Critical care medicine has matured greatly as a field in the past decade. Much has been learned concerning the institution of life support therapies to sustain patients with diverse and multiple organ failures, thus providing patients with a window of opportunity to recover from potentially life-ending insults. The management of critically ill patients has increasingly involved creation of a highly controlled environment by care providers, with patients immobilized, tethered to devices, and receiving multiple drugs to facilitate the entire process. Although it has been assumed that such contro

BACKGROUND: The use of extracorporeal membrane oxygenation (ECMO) for critically ill patients is growing rapidly given recent developments in technology. However, adverse events are frequently reported that have potentially devastating impacts on patient outcomes. The information on predictors and risk factors for thrombotic events, especially that focusing on the comparison of veno-arterial and veno-venous ECMO configurations, are still inconsistent and sparse; therefore, we aimed to close this gap.
METHODS: We performed a retrospective analysis of all patients on extracorporeal life support

BACKGROUND: The aim of this study was to compare the outcome of severe blunt trauma in children receiving prehospital care from either physician-staffed advanced life support (ALS) units, or from basic life support (BLS) units staffed by emergency medical technicians.
METHODS: The records of 288 children with severe blunt trauma who required intensive care in the regional level 1 trauma center or who died from their injuries were analyzed retrospectively. Patients were excluded if resuscitation at the scene was not attempted, if the level of prehospital care was unspecified, or if arrival at

In order to identify the care access and life conditions and non-compliance risk factors of sickle-cell patients, we performed a literature survey on these subjects, and on other chronic pathologies and care systems used as references. The data showed a positive influence of good quality of life, social support and family environment on diabetes mellitus patients compliance, and the negative effect of poor socioeconomic status on adequate prenatal care. Concerning sickle-cell disease, there were few data on the non-compliance risk factors. However, the authors identified factors which influenc

BACKGROUND: Childhood obesity is a chronic disease with negative physical and psychosocial health consequences. To manage childhood overweight and obesity, integrated care as part of an integrated approach is needed. To realise implementation of this integrated care, practical guidance for policy and practice is needed. The aim of this study is to describe the development of a Dutch national model of integrated care for childhood overweight and obesity and accompanying materials for policy and practice.
METHODS: The development of the national model was led by a university-based team in colla

Frailty is common among older adults admitted with multimorbidity and functional decline and has major implications for prognosis, treatment goals and quality of life. Systematic frailty assessment can support clinical and ethical decision-making, facilitate timely palliative care and prevent overtreatment. When combined with structured conversations about goals of care and interdisciplinary collaboration, this review finds, frailty assessment enables more person-centred and dignified care trajectories across sectors.

BACKGROUND/AIM: Chemotherapy administered near death is a recognized quality indicator of end-of-life cancer care intensity. However, simple clinical signals to prompt treatment reassessment and coordinate planned end-of-life care remain unclear in advanced gastric cancer. This study evaluated the interval from last chemotherapy to death and explored factors associated with near-end-of-life chemotherapy and failure to transition to a planned end-of-life care setting.
PATIENTS AND METHODS: This single-center retrospective study included 53 patients with advanced or recurrent gastric cancer who

BACKGROUND: The provision of end-of-life care for children in intensive care is a stressful and traumatic event for parents with immediate and life-long impacts. End-of-life care for children is also challenging for critical care nurses who must balance the child's clinical requirements concurrently with facilitating and supporting the needs of parents and family members.
RESEARCH QUESTION: What is the research evidence about nurses' and parents' experiences of the provision of end-of-life care in intensive care for children?

BACKGROUND: While most people express a preference to be cared for and die at home, not all people have access to a safe or stable home. Government policies often position home death as a proxy measure of quality end-of-life care, yet structurally vulnerable populations, including those who are precariously housed, homeless, or living in supportive or transitional housing, are frequently excluded from this possibility. This scoping review aims to summarize the current literature on the factors that facilitate or prevent palliative care at ‘home’ for structurally vulnerable adults. METHODS: We

A 46-year-old man presented to the emergency room with pain in his left leg, dyspnea, and general cyanosis. During examination he collapsed and required resuscitation. Under suspicion of pulmonary embolism, a new portable "click 'n run" extracorporeal life support system (LIFEBRIDGE-B(2)T [Medizintechnik AG, Ampfing, Germany]) was implanted by the femoral vessels under resuscitation within 15 minutes of presentation. The patient was stabilized, despite severe decompensation (pH, 6.8), and could be transferred for a computed tomographic scan, which confirmed massive pulmonary embolism. Still co

Background: Between 2015 and 2050, the aging population of Uganda (aged 50 years and older) will be nearly doubled. Therefore, later-life problems have become an area of increasing research and policy interest. This study aimed at exploring how aging people living in extreme poverty in a low-income country experience their everyday life and what kind of meaning systems employed by them to understand and cope with their living conditions. Methods: We conducted a qualitative interview with 14 participants in the Buikwe district. In this interview, 11 women and 3 men were included, and a thematic

Multidisciplinary care pathways may improve quality of care, yet little is known about patients' perspectives regarding multidisciplinary treatment for high-risk head and neck cutaneous squamous cell carcinoma. This study examined health-related quality of life, decisional conflict, and satisfaction with care in 78 high-risk head and neck cutaneous squamous cell carcinoma patients undergoing multidisciplinary care. Pre-treatment, patients completed a baseline questionnaire, the EuroQoL-5D-5L, and the Decisional Conflict Scale. One month post-treatment, they completed the EuroQoL-5D-5L, Basal a

BACKGROUND AND PURPOSE: Periodontitis is the leading cause of tooth loss in adults worldwide. The functional loss, nutritional deficiencies, and psychological barriers it causes, as well as its impact on overall health and quality of life, are all significant. The prevalence of periodontal disease is high in China. Our study aimed to determine the intricate relationship between periodontal disease status, dental anxiety, self-rated oral health (SROH), self-efficacy for oral care, perceived social support, socioeconomic status (SES), and the oral health-related quality of life (OHRQoL) among pe

Our goal was to test the effectiveness of collaborative care (CC) versus usual care (UC) to improve treatment of pain, depression, physical inactivity, and quality of life in outpatients with spinal cord injury (SCI). We conducted a single blind parallel group randomized controlled trial. The setting was two outpatient SCI rehabilitation clinics within a large academic medical center. Participants were 174 outpatients who were on average 47.7 years old, 76% male, 76% white, 8% Hispanic, 47% tetraplegic, 95% more than 1 year post-SCI, and 45% on Medicare. The intervention consisted of a mental

INTRODUCTION: Throughout the Global War on Terror (GWOT), the U.S. military built a revolutionary capacity to deliver life-saving care-even in austere environments-through rapid evacuation, far-forward providers and resources, and advances in prehospital and trauma-critical care. However, the operational reality of future Large Scale Combat Operations (LSCO), as evidenced by wars in Ukraine, Sudan, and Gaza, will be marked by high casualty rates and limited medical resources. The number of patients with non-survivable injuries because of operational constraints such as inability to evacuate, m
