Spino-bulbo-spinal pathway mediating vagal modulation of nociceptive-neuroendocrine control of inflammation in the rat.

Stimulation of nociceptors by intradermal capsaicin produces depression of bradykinin (BK)-induced synovial plasma extravasation (PE) that is markedly enhanced by subdiaphragmatic vagotomy. This depression is mediated by the adrenal medullae, a propriospinal pathway between the afferent nociceptive input and preganglionic neurones projecting to the adrenal medullae, and a spino-bulbo-spinal pathway. Here we investigated the role of spinal ascending and descending pathways in the interaction between noxious and vagal afferent inputs, leading to inhibition of BK-induced PE mediated by the adrena

The involvement of nervous and some inflammatory response mechanisms in the acute snuff-induced gingival hyperaemia in humans.

BACKGROUND AND AIM: Tobacco users and especially cigarette smokers are at higher risk than non-smokers for periodontal disease. The pathogenic mechanism has been proposed to be the vasoconstrictive properties of nicotine, with reduced gingival blood flow (GBF) as a contributing factor in the development of periodontal disease. However, in a previous study in humans, we found GBF to increase in response to acute exposure to snuff. The present study was designed to investigate whether the tobacco-induced acute GBF increase is dependent on intact nervous conduction. We further investigated the ef

Long-term foot outcomes following differential abatement of inflammation and osteoclastogenesis for active Charcot neuroarthropathy in diabetes mellitus.

AIMS: Inflammatory osteolysis is sine-qua-non of active Charcot neuroarthropathy (CN) causing decreased foot bone mineral density (BMD) and fractures. We aimed to explore the effect of anti-inflammatory or anti-resorptive agents for effect on foot bone mineral content (BMC) and consequent long-term outcomes of foot deformities, fractures and amputation. METHODS: Forty-three patients with active CN (temperature difference >2°C from normal foot) were evaluated. Patients were off-loaded with total contact cast and randomized to receive either methylprednisolone (1gm) (group A), zoledronate (5mg)

Inhibition of phosphodiesterase IV enzyme improves locomotor and sensory complications of spinal cord injury via altering microglial activity: Introduction of Roflumilast as an alternative therapy.

Despite the great search for an effective approach to minimize secondary injury in spinal cord injury (SCI) setting, there have been limited advances. Roflumilast is a selective inhibitor of phosphodiesterase 4 with potent anti-inflammatory properties. Here, we sought to explore Roflumilast efficacy in the improvement of locomotor and sensory deficits of SCI. In an animal setting, 50 male rats were randomly assigned to five groups: an SCI group receiving Placebo, three SCI groups receiving Roflumilast at the doses of 0.25, 0.5, and 1 mg/kg prior to T9 vertebra laminectomy, and a sham-operated

Transient polyuria related to central diabetes insipidus caused by lymphocytic infundibulo-neurohypophysitis in a patient treated for Graves' disease.

A 45-year-old man was hospitalized because of weight loss, finger tremor, thirst, polydipsia and increased urinary frequency. He was diagnosed with Graves' disease (GD) and central diabetes insipidus (CDI). Magnetic resonance imaging revealed the enlarged posterior pituitary with thickened stalk. Histological examination obtained from biopsy of the pituitary revealed lymphocytic infundibulo-neurohypophysitis. He received treatment with thiamazole (MMI) for GD and desmopressin acetate (DDAVP) for CDI. However, DDAVP administration could be discontinued as GD was gradually improved. This course

Effect of nitric oxide on substance P release from the peripheral endings of primary afferent neurons.

To elucidate the interaction between nitric oxide (NO) and substance P (SP) in neurogenic inflammatory responses, we studied the effect of drugs related to nitric oxide (NO) on the levels of Evans blue and SP released into perfusate from the subcutaneous space in the rat instep. Noxious heat stimulation (47 degrees C for 30 min) caused an increase in SP release in parallel with plasma extravagation. N(omega)-nitro-L-arginine methylester (L-NAME: 100 mg/kg, once daily) given intraperitoneally (i.p.) five times (chronic treatment) significantly suppressed the heat-evoked SP release and Evans blu

TRPV1-linked neuropeptide axes in periodontitis and peri-implantitis.

