Realistic training in aviation now includes the use of sophisticated flight simulators which can provide near total perception of aircraft flight. Reports of simulator sickness seem to be increasing in simulators with multi-directional motion, computer-generated imagery, and infinity optics. A voluntary, subjective, symptom-oriented questionnaire was completed by 141 pilots after training in the UH-60A 2B38 Blackhawk flight simulator. Intrahepatic biliary dilatation on ultrasound has been reported as an early sign of biliary obstruction. This paper reports 12 patients with extrahepatic biliary obstruction in whome the extrahepatic biliary system was increased in caliber despite a normal appearance of the intrahepatic ducts. BACKGROUND: Endoscopic biopsy is mandatory for the diagnosis of malignant and premalignant ampullary tumours. The commonly reported inadvertent complications following routine mucosal biopsy include perforation and haemorrhage. Acute pancreatitis is an extremely rare complication following this procedure.
CASE SUMMARY: This report details the case of a 59-year-old man who underwent biopsy of the ampulla for a suspected periampullary tumour. Following the procedure, the patient presented with symptoms of acute pancreatitis which was substantiated by laboratory and radiological investigations.

Umbilical cord ulceration is a rare condition presenting with sudden fetal bradycardia due to fetal hemorrhage and in most cases leading to intrauterine death. Primary mucinous eccrine adenocarcinoma, a rare malignant neoplasm of the skin adnexa usually occurs in the head and neck region. The most common sites for its occurrence are the eyelid, the peri-orbital region and the scalp. Mediastinal masses in children comprises of a heterogeneous group of tumours. In such cases, biopsy and histological analysis are mandatory for planning of treatment. We have reported an unusual aetiology for a mediastinal mass in a young boy presenting with features of Superior Vena Caval Obstruction (SVCO) who also had marked blood and marrow eosinophilia mimicking Chronic Eosinophilic Leukaemia (CEL). We have also discussed the differential diagnoses of mediastinal tumours with hyper-eosinophilia and possible therapeutic implications.

More than 1000 hemophilic male patients are registered in Hungary, from which only a trace number suffers from factor IX inhibitory hemophilia. For correct dental and oral surgical treatment of these patients mandatory cooperation is required among medical specialties, exerting multi-staged haemostatic principles. Authors represent in this case report the dental and oral surgical treatment of a B hemophilic patient with high inhibitor level and describe possible local haemostatic measures.

Hypogonadism affecting the male pediatric population is uncommon, with that attributed to multiple unrelated etiologies being exceedingly rare. We report a case of septo-optic dysplasia, an atypical cause of delayed puberty, with subsequent workup unveiling 2 coexistent conditions: hypogonadotropic hypogonadism and anorchia. Primary and secondary etiologies must be considered in patients with undescended testes. Thorough evaluation is mandatory to ensure proper diagnosis and care, because Occam's razor can, on unique occasions, be double-edged.

The prognosis of patients with adrenal metastases from hepatocellular carcinoma (HCC) has been poor, and aggressive treatment of these tumors is mandatory to improve patients' survival. Since adrenalectomy may be difficult to perform after previous surgery of the right liver lobe, other approaches are required to treat the adrenal mass. This report aims at demonstrating the feasibility of CT-guided transhepatic radiofrequency ablation of right adrenal HCC metastases pretreated with chemoembolization in patients unable to undergo surgical resection.

Children surviving after haematopoietic SCT (HSCT) are a growing population that need to keep their health status under control. They may experience early and late complications that are related to transplant procedures and to treatments administered before HSCT. Monitoring transplant-related complications is mandatory, especially during the child's growth. The purpose of this report is to define preassessment in patients who are candidates for HSCT to identify any pretransplant, comorbid conditions and to individually tailor the HSCT procedure, whenever possible.

Abdominoscrotal hydrocele (ASH) is an apparently highly uncommon clinical entity especially in childhood. While hydrocele is among the commonest inguinal anomaly in adult, its extreme form ASH is infrequently reported form. ASH consists of two large sacs, both abdominal and scrotal, connecting with the inguinal canal. Surgical correction is mandatory. We present a 34-year-old man with giant ASH. He was treated with excision of hydrocele sac through an inguinal approach.

