Calcium, magnesium and phosphorus in the nutrition of the newborn.

Several factors have been suggested to contribute to inadequate bone mineralization in infants. Calcium and phosphorus intakes in preterm infants are below the intrauterine accretion rates. Calcium retention is influenced by the types of calcium salts used and by alterations in dietary phosphorus, fat and carbohydrates. Dietary intakes of vitamin D, and modifications in the protein base of infant formula, e.g., soy base vs cow milk base, may impact bone mineralization. The major hormonal mechanisms involved in the regulation of bone mineralization are parathyroid hormone, calcitonin and vitami

The pathogenesis of eclampsia: the 'magnesium ischaemia' hypothesis.

'Magnesium ischaemia' is a term used to denote the functional impairment of the ATP-dependent sodium/potassium and calcium pumps in the cell membranes and within the cell itself. The production of ATP and the functioning of these pumps is magnesium-dependent and is critically sensitive to acidosis. Zinc and iron deficiencies may secondarily impair these pumps and thus contribute to 'magnesium ischaemia' (as does acidosis). This term is two-dimensional at its simplest; it refers to a functional magnesium deficiency, whether actual or induced. It is argued that chronic acidosis is the most commo…

Role of magnesium in regulation of lung function.

Magnesium and calcium play multiple dynamic roles in pulmonary structure and function. When magnesium is deficient, the action of calcium is enhanced. In contrast, an excess of magnesium blocks calcium. These interactions are important to the respiratory patient because the intracellular influx of calcium causes bronchial smooth-muscle contraction. The possibility exists that magnesium deficiency contributes to pulmonary complications. During the past few years, there has been an increase in calcium consumption in the US population but little change in magnesium intake, which has caused an imb…

Effect of magnesium on phosphorus and calcium metabolism.

Magnesium is an element that occurs ubiquitously in nature. Magnesium and calcium metabolism are closely related. The intestinal absorption and the renal excretion of the two ions are interdependent. The relationship between phosphorus and magnesium metabolism is more difficult to demonstrate. The most frequent causes of hypomagnesemia in children are reduced intake, impaired intestinal absorption, renal loss and genetic diseases. Hypomagnesemia is reflected clinically in the nervous system, and there are neurophysiological and metabolic changes. Severe hypomagnesemia induces secondary hypocal…

[Magnesium sulfate in prevention of preterm labor].

To observe the efficacy of magnesium sulfate in the treatment of preterm labor, 65 uncomplicated cases of preterm labor between 28 and 36 weeks of gestation were studied prospectively during Sep. 1988-May 1991. They were divided into two groups randomly. 30 cases were treated with magnesium sulfate and 35 cases with barbiturates or bed rest. The prevention of delivery for at least 48 hours after the initiation of therapy was achieved in 23 of the 30 cases (76.67%) of the magnesium sulfate group and in 3 of the 35 cases (8.57%) in the control group. Delay of more than 7 days was achieved in 17 …

Serum and erythrocyte magnesium concentrations and migraine.

Most people suffering from migraine present a hypersensitivity of the cervico-facial muscles, which may be linked to magnesium deficit. This paper describes a comparative study of serum and erythrocyte magnesium levels in 79 migraine patients and 55 controls. Serum magnesium did not differ significantly in either group, but some values were quite low. On the other hand, there was a significantly greater erythrocyte magnesium deficit in persons suffering from migraine than in controls (in both female and male subjects). This fact correlates with the improved state of migraine patients (already …

Blood magnesium, calcium and zinc in osteoporosis.

We examined blood Mg, Ca and Zn in 33 patients with senile, 18 patients with postmenopausal and 9 patients with medicamentous osteoporosis. The biochemical signs of chronic Mg deficiency were present in the first two groups due to lowering erythrocyte Mg. Correlation among these three ions in serum and erythrocytes brought different patterns which were richer in the first two groups compared with the third one. It seems probable that treatment with Mg lactate potentiates the storage of intracellular Zn. The correlation between biochemical and clinical findings was carried out, too. Pain, kypho

Magnesium deficiency in alcohol addiction and withdrawal.

