Monitoring scoliosis and other spinal deformity surgeries.

Surgery to correct a spinal deformity incurs a risk of injury to the spinal cord and roots. Injuries include postoperative paraplegia. Surgery for cervical myelopathy also incurs risk for postoperative motor deficits, as well as nerve injury most commonly at the C5 root. Risks can be mitigated by monitoring the nervous system during surgery. Ideally, monitoring detects an impending injury in time to intervene and correct the impairment before it becomes permanent. Monitoring includes several modalities of testing. Somatosensory evoked potentials measure axonal conduction in the spinal cord pos

Auditory hallucinations: phenomenology, neuropsychology and neuroimaging update.

Auditory verbal hallucinations (AVHs) are a cardinal feature of psychosis. Recent research is reviewed which has attempted to advance our knowledge of the mechanisms underlying this symptom. Phenomenological surveys have confirmed the importance of the content of such hallucinations and their meaning to the voice-hearer. Psychological and neuroimaging studies of inner speech and source monitoring have provided a neuropsychological framework for AVHs as well as some novel therapeutic strategies. There have also been successful attempts to 'capture' neural activity coincident with the experience

Evoked potential tests in clinical diagnosis.

BACKGROUND: Evoked potentials are used to detect conduction disturbances in the central nervous system. This paper provides an overview of the areas in which evoked potentials are used in clinical neurophysiological diagnostics, with the emphasis on coma and demyelinating disease. METHOD: The article is based on a literature search in PubMed and the authors' long experience of neurological and neurophysiological diagnostics. RESULTS: Somatosensory evoked potential (SEP) can be a reliable predictor of failure to regain consciousness as early as 24 hours after anoxic coma has occurred. If coma

Early High-Frequency Network Reorganization After Spinal Cord Stimulation Is Linked to Long-Term Recovery in Disorders of Consciousness.

AIMS: Spinal cord stimulation (SCS) is a promising treatment for disorders of consciousness (DoC), but early neurophysiological markers of durable recovery remain unclear. We aimed to determine whether source-space EEG network energy and communication features capture early brain-network reorganization linked to 6-month recovery in chronic DoC. METHODS: Thirty-one chronic DoC patients undergoing cervical SCS completed resting-state EEG before stimulation and at 1 week and 1 month after stimulation onset. Source-space EEG was analyzed across delta, theta, alpha, and beta bands. NetPower quanti

Biomarkers of Subjective Tinnitus Presence and Clinical Burden: A Systematic Review.

Despite being common and clinically burdensome, subjective tinnitus is diagnosed and characterised primarily through subjective reporting. Objective biomarkers could transform tinnitus care by enabling reproducible identification, stratification, and monitoring, but their clinical utility for detecting tinnitus and quantifying its severity remains unclear. We conducted a PRISMA-guided systematic review of tinnitus biomarkers, critically evaluating their clinical and diagnostic relevance, methodological quality, and readiness for clinical translation. PubMed, Scopus, and Web of Science were sea

Chronic triventricular hydrocephalus: morphological and functional study before and after ventriculoperitoneal shunting.

We have neuroradiologically and neuropsycho-physiologically studied a case of chronic hydrocephalus due to congenital acqueductal stenosis, before and after ventriculoperitoneal shunting. The aims of the study were: 1) to point out the correlation between neuroimaging (Computerized Tomography/Magnetic Resonance Imaging) and mapping of Computerized Spectral Analysis EEG (CSA-EEG); 2) to monitor evoked bioelectrical activity by neurophysiological evaluation of acoustic and visual N1 and P3. Both bioelectrical activity and neuroimaging evaluation showed a considerable recovery of the neurobiologi

False negative intraoperative neuromonitoring alerts during pediatric spinal deformity surgery: the dreaded outcome.

INTRODUCTION: Intraoperative neuromonitoring (IONM) reduces postoperative spinal cord dysfunction during pediatric spinal deformity surgery by allowing intraoperative corrective actions. Currently, data on false negative IONM events in this population are limited. The purpose of this study is to describe false negative IONM cases and explore immediate and final outcomes. METHODS: An institutional neuromonitoring database (November 1992-April 2024) was reviewed to identify patients (0-18 years) who underwent spinal deformity surgery and experienced false negative IONM. The modalities included

Outcomes of Immediate Total Thyroidectomy in First-Side Loss of Neuromonitoring Signal.

