Ventilation tubes and persisting tympanic membrane perforations.

Surgical management of otitis media with effusion and recurrent acute otitis media includes myringotomy and the use of ventilation tubes. Since this procedure was reintroduced by Armstrong in 1954, it has become one of the most commonly performed operations in otolaryngology. In most series perforation of the tympanic membrane in some patients has been reported after spontaneous extrusion or removal of the tympanostomy tubes. We present a retrospective review designed to examine the incidence of persisting perforations of the tympanic membrane in our series of 2604 operated ears. The study als

Permeability of tympanotomy tubes to ototopical preparations.

Myringotomy with tube placement is the most frequently performed otolaryngologic procedure in the United States, and purulent otorrhea after this procedure is not uncommon. Believing that ototopical preparations have proved useful in treating this malady, our group was compelled to examine whether and in what quantities these preparations are able to penetrate tympanotomy tubes. To this end, models were constructed and in vitro testing carried out with several popular formulations.

Tympanic membrane patching with bandage contact lens after removal of ventilation T tubes.

OBJECTIVE: Early closure of the tympanic membrane, after removal of longstanding ventilation T tubes (VTs), is a desirable outcome. A prospective randomized control study was undertaken to determine if spontaneous healing of the tympanic membrane is promoted with a bandage contact lens (BCL). METHOD: Thirty-nine paediatric patients, who required removal of VTs under general anaesthesia, were selected. After removal of the ventilation tubes, one tympanic membrane was randomly selected for patching with a BCL, while the other underwent conservative management. Follow-up examination was performe

Myringosclerosis caused by increased oxygen concentration in traumatized tympanic membranes. Experimental study.

The purpose of this study was to elucidate possible relationships between the oxygen concentration of the middle ear cavity and the development of myringosclerosis. Three groups of rats with myringotomized tympanic membranes were exposed to different oxygen concentrations of 10%, 15%, and 40%, respectively, for 1 week. A fourth group was kept in ambient air. Two other groups of rats with myringotomized and intubated tympanic membranes were exposed to oxygen concentrations of 10% and 40%, respectively, for the same period of time. Otomicroscopically, all hyperoxic animals had more numerous myri

The prevention of tympanic membrane perforation following the removal of long-term Paparella type II ventilation tubes.

Residual perforation following the extrusion or removal of a long-term ventilation tube may occur in between 8.5% and 25% of cases. This prospective clinical trial concerns Paparella II ventilation tubes and demonstrated that (a) extracting (pulling) a tube from the tympanic membrane gives a 6-month perforation rate of 20%, (b) excising (freshening) the edge of the defect at the time of removal decreases the 6-month perforation rate to 3%, and (c) excising the edge significantly accelerates the healing of the tympanic membrane. There was no correlation in this study between the incidence of pe

Epithelial migration on the guinea-pig tympanic membrane: the influence of perforation and ventilating tube insertion.

The guinea-pig tympanic membrane was used to study epithelial migration by using an ink-dot staining technique. The normal tympanic membrane exhibited clear-cut migratory pathways, mainly in a superior direction. Tympanic membranes were acutely perforated and others had ventilating tubes inserted in them to determine their influence on the migratory process. The overall pattern of migration was found to be unaltered; only those cells were affected whose normal pathways had been disrupted by these procedures. The possible relationship of epithelial migration to ventilating tube extrusion is dis

Goode T-tubes: do the benefits of their use outweigh their complications?

In this retrospective study of 130 ears over a 5-year period, the effect of intubation with the Goode T-tube was evaluated. The tubes improved the hearing in 86% of ears with a conductive loss secondary to a middle ear effusion to an average 5 dB airbone gap. They improved the early stage retracted tympanic membrane but had no effect on the established postero-superior retraction pocket. They were successful in treating barotrauma. The main complication with their use was otorrhoea which occurred in 28% of ears, and persistent perforation occurring in 6% of the ears. Seventy-seven per cent of

Relationship between tympanic membrane retraction and habitual sniffing in patients with cholesteatoma.

CONCLUSION: Habitual sniffing affects the pathogenicity and recurrence of cholesteatoma. Postoperative instructions requesting patients to cease sniffing may reduce the retraction and recurrence of cholesteatoma. OBJECTIVE: To examine the relationship between tympanic membrane retraction and habitual sniffing in patients with cholesteatoma. METHODS: We recruited 98 patients (102 ears) who were surgically treated for cholesteatoma by canal wall-down tympanoplasty (22 ears) or canal wall-down tympanoplasty with reconstruction methods (80 ears). We classified these patients into two groups on t

Decreasing Telephone Calls for Tympanostomy Tube Otorrhea: A Pilot Study.

The "Clinical Practice Guideline: Tympanostomy Tubes in Children" published in 2013 by the American Academy of Otolaryngology-Head and Neck Surgery Foundation encourages that an "educational video, or other teaching aid, should be developed to illustrate how parents/caregivers" may manage postoperative complications such as tube otorrhea; however, the current literature is devoid of such patient safety and quality improvement measures. Our objective was to develop an effective educational model to assist parents and caregivers in understanding the signs and symptoms of tympanostomy tube (TT) o

Eustachian Tube Dysfunction in Adhesive Otitis Media: Obstructive Versus Patulous Types.

OBJECTIVE: Understanding the distinct subtypes of adhesive otitis media (AdOM) is crucial for appropriate treatment such as balloon Eustachian tuboplasty (BET), particularly in distinguishing between patulous and obstructive Eustachian tube (ET) function. Nevertheless, AdOM caused by patulous Eustachian tube dysfunction (ETD) remains underexplored. This study aims to describe the detailed characteristics and define the patulous type of AdOM associated with patulous ETD (PET). STUDY DESIGN: A prospective cohort study. SETTING: A total of 62 ears of patients diagnosed with AdOM, PET, and traum

[Use of Airwayscope with pediatric intlock in a patient with first and second branchial arch syndrome].

First and second branchial arch syndrome is a congenital anomaly of craniofacial dysplasia involving organs derived from the second branchial arch. The main characteristics are microtia and mandibular hypoplasia. A 6-year-old boy was scheduled for adenoidectomy and bilateral myringotomy and tube placement. Slow induction was performed with oxygen, nitrous oxide, and sevoflurane. No difficulties were encountered during mask ventilation, and rocuronium was administered intravenously. His epiglottis was not visible during laryngoscopy. Therefore, we used the Airwayscope (AWS). His glottis was vis

Dynamics of eardrum changes following secretory otitis. A prospective study.

Children with secretory otitis who, during the period from 1970 through 1975, had been treated with adenoidectomy and insertion of ventilating tubes were reexamined three to eight years after treatment and, again, ten to 16 years after treatment. The changes of the tympanic membranes were analyzed with regard to the occurrence of attic retractions and changes of the pars tensa of the membranae tympani. At the first follow-up examination, varying degrees of attic retractions were found in 37% of ears and, at the second follow-up examination, in 27% of ears. This general improvement was mainly d

[Chronic purulent otitis media].

This lecture reviews up-to-date information on the classification, etiology, pathogenesis, clinical features, diagnostics, surgical and conservative treatment of chronic purulent otitis media. It is maintained that surgical intervention including sanative and reconstructive procedures as well as hearing improvement measures is currently the preferred approach to the treatment of the disease in question. Special attention is given to different variants of sanative surgery in combination with tympanoplasty. The author emphasizes that the conservative therapy of chronic otitis media has the objec