Periodontitis and peri-implantitis are the two most prevalent destructive diseases encountered in clinical dentistry, both of which arise fr … larization through the cAMP/PKA/CREB signaling cascade, whereas SP is more commonly associated with neurogenic inflammation and osteoclast activation in inflammatory settings, although both axes exhibit context-dependent effects. Special emphasis is placed on the impaired neural innervation of peri-implant tissues resulting from the absence of the periodontal ligament, a …

[Transvaginal/transumbilical hybrid NOTES appendicectomy : Comparison of techniques in uncomplicated and complicated appendicitis].

BACKGROUND: Appendicectomy (AE), the most frequent emergency surgical procedure, can be performed as a transvaginal hybrid natural orifice translumenal endoscopic surgery (NOTES) technique (TVAE). The question of feasibility also arises in cases of advanced inflammation with perforation. MATERIAL AND METHODS: Since May 2012 all female patients with suspected acute appendicitis were offered a TVAE as an alternative to the standard procedure. Preoperative, intraoperative and postoperative parameters were registered prospectively. RESULTS: Until October 2013 a total of 13 TVAEs had been perform

Familiality analysis of provoked vestibulodynia treated by vestibulectomy supports genetic predisposition.

BACKGROUND: Provoked vestibulodynia is a poorly understood disease that affects 8-15% of women in their lifetime. There is significant inflammation and nerve growth in vestibular biopsies from affected women treated by vestibulectomy compared with matched female population controls without vestibulodynia. The triggers leading to this neurogenic inflammation are unknown, but they are likely multifactorial. OBJECTIVE: Our objective was to determine whether vestibulodynia is more common in close and distantly related female relatives of women diagnosed with the disease and those specifically tre…

Rotator Cuff Disorders: Practical Recommendations for Conservative Management Based on the Literature.

Conservative management of rotator cuff disorders remains challenging, with no comprehensive, evidence-based framework integrating diagnosis, prognosis, rehabilitation, and biological therapies. Existing recommendations usually address isolated components of care, leading to inconsistent treatment strategies. This article proposes a global, pragmatic protocol for the non-surgical management of rotator cuff lesions, from initial assessment to long-term follow-up. Drawing on clinical expertise supported by recent literature, we outline a stepwise approach that begins with a comprehensive diagnos

Lipocalin-2 links spinal cord injury to neurogenic lung injury through MAPK/ERK-ferroptosis signaling: Preliminary evidence for a spinal cord-lung axis.

BACKGROUND: Lung injury secondary to acute spinal cord injury (ASCI) is a critical pathological basis for respiratory complications such as pneumonia and respiratory failure, which are major contributors to early mortality and poor outcomes in affected individuals. Despite its clinical relevance, the underlying mechanisms linking ASCI to pulmonary dysfunction remain poorly understood. In this study, we evaluated the role of Lipocalin-2 (LCN2) in ASCI-induced lung injury and investigated the potential involvement of the LCN2/MAPK/ERK signaling axis in this process. METHODS: A mouse model of AS

Resveratrol Alleviates Neurogenic Bladder Fibrosis and Urothelial Pyroptosis by Inhibiting the AKT/mTOR Signaling Pathway.

Neurogenic bladder (NB) is a debilitating urological disorder characterized by progressive bladder remodeling, fibrosis, and functional deterioration, yet effective mechanism-based therapies remain limited. The AKT/mTOR signaling pathway is a central regulator of cellular growth, inflammation, and tissue remodeling, but its contribution to NB pathogenesis remains incompletely understood. In this study, we established an NB rat model by bilateral L6-S1 spinal nerve transection and evaluated the therapeutic effects of resveratrol. Urodynamic and histological analyses showed that resveratrol redu

Sesquiterpene Lactone Derivatives of Ambrosia artemisiifolia Act as Agonists of the Transient Receptor Potential Ankyrin 1 Ion Channel.

