions of gestational diabetes mellitus (GDM) and large-for-gestational age (LGA) on delivery outcomes. METHODS: In a retrospective cohort study of term singleton deliveries between 2007 and 2014 in Tel Aviv, Israel, outcomes were compared between non-GDM/AGA pregnancies (reference) and three study groups: non-GDM/LGA, GDM/appropriate for gestational age (AGA) and GDM/LGA. RESULTS: Overall, there were 62 102 deliveries, of which 53 201 (85.7%) were eligible for inclusion. Of these, 43 775 (82.3%) were non-GDM/AGA, 6441 (12.1%) non-GDM/LGA,

Effects of World Health Organization labor care guide on maternal and neonatal outcomes: A systematic review and meta-analysis.

BACKGROUND: Arrest in the progress of labor is one of the important cases of early cesarean section, especially in primiparous mothers, but correct diagnosis of slow progress and measures at the right time based on evidence are the most important issues in the prevalence of cesarean section during vaginal delivery. OBJECTIVE: The present study investigated the impact of the World Health Organization labor care guide (WHO LCG) on maternal and neonatal outcomes. MATERIALS AND METHODS: A comprehensive literature search was performed using PubMed, Scopus, Embase, and Google Scholar up to Septemb

Risk factors of neonatal stroke from different origins: a systematic review and meta-analysis.

UNLABELLED: Given the persistent ambiguity regarding the etiology of neonatal stroke across diverse origins, our objective was to conduct a comprehensive evaluation of both qualitative and quantitative risk factors. An exhaustive search of eight databases was executed to amass all pertinent observational studies concerning risk factors for neonatal stroke from various origins. Subsequent to independent screening, data extraction, and bias assessment by two researchers, a meta-analysis was conducted utilizing RevMan and Stata software. Nineteen studies, encompassing a total of 30 factors, were

Impact of selected dimensions of pelvic floor structures on the process of delivery - a review of the literature.

Muscular and connective tissue structures of the pelvic floor play a crucial role throughout a woman's life, exerting significant influence on the course of childbirth. Injury to these soft tissues during delivery can substantially affect a woman's postpartum quality of life. In recent years, research has increasingly focused on evaluating pelvic floor structures prior to labor to determine whether functional or anatomical changes contribute to the labor process and its outcomes. Transperineal ultrasound has emerged as a promising tool for objectively assessing the preparedness of pelvic floor

Hands-on vs hands-off technique for the prevention of perineal injury: a randomized clinical trial.

BACKGROUND: Perineal trauma after vaginal delivery is common and is associated with severe morbidity, including incontinence, pelvic pain, and sexual dysfunction. Morbidity is mainly related to third- or fourthdegree lacerations. OBJECTIVE: This study aimed to test the hypothesis that in nulliparous women with singleton pregnancies at term, hands-off technique during spontaneous vaginal delivery could reduce the rate of perineal laceration. STUDY DESIGN: This was a parallel-group, nonblinded, randomized clinical trial. Nulliparous women in spontaneous labor with singleton pregnancies and ver

Modeling childbirth: elucidating the mechanisms of labor.

The process of childbirth and the mechanisms of labor have been studied for over a century, beginning with simple measurements of fetal skull and maternal pelvis dimensions. More recently, X-rays, ultrasound, and magnetic resonance imaging have been used to try and quantify the biomechanics of labor. With the development of computational technologies, biomechanical models have emerged as a quantitative analysis tool for modeling childbirth. These methods are well known for their capabilities to analyze function at the organ scale. This review provides an overview of the state-of-the-art finite
