Rehabilitation Of Atrophic Maxilla With Immediate Loading And Long Trans-Sinus Implants: 5 Year Prospective Case Series.

PURPOSE: This prospective clinical case series evaluated the 5-year success of a bone anchorage technique using long trans-sinus implants for the rehabilitation of atrophic maxilla. MATERIALS AND METHODS: Twenty patients were treated according to the all-on-four concept, receiving 45 standard implants and 35 long implants (18-24 mm) placed through the maxillary sinus. All patients were rehabilitated with metal-acrylic resin fixed complete prostheses. Prosthesis survival was defined as the presence of the prosthesis, with or without modifications. Follow-ups were conducted every 6 months, incl…

FGF9-FGFR2 Signaling via Osteocytes-Preosteoblasts Crosstalks to Mediate Mechanotransduction-Driven Intramembranous Osteogenesis in the Underdeveloped Maxilla.

Maxillary underdevelopment is a critical component of skeletal Class III malocclusion, closely linked to altered biomechanical signaling. Mechanical stimulation through early facemask protraction can effectively promote maxillary growth, yet the underlying mechanotransduction mechanisms remain unclear. In this study, fibroblast growth factor 9 (FGF9) is identified as a key biomechanical responder in maxillary development. Secreted predominantly by osteocytes, FGF9 interacts with fibroblast growth factor receptor 2 (FGFR2) on preosteoblasts to inhibit osteogenic differentiation. Mechanical stre…

Maxilla-first patient-specific osteosynthesis vs mandible-first bimaxillary orthognathic surgery using splints: a randomized controlled trial.

Patient-specific osteosynthesis plates and three-dimensional virtual surgical planning with patient-specific surgical guides have significantly advanced orthognathic surgery, enhancing surgical accuracy. This study compares the outcomes of the mandible-first approach using an intermediate splint and manually bent osteosynthesis plates with the maxilla-first approach utilizing patient-specific osteosynthesis fixation in bimaxillary orthognathic surgery. This multi-centre randomized controlled trial included 88 patients, with 77 completing the study. Patients were randomly assigned to either the…

Dimensional changes in buccopalatal alveolar bone following immediate implant placement in the anterior maxilla: a retrospective study.

This retrospective study examines the factors influencing bone thickness buccal to the implant (BTFU) and alveolar width at follow up (AW1) in immediate implant placement procedures in the upper aesthetic zone. After reviewing university records, thirty patients who underwent immediate implant placement before 2020 were recruited for cone-beam computed tomographic (CBCT) scans. Variables such as buccal gap, alveolar width, and buccal bone plate thickness were measured at T0 (pre-extraction) and T1 (study follow up). The results indicated significant effects of the type of bone graft used, bucc

Mistaken identity of polymorphous low-grade adenocarcinoma treated as periapical lesion in anterior maxilla: A case report.

This report presents a rare case of polymorphous low-grade adenocarcinoma (PLGA) in the anterior maxilla, mimicking a periapical lesion. A 56-year-old male reported with a painless swelling in the maxillary right canine-premolar region. The patient gave a history of root canal therapy for the same teeth one month ago. On intraoral examination, the involved teeth were restored with ceramic crowns. Radiographic examination revealed inadequate obturation with an ill-defined radiolucency in the periapical region. 3D cone beam computer tomography (CBCT) revealed erosion of the buccal cortical plate…

Rehabilitation of an edentulous atrophic maxilla with four unsplinted narrow diameter titanium-zirconium implants supporting an overdenture.

The edentulous maxilla is often affected by bone resorption, sometimes making it difficult to place standard diameter implants. Narrow diameter implants made of titanium-zirconium (Ti-Zr) alloy, which has superior mechanical properties compared with titanium, have been proposed for these difficult situations. This retrospective clinical observation reports the outcome of the use of reduced diameter implants made of Ti-Zr alloy supporting maxillary overdentures retained with locator abutments. The charts of all patients who received maxillary overdentures supported by four unsplinted implants f…

Dental rehabilitation using an implant-carrying plate system in a severely resorbed edentulous maxilla: a case report.

This clinical article describes a case of dental rehabilitation using an implant-carrying plate system (EPITEC) for a patient with severely resorbed edentulous maxilla and microstomia. In this case, the presence of microstomia prevented bone augmentation procedures through an intraoral approach. Treatment using 2 endosseous implants inserted in the canine regions and an implant-supported overdenture was planned. However, endosseous implants were not feasible on the right side because of insufficient available bone volume. An implant-carrying plate system was then utilized on the right side. Fo…

[A long-term follow-up case of multiple impacted teeth associated with large follicular cyst in maxilla].

Longitudinal record of a case of multiple impacted teeth associated with large follicular cyst in the right maxilla was presented. The patient was an 8-year-10-month-old girl whose chief complaint was delayed eruption of the right upper incisor. Clinical examination revealed a large follicular cyst in the right maxillary sinus, which greatly displaced teeth germs. Marsupialization followed by orthodontic extrusion successfully brought unerupted teeth into their positions. Greatly displaced upper right canine, which was as high as the floor of the orbit, erupted spontaneously after reduction of…

Fibrous Dysplasia of Frontal Process of Maxilla: False-Positive Finding on 18F-FDG PET in Multiple Myeloma.

18F-FDG focal uptake in bone is always concerning, especially in patients with a history of cancer. We report a case of increasing hypermetabolic activity of a maxillary bone lesion in a patient with a history of multiple myeloma that was managed with definitive treatment. A targeted CT scan revealed that the abnormal 18F-FDG uptake was fibrous dysplasia. Vigilant interpretation is required for focal osseous activity on PET/CT in patients with a history of cancer to avoid false-positive readings.

Mandibular autorotation: a critical virtual parameter in clinical decision-making regarding maxilla-first versus mandible-first sequence.

Large degrees of mandibular autorotation during intermediate splint design are prone to transfer error and decrease accuracy. The purpose of this study was to evaluate the amount of mandibular autorotation necessary to design intermediate splints for maxilla- and mandible-first sequences using virtual planning software, to help the clinical decision-making regarding the most adequate sequence for each patient. The influence of specific orthognathic movements (different vertical and sagittal changes at the Le Fort I level, cant correction) and the type of maxillofacial deformity (skeletal Class…
