Recurrent inflammatory myofibroblastic tumors (IMTs) of bladder managed with transurethral resection; case report.

INTRODUCTION AND IMPORTANCE: Bladder IMTs are uncommon subtypes of sarcoma arising from mucosal epithelium and mesentery. They can occur various organs like eyes, nose, mouth, digestive tract, lungs, and genital and urinary tracts [1]. Typically benign, these tumors can occasionally exhibit local aggression. Diagnosis challenging due to its pathology confusion with bladder sarcomas, immunohistochemical analysis may be necessary to differentiate [6]. Here we report a 52 years old lady with recurrent bladder IMT who was treated with transurethral resection of bladder tumor(TURBT). This case repo

Crocetin Attenuates Fibroblast-to-Myofibroblast Transition after Myocardial Infarction in Mice by Regulating Autophagy.

PURPOSE: Myocardial infarction (MI) triggers pathological remodeling characterized by fibrosis and subsequent fibroblast-to-myofibroblast transition (FMT). Autophagy exerts a dual role in cardiac repair, balancing protective and detrimental effects. Crocetin, a carotenoid derived from saffron, exhibits anti-inflammatory and cardioprotective properties; however, its influence on autophagy-related FMT following MI remains unclear. This study aims to determine whether crocetin mitigates post-MI fibrosis and FMT by modulating autophagy in cardiac fibroblasts (CFs), while also evaluating its effects

Inflammatory myofibroblastic tumor of female genital tract with unusual features and potential diagnostic pitfalls.

BACKGROUND: Inflammatory myofibroblastic tumor (IMT) of the female genital tract is frequently misdiagnosed as uterine leiomyoma or mesenchymal stromal tumors due to overlapping morphological features. MATERIALS AND METHODS: A total of 25 cases of IMT were collected between 2019 and 2024 and 8 of them were classified as IMT with unusual features. We conducted a retrospective analysis of 8 cases of unusual features IMT, focusing on histopathological features, immunophenotype, molecular alterations, and clinical follow-up. RESULTS: The misdiagnosis rate is as high as 62.5%. Among these, 3 pati

Case Report: First case of low-grade myofibroblastic sarcoma of the vulva during pregnancy.

Low-grade myofibroblastic sarcoma (LGMFS) of the vulva is exceptionally rare, with only two prior cases reported. We present the third documented case globally and the first occurring during pregnancy, highlighting diagnostic and therapeutic challenges in this unique clinical scenario. A 34-year-old woman presented with a recurrent vulvar mass initially misdiagnosed as angiomyofibroblastoma. The lesion reappeared asymptomatically during pregnancy and was conservatively managed with ultrasound surveillance, followed by term cesarean delivery to mitigate perineal trauma risks. Postpartum evaluat

Diagnostic challenges and clinical management of inflammatory myofibroblastic tumor: a retrospective case series study.

BACKGROUND: Inflammatory myofibroblastic tumor (IMT) is a relatively rare mesenchymal tumor in clinical practice and is difficult to diagnose. At present, clinical reports on this disease are still relatively scattered. Especially for IMT occurring in special sites such as the nasal cavity, sacrum, and bladder, systematic reports on its clinical manifestations, treatment responses, and prognostic characteristics are still lacking. CASE DESCRIPTION: This study retrospectively reported six patients with low-grade IMT diagnosed by pathology (three males and three females), aged 15 to 63 years (w

A Rare Case of Inflammatory Myofibroblastic Tumor Mimicking Fibrous Adhesions Resulting in Bowel Obstruction.

Inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal tumors of unknown etiology composed of myofibroblastic cells admixed with inflammatory cells. Presented is a 72-year-old male hospitalized for severe abdominal pain and hematochezia with onset of associated symptoms of fever and sweats a few hours prior to abdominal pain. A computed tomography (CT) demonstrated left colonic thickening interpreted as partial obstruction, gross adhesions, and ischemia. At surgery, marked bowel ischemia from the distal transverse to proximal sigmoid colon was seen with extensive gross adhesions. Histo

Inflammatory myofibroblastic tumor of the cecum presenting as acute abdomen, a rare pathology with a rarer presentation.

Inflammatory myofibroblastic tumor is an extremely rare neoplastic lesion with a predilection for aggressive local and recurrent behavior. The tumor tends to occur in the lungs of children and young adults, and although it can develop in older patients and other organs, this is extremely rare. Symptoms are nonspecific and depend on the location and size of the tumor. The gastrointestinal tract is rarely this mass's primary site of origin, and the cecum is an even rarer location. We present the case of an otherwise healthy 55-year-old female who presented with an acute abdomen and a mass in her

Gα12 and Gα13 proteins are required for transforming growth factor-β-induced myofibroblast differentiation.