IMPORTANCE: Use of intraoperative neuromonitoring (IONM) during thyroidectomy can nearly eliminate the risk of postoperative bilateral vocal cord palsy (VCP) by indicating staged surgery in cases of loss of signal (LOS) on the first side of planned total thyroidectomy. However, aborting planned total thyroidectomy may lead to persistence of symptoms, delay in adjuvant treatment, and patient inconvenience and distress. There are few data to guide a selective approach to total thyroidectomy in patients with first-side LOS. OBJECTIVE: To investigate outcomes of immediate bilateral surgery in pat

Cognitive inflexibility and heightened error monitoring are related to lower sexual functioning.

Sexual functioning is an important predictor of well-being and relationship satisfaction. Previous research indicates that several aspects of cognitive function are related to sex-related behaviors and functioning among individuals with sex-related disorders, neurological disorders, and in older adults; however, this has been relatively underexamined in younger populations. To examine this, the present study assessed whether behavioral and/or neurophysiological measures of cognitive function are associated with sexual functioning in a community sample of young 489 adults (64 % female) ages 18-

The effects of anxiety, depressive, and obsessive-compulsive subclinical symptoms on performance monitoring.

Dysfunctional performance monitoring has been proposed as a potential neurocognitive biomarker of various internalising psychopathological symptoms, such as anxiety, depressive, and obsessive-compulsive symptoms. Given the overlapping neurophysiological findings and high rates of comorbidity amongst these internalising symptoms, an important research gap pertains to the specificity of performance monitoring to each of these symptoms. The aim of the present study was to examine the effects of anxiety, depressive, and obsessive-compulsive symptoms on performance monitoring in healthy adults. The

Behavioral and neurophysiological evidence for the enhancement of cognitive control under dorsal pallidal deep brain stimulation in Huntington's disease.

Deep brain stimulation of the dorsal pallidum (globus pallidus, GP) is increasingly considered as a surgical therapeutic option in Huntington's disease (HD), but there is need to identify outcome measures useful for clinical trials. Computational models consider the GP to be part of a basal ganglia network involved in cognitive processes related to the control of actions. We examined behavioural and event-related potential (ERP) correlates of action control (i.e., error monitoring) and evaluated the effects of deep brain stimulation (DBS). We did this using a standard flanker paradigm and eval

Risk factors for neurophysiological events related to intraoperative halo-femoral traction in spinal deformity surgery.

PURPOSE: This study identifies risk factors for neurophysiological events caused by intraoperative halo-femoral traction (IOHFT) in patients with adolescent idiopathic scoliosis (AIS), and neuromuscular scoliosis (NMS). METHODS: Neurophysiological integrity was monitored using motor evoked potentials (MEPs). IONM event was defined as a decreased MEP amplitude of more than 80% of baseline in, at least, one muscle. Time between application of IOHFT and event, affected muscles, surgical stage, and time between removal of IOHFT and recovery of MEPs were described. Characteristics (age, height, we

Clinical and Neurophysiological Changes after Targeted Intrathecal Injections of Bone Marrow Stem Cells in a C3 Tetraplegic Subject.

High-level quadriplegia is a devastating condition with limited treatment options. Bone marrow derived stem cells (BMSCs) are reported to have immunomodulatory and neurotrophic effects in spinal cord injury (SCI). We report a subject with complete C2 SCI who received three anatomically targeted intrathecal infusions of BMSCs under a single-patient expanded access investigational new drug (IND). She underwent intensive physical therapy and was followed for >2 years. At end-point, her American Spinal Injury Association Impairment Scale (AIS) grade improved from A to B, and she recovered focal pr

Feasibility of Using Intraoperative Neuromonitoring in the Prophylaxis of Dysesthesia in Transforaminal Endoscopic Discectomies of the Lumbar Spine.

(1) Background: Postoperative nerve root injury with dysesthesia is the most frequent sequela following lumbar endoscopic transforaminal discectomy. At times, it may be accompanied by transient and rarely by permanent motor weakness. The authors hypothesized that direct compression of the exiting nerve root and its dorsal root ganglion (DRG) by manipulating the working cannula or endoscopic instruments may play a role. (2) Objective: To assess whether intraoperative neurophysiological monitoring can help prevent nerve root injury by identifying neurophysiological events during the initial plac…
