A recurrent melanocytic nevus phenomenon in the setting of Hailey-Hailey disease.

Atypical acquired melanocytic nevi in patients with epidermolysis bullosa (EB) have been referred to as EB nevi and are considered to be a type of recurrent nevus with atypical but distinctive histopathologic findings. Herein, we describe an atypical nevus in a patient with Hailey-Hailey disease with different histopathologic findings from EB nevi because of presumably different pathogenesis. It is important to be aware that the recurrent nevi phenomenon can be seen in acantholytic conditions as well as blistering disorders, given these lesions may clinically resemble melanoma.

Large Congenital Melanocytic Nevus on the Breast Sparing the Nipple and Areola.

BACKGROUND: Divided nevus is a rare entity that has been described for a special appearance of congenital melanocytic nevi (CMNs) occurring on the eyelid and penis. It is presumed that the formation of divided nevi is related with the embryologic development of the eyelid and penis, thus giving a hint about the occurrence time of CMN in utero. This article focuses on a formerly undescribed observation of another special clinical appearance of CMN discussing possible relation with embryogenesis. METHODS: The data including photographic documentations of a total number of 86 patients with large…

Nevus Lipomatosis Cutaneous Superficialis - A clinicopathologic study of the solitary type.

BACKGROUND: Nevus Lipomatosis Cutaneous Superficialis (NLCS) is a rare, benign hamartomatous lesion characterized by the ectopic presence of mature adipocytes in the reticular dermis not associated with the underlying subcutaneous tissue. Two clinical forms-classical and solitary occur. The solitary form is relatively uncommon, due to which these lesions are commonly clinically misdiagnosed. The aim was to study the clinical and histopathological features of the solitary type of NLCS. METHODS: Seven cases of histopathologically documented solitary type of NLCS which presented at our institute…

Kissing nevus of the penis. Report of two cases and review of the literature.

Kissing nevus is a curious type of nevus that was first described on the eyelids and rarely described on the penis. We report two cases of kissing nevus of the penis and review previously reported cases. The lesions of the kissing nevus of the penis showed characteristic mirror-image symmetry relative to the coronal sulcus. On histopathology, the lesion showed a compound nevus.

Nevus of hoffmann-zurhelle: a case around the right parotid duct.

Nevus lipomatosus cutaneous superficialis (NLCS) is an extremely rare hamartomatous disorder comprising of ectopic mature adipose tissue. The lesions take the form of large, slow-growing, sessile or pedunculated tumors. We report the case of an 11-year-old boy with NLCS, dating back many months, that was growing as a cuff around the parotid duct. The lesion was successfully removed by diode laser excision (980 nm) and a diagnosis of NLCS was rendered. At one-year follow-up the tumor has not recurred. To our knowledge this is the first report of NLCS in the oral cavity; the characteristic clini…

Nevus lipomatosus superficialis unseen or unrecognized: a report of eight cases.

BACKGROUND: Nevus lipomatosus superficialis (NLS) is a unique developmental anomaly or nevoid form of lipoma characterized by the ectopic presence of mature adipocytes in reticular dermis. The condition is rare; apart frrom isolated case reports, there are no large case series dealing with the clinicoepidemiologic characteristics and posttreatment long-term follow-up in patients with NLS and little published information on treatment outcome. OBJECTIVE: To study the clinicoepidemiologic characteristics and long-term posttreatment follow-up in patients with NLS. METHODS: This was an 11-year re…

Analysis of mutations in the PIK3CA and FGFR3 genes in verrucous epidermal nevus.

Verrucous epidermal nevi are congenital hamartomas composed of keratinocytes and may occur alone or in association with developmental abnormalities. A close relationship between variations in the PIK3CA and FGFR3 genes and the appearance of nevi has been recently reported. Based on that, we performed molecular assays for the identification of E542K, E545G/K and H1047R mutations in the PIK3CA gene and of the R248C mutation in the FGFR3 gene. Interestingly, during the amplification process, we did not observe the PCR product of exon 9 of the PIK3CA gene, a region comprising amino acids 542-545.

Unilateral open-angle glaucoma associated with the ipsilateral nevus of ota.

The nevus of Ota also known as "congenital melanosis bulbi" and "oculodermal melanocytosis" is a blue-gray hyperpigmentation that occurs on the face and eyes. The sclera is involved in two-thirds of cases (causing an increased risk of glaucoma). Women are nearly five times more likely to be affected than men. It is rare among Caucasian people. The nevus of Ota is congenital or acquired. Most cases of the nevus of Ota are unilateral (90%), although pigmentation is present bilaterally in 5%-10%. Ocular abnormalities included pigmentation of the sclera, cornea, retina, and optic disc and cavernou…

Congenital combined melanocytic nevus of the scalp with associated alopecia areata.

