Twin-to-twin transfusion syndrome: from pathophysiology to long-term outcome-a narrative review.

UNLABELLED: Twin-to-twin transfusion syndrome (TTTS) is a severe complication of monochorionic twin pregnancies caused by unbalanced placental vascular anastomoses. Fetoscopic laser photocoagulation (FLP) is the standard treatment, although neonatal outcomes remain variable. This review aimed to summarize immediate, short-term, and long-term neonatal outcomes following FLP for TTTS. A narrative review of published clinical trials, cohort studies, and meta-analyses was conducted, focusing on survival, neurological, cardiovascular, and renal outcomes in neonates following FLP, including comparis

Efficacy of Continuous-Wave Transscleral Cyclophotocoagulation Post-Pars Plana Vitrectomy in Glaucoma Patients: A Retrospective Study from Poland.

BACKGROUND Glaucoma, a vision-threatening condition, results from optic nerve damage and affects millions of people worldwide. Often asymptomatic, it is hereditary, with risk factors like hypertension, diabetes, and steroid use. Despite its link with intraocular pressure (IOP), not everyone with high IOP develops glaucoma. After pars plana vitrectomy (PPV), patients face increased IOP risks. Traditional treatment includes pharmacotherapy, and, when ineffective, surgical interventions. Continuous-wave transscleral cyclophotocoagulation (CW-TSCPC) is an alternative for refractory glaucoma but ca

Photocoagulation with GREENLIGHT© laser in radiation-induced haemorrhagic cystitis: Results of a single-center series.

INTRODUCTION: Radiation-induced haemorrhagic cystitis (RIHC) is one complication of the pelvic radiotherapy. The GREENLIGHT© laser (GL) has been barely studied in the treatment of radiation cystitis. The primary objective was to evaluate the efficacy of GL in refractory RIHC patients (RRC) in a single-centre series. MATERIALS AND METHODS: Twenty-nine patients were treated by GL bladder photocoagulation (GLBP). These patients showed signs of refractory haematuria in the context of RIHC. The primary endpoint was the absence of haematuria that would require a subsequent surgical intervention. Se

Potential Mechanisms of Intraocular Pressure Reduction by Micropulse Transscleral Cyclophotocoagulation in Rabbit Eyes.

PURPOSE: To evaluate the histological changes associated with, and the potential mechanisms of, intraocular pressure (IOP) reduction by micropulse cyclophotocoagulation (MP-CPC) in rabbit eyes. METHODS: MP-CPC was performed on the right eyes of Dutch belted rabbits, whereas the left eyes served as controls. The laser power settings were 250, 500, 750, 1000, 1500, and 2000 mW, 10 seconds per sweep, 100 seconds in total. IOP, outflow facility, and uveoscleral outflow tract imaging, using a fluorescent tracer, were examined at one week after MP-CPC. Changes of morphology and protein expressions

Avascular island in the vascularised retina in zone II ROP.

We describe an atypical form of avascular island demarcated by a ridge in the vascularised retina of retinopathy of prematurity. Fundus fluorescein angiography showed an unusual finding of an avascular island demarcated by shunt vessels in vascularised retina posterior to the ridge. There is a need to document this finding as this could throw some light on the aetiopathogenesis of the disease.

Segmentation of the placenta and its vascular tree in Doppler ultrasound for fetal surgery planning.

PURPOSE: Twin-to-twin transfusion syndrome (TTTS) is a serious condition that occurs in about 10-15% of monochorionic twin pregnancies. In most instances, the blood flow is unevenly distributed throughout the placenta anastomoses leading to the death of both fetuses if no surgical procedure is performed. Fetoscopic laser coagulation is the optimal therapy to considerably improve co-twin prognosis by clogging the abnormal anastomoses. Notwithstanding progress in recent years, TTTS surgery is highly risky. Computer-assisted planning of the intervention can thus improve the outcome. METHODS: In …

Treatment of tongue telangiectasia in a patient with hereditary haemorrhagic telangiectasia.

