Non-tuberculous mycobacterial cochlear implant infection: An emerging pathogen.

OBJECTIVE AND IMPORTANCE: To report one of the first cases of a cochlear implant infected by non-tuberculous mycobacteria (NTM), an emerging otologic pathogen. CLINICAL PRESENTATION: We report the case of a 78-year-old woman who underwent uncomplicated cochlear implantation though subsequently developed wound dehiscence and device exposure. Tissue culture grew non-tuberculous mycobacterial infection with Mycobacterium abscessus. INTERVENTION: The device was explanted, the surgical bed debrided, and the patient was successfully treated with a prolonged course of oral and parenteral antibiotic…

Molecular epidemiology of non-tuberculous mycobacteria in clinical samples from Jiangxi Province, 2021-2023.

BACKGROUND: Non-tuberculous mycobacteria (NTM) pose diagnostic and therapeutic challenges in tuberculosis (TB)-endemic regions like Jiangxi … 95% CI: 5.288–31.578, P < 0.001). Dominant species were Mycobacterium intracellulare (MIN, 51.7%), Mycobacterium abscessus (MAB, 30.9%), and Mycobacterium avium (MAV, 9.3%). MIN and MAB predominated in pulmonary samples (97.9% of cases), while MAV showed significant extrapulmonary tropism (42.9% vs. 8.6% pulmonary, P = 0.02). Detection rates fluctuated temporally (peak: …

Seven decades of nontuberculous mycobacteria in Denmark: shifts in species distribution and clinical relevance.

The global burden of human disease caused by nontuberculous mycobacteria (NTM) is steadily increasing. Using our unique historical collectio … notable changes in prevalence over time at the single species level were observed for M. avium and Mycobacterium abscessus. Further genomic analyses of historical Mycobacterium sp. isolates revealed several uncommon or previously unrecognized species rarely detected in current diagnostic settings. In conclusion, our findings suggest a significant shift in the spectrum o …

Genomic insights into persistent infections, reinfections, and subspecies diversity of Mycobacteroides abscessus: A whole-genome sequencing study of Thai and global isolates.

Mycobacteroides abscessus is a highly resistant pathogen with significant genetic diversity and complicating clinical management. This study used whole-genome sequencing to analyze genomic variations in seven serially collected isolates from three Thai patients, distinguishing between persistent infections and reinfections. Comparative analysis with 43 global isolates revealed subspecies-specific genetic diversity and distribution patterns. Among Thai isolates, two paired samples (P7.1-P7.2 and P15.1-P15.2) were persistent infections (same clone), while two (P8.1-P8.2 and P15.2-P15.3) were rei

Redesign of Rifamycin Antibiotics to Overcome ADP-Ribosylation-Mediated Resistance.

Rifamycin antibiotics are a valuable class of antimicrobials for treating infections by mycobacteria and other persistent bacteria owing to their potent bactericidal activity against replicating and non-replicating pathogens. However, the clinical utility of rifamycins against Mycobacterium abscessus is seriously compromised by a novel resistance mechanism, namely, rifamycin inactivation by ADP-ribosylation. Using a structure-based approach, we rationally redesign rifamycins through strategic modification of the ansa-chain to block ADP-ribosylation while preserving on-target activity. Validate…

Investigating bacterial infections in Galleria mellonella larvae: Insights into pathogen dissemination and behavior.

The insect Galleria mellonella is an alternative animal model widely used for studying bacterial infections. It presents a wide range of advantages, including its low cost, easy maintenance and lack of ethical constraints. Among other features, their innate immune system is very similar to that of mammals. In this study, we dissected several larvae infected with important human pathogens: Mycobacterium abscessus, Staphylococcus aureus and Pseudomonas aeruginosa. By observing the fat body, gut, trachea, and hemolymph under the microscope, we were able to describe where bacteria tend to dissemin…

Isolated subcutaneous nontuberculous mycobacterial infection: a rare case initially mischaracterized as a soft tissue malignancy.

The occurrence of nontuberculous mycobacterial (NTM) infection is rare, and the involvement of the musculoskeletal system is even less common. However, the incidence of soft tissue and skin NTM infection is increasing, particularly in patients who undergo injections and minor surgical procedures. Given the non-specific clinical manifestations of NTM infection, the lack of knowledge among physicians regarding this rare infection could lead to inaccurate and delayed diagnosis. Herein, we present a case of an isolated subcutaneous NTM infection caused by Mycobacterium abscessus in the upper back …

Mucoid Pseudomonas aeruginosa-derived alginate modulates innate immune transcriptional responses and promotes intracellular growth of Mycobacterium tuberculosis and non-tuberculous mycobacteria.

Pulmonary dysbiosis has been increasingly associated with tuberculosis susceptibility, relapse, and treatment failure; however, the mechanis … heat-killed mucoid P. aeruginosa significantly enhances intracellular growth of M. tuberculosis and Mycobacterium abscessus, compared with the non-mucoid phenotype. Exogenous alginate reproduced these effects in a dose- and time-dependent manner without directly promoting mycobacterial growth. Vacuolar-ATPase inhibitors are antimicrobial agents active against intracellular mycobacteria.

Mycobacterium tuberculosis (Mtb) evades host defenses by inhibiting phagosome acidification in part through the secreted phosphatase PtpA, binding to the vacuolar ATPase (v-ATPase), and disrupting downstream cellular events. We investigated the antimicrobial effects of three v-ATPase inhibitors, Bafilomycin A1 (BafA1), Bafilomycin D, and Cladoniamide B (ClaB), on the growth of Mtb, M. abscessus (Mabs), and M. bovis BCG in THP-1 and murine infection models. We found potent inhibition of intracellular growth with MIC50 in the nanomolar range, with compounds showing a bacteriostatic inhibition of

HelD is a global transcription factor enhancing gene expression in rapidly growing mycobacteria.

HelD protein, also named HelR (encoded by MSMEG_2174 in Mycobacterium smegmatis), interacts with mycobacterial RNA polymerase (RNAP) and affects rifampicin resistance in Mycobacterium abscessus. Here, we provide data on rifampicin resistance and helD presence in the genomes of other clinically relevant nontuberculous mycobacteria. We show that helD is primarily found in rapidly growing mycobacteria, such as M. smegmatis, where we detected HelD at a subset of promoters that can also associate with CarD and RbpA. Transcriptome analysis of a helD deletion strain revealed that HelD has a dual role…