We report a case of a 20-year-old Chinese man with an alopecic congenital combined compound and blue melanocytic nevus of the scalp, associated with alopecia areata. The diagnosis of a combined melanocytic nevus was confirmed by histopathological examination and immunohistochemical stains, with exclusion of neurocristic hamartoma, which can have a similar clinical and histopathological appearance but different prognosis. In addition, we explore the association of this large melanocytic lesion with alopecia areata.

Characteristics of spitzoid melanoma and clues for differential diagnosis with spitz nevus.

The different features of spitzoid melanoma are not well characterized in the literature, and the lesion often has to be described in comparison with Spitz nevus. We evaluated the histopathological appearance of spitzoid melanoma by reviewing all spitzoid melanomas treated at our hospital and all referrals from 1998 to 2010. The final study sample comprised 18 cases, 11 from our institution and 7 referrals from other centers. We recorded clinical parameters (eg, age, sex, site, time between onset and excision, recurrence, and death) and a series of histopathological parameters (eg, size, ulcer…

Impressive pseudotumoral proliferative nodules in a giant congenital nevus of a newborn.

The potential for malignant transformation of a giant congenital melanocytic nevus (GCMN), although rare, should be considered, especially in proliferative lesions that have nodules on their surface, as they can clinically and histologically mimic melanoma and generally have a poor prognosis. We present the case of a newborn boy with a giant nevus of uneven blackish color on the back with variable degrees of intensity of pigmentation, almost entirely covered by nodules of large and variable sizes, soft texture, and a tendency to ulcerate and bleed. The unusual clinical and histologic appearanc…

Inhibiting the hedgehog pathway in patients with the basal-cell nevus syndrome.

BACKGROUND: Dysregulated hedgehog signaling is the pivotal molecular abnormality underlying basal-cell carcinomas. Vismodegib is a new orally administered hedgehog-pathway inhibitor that produces objective responses in locally advanced and metastatic basal-cell carcinomas. METHODS: We tested the anti-basal-cell carcinoma efficacy of vismodegib in a randomized, double-blind, placebo-controlled trial in patients with the basal-cell nevus syndrome at three clinical centers from September 2009 through January 2011. The primary end point was reduction in the incidence of new basal-cell carcinomas …

Nevus/Melanocytoma/Melanoma: an emerging paradigm for classification of melanocytic neoplasms?

CONTEXT: Until recently, the prevailing paradigm in classification and clinical management of melanocytic proliferations mandated dichotomous classification of all melanocytic lesions as either entirely benign (nevus) or entirely malignant (melanoma). However, some diagnostically challenging lesions cannot be unequivocally classified as nevus or melanoma by histologic evaluation of the primary tumor. Such lesions have been referred to as borderline or melanocytic tumors of uncertain malignant potential. OBJECTIVE: To review and update the problem of diagnostically difficult melanocytic prolif…

Nevus trichilemmocysticus: report of a new case of a recently recognized entity.

A new type of organoid epidermal nevus, characterized by the presence of multiple trichilemmal cysts disposed in a linear arrangement, was recently recognized and named nevus trichilemmocysticus. We report a 10-year-old girl with a congenital linear dermatosis affecting the head, neck, trunk, limbs, palms, and soles. Initially, the lesions that followed the lines of Blaschko consisted of yellowish plaques covered by verrucous papules and filiform hyperkeratoses. Additionally, since the age of 6 years, numerous cyst-like structures developed on the linear lesions involving the limbs. On histopa…

Linear nevus sebaceous syndrome in a patient with atypical associated abnormalities.

A 3-year-old girl was diagnosed as having linear nevus sebaceous syndrome. The nevus sebaceous was located on the left side of the face and neck. An unusually large lacrimal gland was completely ectopic below the entire left conjunctiva, with secondary ectropion of the left upper eyelid. The patient had the unusual finding of dentigerous cysts in both maxillary sinuses. Computed tomography scan showed atrophy of the left cerebral hemisphere, accompanied by a history of seizures; furthermore, the patient was mentally retarded. This phenotypic pattern of linear nevus sebaceous syndrome is typica…

The anatomic distribution of melanoma and relationships with childhood nevus distribution in Colorado.

The nature of the relationship between nevus development in childhood and later development of melanoma is unclear. Data on melanoma diagnoses by histologic type and anatomic site were obtained for 2351 white, non-Hispanics in Colorado from the Colorado Central Cancer Registry between 2000 and 2004. Nevus size and body site were ascertained during skin exams conducted in the summer of 2007 on 717 white, non-Hispanic children aged 8-9 years. Chi-square goodness-of-fit analysis was used to assess the association between the anatomic site distributions of nevi versus melanoma. Superficial spreadi…
