Bigger Is Not Always Better: Computational Efficiency in Lexical Prehospital Triage Modeling.

INTRODUCTION: In future large-scale combat operations, military medics will likely face staggering numbers of casualties in the face of resource constrained evacuation and forward medical care. Artificial intelligence (AI) may enable triage at scale and improve operational efficiencies. Although many assume that sophisticated large language models (LLMs) hold the most promise, simpler machine learning models (MLMs) may offer advantages in edge implementation because of lower computational burden. Here we search for computational efficiency-achieving equivalent clinical performance with computa

Association of Sickle Cell Trait on Career and Operational Outcomes in the United States Air Force.

INTRODUCTION: Sickle cell trait (SCT) is a heterozygotic state defined by having one normal hemoglobin gene and one sickle hemoglobin gene. Individuals with SCT are at increased risk for negative health outcomes during intense physical exertion, especially in hot climates and high-elevation locations, or when dehydrated. The U.S. Air Force mitigates this risk through universal screening after accession followed by education of SCT-positive airmen. Airmen who are SCT positive but remain asymptomatic are not restricted in occupation choice or deployment/duty locations based on their SCT status.

Assessment of the prevalence of respiratory pathogens and the level of immunity to respiratory viruses in soldiers and civilian military employees in Poland.

BACKGROUND: This study provides a detailed analysis of respiratory tract infections (RTIs) and immunity levels against influenza and SARS-CoV-2 among soldiers and military personnel in Poland. Owing to their unique service environments, this occupational group is at high risk. During deployments, they often face adverse physical conditions, close living quarters, and exposure to both local and endemic pathogens. It particularly increases their susceptibility to RTIs, which remain a leading cause of illness worldwide. METHODS: The study cohort included 379 participants aged between 19 and 60 y

After the Battlefield: Infectious Complications among Wounded Warriors in the Trauma Infectious Disease Outcomes Study.

INTRODUCTION: During recent wars in Iraq and Afghanistan, improved survivability in severe trauma corresponded with a rise in the proportion of trauma-related infections, including those associated with multidrug-resistant organisms (MDROs). Significant morbidity was reported in association with the infections. There is also concern regarding potential long-term impacts of the trauma-related infectious complications. Therefore, to meet the critical need of prospective collection of standardized infection-related data to understand the disease burden and improve outcomes of wounded personnel, t

Comparative Analysis of Dental Emergency Visits and Dental Readiness Classifications on a Major U.S. Army Installation.

INTRODUCTION: Optimal oral health is essential to service members' well-being, deployability, and operational performance. Dental emergencies (DEs) result in lost duty time and degrade combat effectiveness, particularly during large-scale combat operations (LSCO), when evacuation and definitive dental care may be limited. Dental Readiness Classifications (DRC) are used to determine service member deployability; however, their effectiveness in predicting and preventing DEs has not been well-characterized using contemporary data. This study evaluated the nature of DEs and its relationship with D

Plasma Biomarkers, Brain Volume, and Cognitive Performance in Service Members and Veterans With mTBI: A LIMBIC-CENC Study.

IMPORTANCE: Repetitive mild traumatic brain injury (mTBI) may result in neurobiological changes that contribute to persistent symptoms of cognitive dysfunction. Elevations in plasma biomarkers (glial fibrillary acidic protein [GFAP], neurofilament light chain [NfL], total-tau [t-tau], and ubiquitin C-terminal hydrolase-L1 [UCH-L1]) have been reported acutely after mTBI, but few studies have assessed these biomarkers during the chronic phase of mTBI. OBJECTIVE: To examine whether the associations among plasma biomarkers, brain volume, and cognition varied by mTBI among US active-duty service m

The role of freeze-dried plasma in a world of whole blood: a Canadian prehospital and transport perspective.

BACKGROUND: Hemorrhage is the leading preventable cause of trauma death, and timely transfusion remains a major challenge in Canada, particularly in rural and remote regions where long transport times create "blood deserts." Whole blood has re-emerged as the gold standard for resuscitation, but donor limitations, short shelf-life, and cold-chain requirements restrict its universal deployment. ROLE OF FREEZE-DRIED PLASMA: Freeze-dried plasma (FDP) offers a complementary solution with unique advantages for Canadian prehospital and transport systems. FDP is pathogen-reduced, shelf-stable for up

Conditional probability of posttraumatic stress disorder in the Canadian Armed Forces: A longitudinal study.

BACKGROUND: Posttraumatic stress disorder (PTSD) can be impacted by the type of trauma exposure. This is important to consider when evaluating the likelihood of having PTSD among Canadian veterans, which previous literature has not examined. The aim of this study was to: 1) Examine the prevalence and conditional probability of PTSD over a 16 year period by trauma type, differentiating by sex, and 2) Explore this relationship by worst trauma type. METHODS: We examined the prevalence of PTSD by sex among those with any exposure to 26 traumatic events, and used logistic regressions to examine tr

Exposure to Acute Psychological Trauma Prior to Blast Neurotrauma Results in Alternative Behavioral Outcomes.

Stress is a common occurrence for military personnel. This can include the stress of deployment and active combat. Anxiety is considered a reaction to stress, and with anxiety-related disorders on the rise, it is imperative that stress be considered a preexisting condition when studying a number of neurological conditions. To determine the effects of stress on the behavioral outcomes of traumatic brain injury (TBI), we used a 3 d acute unpredictable stress (AUS) model followed by blast-induced neurotrauma (BINT) to assess social anhedonia and anxiety-like behaviors in male and female rats. The