Myofibroblast differentiation, characterized by accumulation of cytoskeletal and extracellular matrix proteins by fibroblasts, is a key process in wound healing and pathogenesis of tissue fibrosis. Transforming growth factor-β (TGF-β) is the most powerful known driver of myofibroblast differentiation. TGF-β signals through transmembrane receptor serine/threonine kinases that phosphorylate Smad transcription factors (Smad2/3) leading to activation of transcription of target genes. Heterotrimeric G proteins mediate distinct signaling from seven-transmembrane G protein coupled receptors, which ar

Massive Hemorrhage Following Spontaneous Mediastinal Inflammatory Myofibroblastic Tumor Rupture: A Case Report.

A 45-year-old male in a hypertensive emergency was admitted with complaints of frontal headache, progressive chest discomfort, shortness of breath, dysphagia, and right upper quadrant abdominal pain radiating across the epigastrium and to the back that increases in intensity with deep inspiration. He denied any history of abdominal pain, vomiting, dyspnea, nausea, and weight loss. A computed tomography (CT) scan of the chest showed a posterior mediastinal mass between the esophagus and descending aorta. A magnetic resonance imaging (MRI) scan revealed a non-enhancing posterior mediastinal mass

PCSK9 Inhibition Regulates Infarction-Induced Cardiac Myofibroblast Transdifferentiation via Notch1 Signaling.

Increasing evidence suggests that PCSK9 inhibition protects cardiomyocytes against ischemia-reperfusion injury after myocardial infarction. However, it is not clear whether PCSK9 inhibitor (PCSK9i) affects cardiac fibroblasts (CFs) activation after MI. In this study we used SBC-115076, an antagonist of PCSK9, to investigate the role of PCSK9i in the conversion of CFs to cardiac myofibroblasts (CMFs) after MI and provided a basic for its clinical application in cardiac fibrosis after MI. In vivo study, PCSK9i was injected into mice 4 days after MI. Cardiac function and degree of fibrosis were e…

Intracranial Inflammatory Myofibroblastic Tumor: A Literature Review and a Rare Case Misdiagnosed as Acoustic Neuroma.

Inflammatory myofibroblastic tumor (IMT) stands as a rare neoplasm, initially documented by Bahadori and Liebow in 1973; however, its biological behavior and underlying pathogenesis continue to elude comprehensive understanding. Throughout the years, this tumor has been designated by various alternative names, including pseudosarcomatoid myofibroblastoma, fibromyxoid transformation, and plasma cell granuloma among others. In 2002, the World Health Organization (WHO) officially classified it as a soft tissue tumor and designated it as IMT. While IMT primarily manifests in the lungs, the common

Inflammatory myofibroblastic tumor of colon and duodenum: Metachronous, metastatic or local recurrence? An unusual presentation.

Inflammatory myofibroblastic tumor (IMT) is a tumefactive proliferation of spindled myofibroblastic cells admixed with inflammatory infiltrate. This tumor has a predilection for the involvement of visceral soft tissues and has a tendency for local recurrence. Occurrence of metastatic or metachronous IMTs is a rare presentation. We report a rare case of a 50-year-old man with metastatic IMTs in the colon and duodenum.

Extracellular matrix stiffness controls cardiac valve myofibroblast activation through epigenetic remodeling.

Aortic valve stenosis (AVS) is a progressive fibrotic disease that is caused by thickening and stiffening of valve leaflets. At the cellular level, quiescent valve interstitial cells (qVICs) activate to myofibroblasts (aVICs) that persist within the valve tissue. Given the persistence of myofibroblasts in AVS, epigenetic mechanisms have been implicated. Here, we studied changes that occur in VICs during myofibroblast activation by using a hydrogel matrix to recapitulate different stiffnesses in the valve leaflet during fibrosis. We first compared the chromatin landscape of qVICs cultured on so…

ROS1-positive Inflammatory Myofibroblastic Tumor of the Small Bowel Causing Obstruction: A Case Report.

Inflammatory Myofibroblastic Tumors (IMTs) are rare fibroblastic/myofibroblastic neoplasms that affect predominately pediatric patients and young adults. Almost half of the patients with IMTs have a chromosomal abnormality in the Anaplastic Lymphoma Kinase 1 gene on chromosome 2p23. Although these tumors occur primarily in the lung, lesions have been reported in a variety of intra-abdominal organs like the liver, spleen, and mesentery. Small bowel IMTs are particularly rare. IMTs generally pursue a benign clinical course, however intra-abdominal and retroperitoneal tumors have typically shown

Unusual case of a right atrial inflammatory myofibroblastic tumor presenting with an episodic cyanosis in an infant.

Cardiac inflammatory myofibroblastic tumor (IMT) is a rare benign tumor that occurs mainly in infants and children. We report a case of right atrial IMT in a 1-month-old girl with episodic cyanosis. Both echocardiography and cardiac computed tomography revealed a right atrial mass. The patient underwent a complete surgical resection of the mass and the postoperative histological examination confirmed the diagnosis of cardiac IMT. The patient recovered well after the operation and further follow-up evaluation was uneventful.
