A 2-year-old boy received, by mistake, 50 mg racemic adrenaline intravenously, equivalent to 1.8 mg kg-1 of L-adrenaline. The blood pressure increased to 160/105 mmHg, the heart rate to 160 beats min-1, and pulmonary oedema developed over the next 2 h. He was treated with nitroprusside, nitroglycerin and digitoxin, and was intubated and ventilated. After 3 h a hypotensive phase occurred which required infusions of very high concentrations of catecholamines for 72 h. Renal failure required renal transplantation after which the child made an uneventful recovery.

A case of reversible vasospasm is reported in a 54-year-old man with a closed bimalleolar ankle fracture. On admission the patient had normal distal pulses and laboratory studies. He was a heavy smoker who continued to smoke in the hospital. Deep venous thrombosis (DVT) prophylaxis included dihydroergotamine and heparin (DHE-H). BACKGROUND AND PURPOSE: Intracerebral haemorrhage volume (ICHV) is prognostically important but does not account for intracranial volume (ICV) and cerebral parenchymal volume (CPV). INTRODUCTION: Various triggers can originate a migraine attack. In healthy volunteers and patients with migraine, the nitroglycerin (NTG) provocation model induces a headache that resembles migraine in pain characteristics and vascular manifestations. This headache is reversible and treatable in monitored conditions, providing an opportunity to test novel antimigraine medications in early clinical development.

Nitromint aerosol has been used for controlling 464 attacks of 116 angina pectoris patients in emergency cases. The anti-anginous effect of the sublingual spray developed very rapidly and only caused mild-moderate unwanted effects, which commonly accompany the administration of nitroglycerin. In a comparison of the spray with the sublingual tablet, the majority of the patients found the aerosol more favourable. In addition to its anti-anginous action, the drug is an absolutely necessary means in emergency cases such as cardiac asthma and left ventricular failure.