A 61-year-old Caucasian woman presented to an outpatient otolaryngology clinic with increased bleeding from a dorsal tongue telangiectasia for 3 weeks. Her history was significant for hereditary haemorrhagic telangiectasia (HHT), a rare condition that causes vascular dysplasia, and recent symptomatic anaemia requiring blood transfusions. After failing medical management with topical haemostatic agents, she was offered and underwent surgical intervention to remove the tongue telangiectasia with duel therapy potassium titanyl phosphate (KTP) laser coblation and bevacizumab injections. A team of

Photoacoustic imaging of the human placental vasculature.

Minimally invasive fetal interventions require accurate imaging from inside the uterine cavity. Twin-to-twin transfusion syndrome (TTTS), a condition considered in this study, occurs from abnormal vascular anastomoses in the placenta that allow blood to flow unevenly between the fetuses. Currently, TTTS is treated fetoscopically by identifying the anastomosing vessels, and then performing laser photocoagulation. However, white light fetoscopy provides limited visibility of placental vasculature, which can lead to missed anastomoses or incomplete photocoagulation. Photoacoustic (PA) imaging is

Laser Treatment of Twin-to-Twin Transfusion Syndrome.

OBJECTIVE: Laser ablation of all placental vascular anastomoses is the optimal treatment for twin-twin transfusion syndrome (TTTS). However, two important controversies are apparent in the literature: (a) a gap between concept and performance, and (b) controversy regarding whether all the anastomoses can be identified endoscopically and whether blind lasering of healthy placenta is justified. The purpose of this article is: (a) to address the potential source of the gap between concept and performance by analyzing the fundamental steps needed to successfully accomplish the surgery, and (b) to

Novel Application of Laparoscopic Ultrasound for Fetoscopic Laser Ablation in Twin-Twin Transfusion Syndrome with Complete Anterior Placenta.

Fetoscopic laser coagulation of the placental communicating vessels has become the standard treatment for monochorionic/diamniotic twin pregnancies complicated by severe twin-twin transfusion syndrome. Fetoscopic trocar placement can be performed with transabdominal ultrasound guidance with a posterior placenta and most anterior placentas that have a safe avascular window for entry. However, trocar insertion is challenging in cases of a complete anterior placenta without an avascular window. Current techniques to deal with this situation include mini-laparotomy with exteriorization to allow fo…

Surgical management of retinopathy of prematurity.

The surgical treatment and management of retinopathy of prematurity (ROP) has evolved significantly over the last decade. The introduction of new techniques, instrumentation, and surgical adjuvants has added to the repertoire of options vitreoretinal surgeons have to treat complex surgical ROP cases. This chapter focuses on a review of the clinical outcomes, indications, techniques, and adjuncts for the surgical treatment of ROP.

Prophylactic treatment of retinal breaks--a systematic review.

Prophylactic treatment of retinal breaks has been examined in several studies and reviews, but so far, no studies have successfully applied a systematic approach. In the present systematic review, we examined the need of follow-up after posterior vitreous detachment (PVD) - diagnosed by slit-lamp biomicroscopy or Goldmann 3-mirror examination - with regard to retinal breaks as well as the indication of prophylactic treatment in asymptomatic and symptomatic breaks. A total of 2941 publications were identified with PubMed and Medline searches. Two manual search strategies were used for papers in

The effect of probe placement and type of viscoelastic material on endoscopic cyclophotocoagulation laser energy transmission.

BACKGROUND AND OBJECTIVE: Endoscopic cyclophotocoagulation is a laser modality used to treat refractory glaucoma through the ablation of ciliary epithelium on the ciliary processes, which results in reduced aqueous production. The purpose of this study was to investigate the effect of various viscoelastics and optimal probe positioning on endoscopic cyclophotocoagulation treatment. MATERIALS AND METHODS: Six different viscoelastic materials and a balanced salt solution, which was used as a control, were evaluated by placing 2 mL of each in a separate 2 mL glass tube. Laser power was measured