Background: Transient receptor potential ankyrin 1 (TRPA1) and vanilloid 1 (TRPV1) are key nociceptive ion channels involved in chemosensation, pain signaling, and neurogenic inflammation. Several sesquiterpene lactones target TRPA1, but the activity profile of these secondary metabolites of the common ragweed (Ambrosia artemisiifolia) remains uncharacterized. Purpose: The purpose of this study was to explore the activity of five ragweed sesquiterpene lactones on TRPA1 and TRPV1, respectively. Methods: We investigated the effects of five ragweed-derived sesquiterpene lactones-acetoxydihydrodam…

The active role of pulmonary immunity in ischaemic stroke.

Previous stroke studies have mostly regarded the lungs as passively damaged target organs, and in clinical practice in the context of stroke, the role of the lungs has been habitually limited to secondary complications such as stroke-associated pneumonia or neurogenic pulmonary oedema. However, recent studies have indicated that the lung plays a more active and central role in the pathophysiology of stroke. In this review, we propose and systematically elaborate on an emerging concept: the lung, as one of the largest immune organs in the body, functions as an active initiator and amplifier of

NGF, BDNF, and NO in Myopic Subjects: Relationships Between Aqueous Levels and Lens Epithelial Cells' Activation.

Several soluble mediators are activated during myogenesis and progression, and severe neurodegeneration, with related biomarkers, characterizes high myopia-related retinal atrophy. Targets of oxidative stress, epigenetics and neurogenic inflammation have been reported in the prospecting of some bioindicators to mirror retinal insults occurring in high myopia. The aim of the present study was to assess the expression of a few selected biomarkers belonging to the neurotrophin (NGF and BDNF), oxidative (NO, KEAP1/NRF2), and epigenetic (DNMT3 and HD1) pathways. Sixty-five (65; 76.25 ± 9.40 years) …

Organ crosstalk: brain-lung interaction.

The interaction between the brain and the lungs is bidirectional: ICU patients with acute brain injury develop pulmonary complications, while ARDS patients frequently manifest neurological sequelae. Research is indeed focusing on both aspects of this cross-talk. On one side, ARDS survivors experience poor neurological outcomes both in the short and long term, with high incidence of delirium and post- discharge neurocognitive impairment. The underlying mechanisms have been investigated either in the pre-clinical and in the clinical field. Ventilator associated brain injury is the new recent ter

Pediatric Neurogenic Pulmonary Edema After Brain Tumor Removal Complicated with Severe Myocardial Injury: A Case Report.

BACKGROUND Neurogenic pulmonary edema (NPE) is a rare complication of neurological insults, such as traumatic brain injury and intracranial hemorrhage, in children. NPE frequently accompanies left ventricular (LV) dysfunction mediated via central catecholamine surge and inflammation. A high serum natriuretic (BNP) level was prolonged even after the LV contraction was improved in this case with severe myocardial injury. The overloading stress to the LV wall can last several days over the acute phase of NPE. CASE REPORT A 6-year-old boy developed NPE after the removal of a brain tumor in the cer

Severe Acute Transverse Myelitis With a Rapid Progression in an Infant.

Acute transverse myelitis (ATM) is a rare inflammatory disorder of the spinal cord, often triggered by infections, systemic diseases, or occurring without a known cause (idiopathic). It is a diagnosis of exclusion, characterized by motor, sensory, and autonomic dysfunction, with spinal cord inflammation confirmed through cerebrospinal fluid analysis or MRI. While most children experience a favorable recovery, severe cases can lead to lasting neurological deficits. This report presents the case of a 10-month-old male who presented with severe para-infectious ATM, likely triggered by Campylobact