The earliest description of clinical magnesium deficiency was reported in 1934. In 1954, Flink reported alcoholism as a cause of magnesium deficiency. This has been confirmed by low serum and tissue levels, balance studies, low exchangeable 28Mg and parenteral Mg retention tests. Alcohol causes urinary Mg wastage, but other mechanisms related to alcoholism contribute to the magnesium deficiency including malnutrition, gastrointestinal losses, phosphate deficiency, acidosis and/or alkalosis, vitamin D deficiency and free fatty acidemia associated with alcohol withdrawal. Mg replacement therapy …

Relation of magnesium to osteoporosis and calcium urolithiasis.

Magnesium influences mineral metabolism in hard and soft tissues indirectly through hormonal and other modulating factors, and by direct effects on the processes of bone formation and resorption and of crystallization (mineralization). Its causative and therapeutic relationships to calcium urolithiasis (CaUr) are controversial despite an association between low urinary Mg and CaUr. Recent studies have also found a tendency to low serum and/or lymphocyte Mg levels in CaUr. Despite earlier studies demonstrating an inhibitory effect of Mg supplementation on experimental CaUr in animals and in spo…

The role of magnesium in clinical biochemistry: an overview.

Magnesium is the second most abundant intracellular cation. It is essential for a wide variety of metabolically important reactions, in particular those involving ATP. Hypomagnesaemia is surprisingly common in hospital populations but is sometimes either undetected or overlooked. Serum magnesium concentrations provide a guide to magnesium status but while hypomagnesaemia is a reliable indicator of magnesium deficiency, normomagnesaemia does not exclude magnesium depletion. A wide variety of conditions predispose to magnesium depletion. Clinical magnesium deficiency has potentially fatal conseq…

Changes of magnesium concentrations in endurance athletes.

Erythrocitary and serum magnesium (Mg) were determined in a group of 11 well-trained athletes before and after a 25-km running race, and in a group of 30 sedentary controls. The significant increase of mean erythrocitary Mg (EMg) concentration observed in the athletes after physical strain (2.58 +/- 0.34 mEq/l before, 3.10 +/- 0.45 mEq/l after the race: significance level = 5%) leads to the assumption that the possible Mg uptake is effected by the red blood cell to enhance some enzymatic reactions. The decrease of mean serum Mg concentration observed in the same subjects after the effort (1.70…

Magnesium as a modifier of smooth muscle contractility.

In smooth muscle, Ca2+ enters the cell through two different types of Ca2+ channels, activates contractile filaments and induces contraction. These Ca2+ channels are inhibited by Ca2+ channel blockers and/or nitrocompounds. Mg2+ inhibits both of these Ca2+ channels and relaxes the muscle. A portion of the smooth muscle contraction is due also to the release of Ca2+ from the cellular storage site. Caffeine and norepinephrine release Ca2+ from this store to induce a transient contraction. The contraction induced by caffeine is greatly augmented in the absence of Mg2+. The other effect of Mg2+ de

Magnesium and the pathogenesis of toxic shock syndrome.

Excess production of toxic shock syndrome toxin 1 by appropriate strains of Staphylococcus aureus occurs when the organisms are grown in an environment deficient in Mg++. Since many of the fibers previously used in tampons combine with Mg++, an explanation for the pathogenesis of menstrually related toxic shock syndrome presents itself. Pitfalls in the reproduction of these experiments have been investigated and include attention to inoculum size, to the effect of washing the inoculum to rid it of magnesium from the parent culture, and related variables. The growth of staphylococci in magnesiu…

Magnesium therapy in infants with postneonatal apnea.

Idiopathic postneonatal apnea is defined here as a sudden, unexpected attack of apnea that first occurs after the infant has been discharged from the neonatal nursery to his home. Little has been published concerning the patient's physical and clinical laboratory findings during the acute episode. The present report of 20 infants with postneonatal apnea retrospectively addressed these parameters, including all investigations conducted for magnesium deficiency. The first episode of apnea occurred at 48 +/- 12.5 days of age and was unpredicted, transient, and self-limited. In the sickest infants…