Intracranial infection after neurosurgical intervention most often is caused by bacteria. A rare case of fatal herpes simplex encephalitis after removal of a meningioma is described and similar cases reported in the literature are reviewed. Recent diagnostic tools, including detection of herpes viral DNA sequences by polymerase chain reaction, complement clinical suspicion and facilitate mandatory early diagnosis, because herpes encephalitis, without rapid initiation of treatment, may lead to severe disability or death.

Annular elastolytic giant cell granuloma is an uncommon granulomatous cutaneous disease that usually affects sun-exposed skin. Non-scarring alopecia is a possible presentation. Although histopathology is mandatory for the diagnosis, dermoscopy may help to narrow down the clinical differential diagnosis. The authors report a case of annular elastolytic giant cell granuloma in the scalp of a female adult patient, showing multiple yellowish/orange follicular dots in a diffuse erythemato-whitish background in the dermoscopy.

We report a rare case of vaginal amelanotic melanoma. Malignant melanomas are cutaneous and extracutaneous tumors that arise from embryological remnants of neural crest cells/melanocytes. Amelanotic melanomas at such rare locations can be misdiagnosed both clinically and radiologically. Therefore, histopathological examination and immunohistochemistry are mandatory for the diagnosis of these tumors. We diagnosed this case using histopathology and confirmed the diagnosis based on the presence of immunohistochemical markers human melanoma black 45 (HMB45) and S-100.

Coronary artery interventions in the setting of Immune Thrombocytopaenic Purpura (ITP) carry the twin risks of bleeding and thrombosis related to the mandatory dual anti-platelet therapy. This condition therefore may require a splenectomy prior to the coronary intervention. Surgical splenectomy in the presence of high-grade coronary stenosis carries greater morbidity and mortality. We report here a unique solution to this therapeutic dilemma, which was splenic artery embolisation followed by percutaneous coronary intervention (PCI).

Splenic infarction is a rare disorder, commonly without a characteristic symptomatology, that rarely requires surgical procedure; in fact it has the peculiar feature of an high tendency for complete healing after the only medical approach. Furthermore in order to prevent OPSI, only in case of clear, persistent symptoms or complications it may be necessary surgical laparoscopic or open approach. The Authors report on a recent case observed in emergency of splenic infarction with infectious complications that made the surgical procedure mandatory.

BACKGROUND: Bilateral emphysematous pyelonephritis is a life threatening condition usually occurring in diabetics. Management of this condition has traditionally been aggressive and surgery is considered mandatory. However, this is itself a hazardous intervention in a septic, unstable patient with circulatory or liver failure. When bilateral disease is present, the need for long-term dialysis is obviously unavoidable.
CASE PRESENTATION: We herein report one of the few cases of bilateral emphysematous pyelonephritis successfully managed by non-surgical treatment.

Agenesis of corpus callosum (ACC) is one of the most common brain malformations observed in humans. It is a heterogeneous malformation, with many etiologies. Isolated ACC is usually sporadic but familial cases have been reported. ACC can complicate numerous polymalformative, either monogenic or chromosomal syndromes. In some of them, for example Aicardi syndrome, Anderman syndrome or acrocallosal syndrome, ACC is a mandatory manifestation. Further identification of the molecular bases of these syndromes will be helpful in understanding the causal heterogeneity of this malformation.

Twelve cases with primary ureteral carcinoma are reported with a review of literatures. Urography was the main method of diagnosis, and retrograde uretero-pyelography was more helpful. Cystoscopy was mandatory before operation. CT was useful for diagnosis and staging. If needed, ureteroscopy could be used to increase diagnostic accuracy. The traditional total nephroureterectomy with a cuff excision of the urinary bladder was the first and basic choice of treatment. Regular cystoscopy, excretory urography and adjuvant intravesical therapy should be carried out after operation.

OBJECTIVE: To report the unusual case of a mediastinal pseudocyst in a patient with chronic pancreatitis, which resolved after conservative treatment.
PATIENT: A 59-year-old patient with alcohol-induced chronic pancreatitis and a large mediastinal pseudocyst.
INTERVENTIONS: Bowel rest and total parenteral nutrition for 4 weeks.
MAIN OUTCOME MEASURES: The pseudocyst resolved completely.
CONCLUSION: Conservative treatment is a valuable option in patients with chronic pancreatitis presenting with a mediastinal pseudocyst, but close supervision is mandatory.
