Measles in pregnancy: a threat for Italian women?

Measles in pregnancy may lead to serious sequelae for newborns and mothers. In Italy assessment of immunity against measles is not recommended as pregnancy screening. This study aimed to assess the immunity against measles in pregnant women from Apulia Region in Southern Italy between 2016 and 2017. Overall, 96.9% of pregnant women were positive for anti-measles IgG, younger women aged 19-29 years had a seroprevalence below 90%. No samples were positive for anti-measles IgM. In conclusion, younger pregnant women showed to be at higher risk of contracting measles during pregnancy. These finding…

Global landscape of measles and rubella surveillance.

BACKGROUND: All six World Health Organization (WHO) regions have committed to eliminate measles, and three WHO regions have committed to eliminate rubella. One of the key tenets of measles and rubella elimination is to have a strong surveillance system in place. The presence of a case-based measles and rubella surveillance system that is national, population-based, provides laboratory confirmation, and directs action, is one of the requirements for elimination-standard surveillance. METHODS: In order to understand the global landscape for measles and rubella surveillance, a questionnaire was …

Measles: a disease often forgotten but not gone.

Measles (rubeola) is a highly contagious vaccine-preventable disease caused by the measles virus-a virus of the Paramyxoviridae family. The illness typically begins with fever, runny nose, cough, and pathognomonic enanthem (Koplik spots) followed by a characteristic erythematous, maculopapular rash. The rash classically begins on the face and becomes more confluent as it spreads cephalocaudally. Laboratory confirmation of measles virus infection can be based on a positive serological test for measles-specific immunoglobulin M antibody, a four-fold or greater increase in measles-specific immuno…

A Rare Coincidence of Measles with Typhoid Fever.

Typhoid, otherwise known as enteric fever, and measles both have a high incidence worldwide. However, a coincidence of both has been only documented twice previously in 1866 and 1949. We present a case of a 24-year-old male who presented with high-grade fever and diffuse abdominal tenderness. He was diagnosed with typhoid initially, but during the course of his illness, he developed a maculopapular rash and pathognomonic Koplik's spots. Further investigations confirmed measles that was concurrent with typhoid. This highlights the importance of further comprehensive investigations even in diagn…

A Moral Imperative to Eradicate Measles, Rubella.

On Sept. 27, 2016, the Pan American Health Organization — the World Health Organization’s Americas branch — announced that measles had been eliminated in the region. It was exciting news! PAHO is the first WHO region to accomplish the task, a triumphant sequel to its 2015 announcement that, thanks to widespread vaccination, rubella (along with congenital rubella syndrome) had been eliminated in the Americas.1

Measles Lymphadenopathy in a Child With PFAPA Syndrome.

Periodic fever, aphthous stomatitis, pharyngitis, and cervical adenitis (PFAPA) syndrome is a common cause of periodic fever in children. The pathogenesis of PFAPA is unknown but likely involves immune system dysregulation and may be initiated by an environmental trigger. Tonsillectomy resolves or improves symptoms in some patients, but the reason for this is unknown; moreover, specific abnormalities in tonsillectomy specimens from PFAPA patients have not been described. Here, we report measles virus in tonsil from a child with PFAPA. Measles-type viral cytopathic effect was discovered on hist…

Prevention of measles spread on a paediatric ward.

Since measles is a highly contagious respiratory infection with significant airborne transmission risk in hospitals, effective prevention measures are crucial. After a mother accompanying her child on a paediatric ward lacking a negative pressure room was diagnosed with measles, exposed persons without evidence of immunity (documentary evidence of receiving two doses of measles-mumps-rubella vaccine) were treated with vaccination or intravenous immunoglobulin (IVIG). The interruption of transmission with these treatments was evaluated. There were 44 children and 101 adults exposed to the index…

Measles elimination goal: is it feasible for India?

OBJECTIVE: To assess whether measles elimination goal would be feasible for India or not. METHODOLOGY: Secondary review of data from WHO Regional Consultative Meet on Measles and Workshop on Rubella. Discussion with stakeholders such as UNICEF experts, PATH, and Government officials. RESULTS: The National Technical Advisory Group of India (NTAGI) has chalked out two broad strategies depending on the routine measles coverage of first dose (MCV1). In 18 states with sustained MCV1 coverage ≥80%, a second routine dose of measles would be given, whereas those states with <80% coverage, supplement…

[Measles elimination programme on the world and Poland].

Measles is still one of the leading causes of children mortality, despite of availability of a cheap, effective and safe vaccine for more than 40 years. Effective global eradication of smallpox and the success of polio eradication have provided an incentive to achieve the measles eradication all over the world. Elimination is achieved when no endemic measles cases has been observed in given area. This study analyzes epidemiological situation of measles, measles vaccination and laboratory diagnosis. In this report we describe the role of Global Measles Laboratory Network and integrated measles/…

Evaluation of the measles clinical case definition.

An accurate system of identifying and classifying suspected measles cases is critical for the measles surveillance system in the United States. To examine the performance of the clinical case definition in predicting laboratory confirmation of suspected cases of measles, we reviewed 4 studies conducted between 1981 and 1994. A clinical case definition was examined that included a generalized maculopapular rash, fever (>or=38.3 degrees C, if measured), and either a cough, coryza, or conjunctivitis. Serological confirmation of measles was done either by hemagglutination inhibition assay, complem…
