Out of town guest: A healthy 7 year old from a non-endemic area presents with histoplasmosis granulomatous disease.

Histoplasmosis is a common fungal infection, normally infecting people exposed to demolition sites or bat/bird droppings in the central and eastern states. When a child presents with a chest mass and eosinophilia in a non-endemic region the likelihood of an infectious process like pulmonary histoplasmosis is unknown. A seven year old immunocompetent child with a mediastinal mass and eosinophilia presented with acute cough, fever, non-bloody emesis, and four pound weight loss. A neoplastic work up was negative. Further evaluation showed a positive M band (chronic histoplasmosis infection) and n

Inter-rater Reliability in Videos of Patients With a Suspected Diagnostic of Autism and Child Psychosis.

INTRODUCTION: Diagnosing and treating autism and child psychoses is very difficult; these pathologies impact not only the child's life but also the family as a whole. Therefore caution is required when giving a diagnosis with prognostic implications. We use the psychodynamic perspective in order to take into consideration both pathological and healthy diagnostic aspects, and to promote evolutionary potential of each patient. OBJECTIVE: To determine the inter-rater reliability and to test quantitatively and qualitatively the variables involved in the diagnoses of patients with suspected autism

Making sense of children's sexual behavior in child care: An analysis of adult responses in special investigation reports.

This paper investigates how adults respond to sexual behavior among children in child care. Culturally, childhood sexuality is variously understood as natural curiosity, a sign of sexual abuse, or a symptom of a sex-offender in the making. Given these competing cultural meanings, how are sexual-like behaviors by children managed by the adults who care for them? An analysis of qualitative data from Special Investigation Reports by childcare licensing consultants in the state of Michigan is used to examine how parents, child care providers, and child care licensing consultants manage and respond

Embryo donation parents' attitudes towards donors: comparison with adoption.

BACKGROUND: Embryo donation produces a family structure where neither rearing parent is genetically related to the child, as in adoption. It is not known how embryo donation parents view the donors compared with how adoptive parents view the birth parents. METHODS: 21 couples with an embryo donation child aged 2-5 years were compared with 28 couples with an adopted child. Parents were administered a semi-structured interview, assessing knowledge of the donors/birth parents, frequency of thoughts and discussions about the donors/birth parents and disclosure of the donor conception/adoption to

Validity, reliability, and responsiveness of a new measure of health-related quality of life in children with immune thrombocytopenic purpura: the Kids' ITP Tools.

OBJECTIVE: To refine the disease-specific health-related quality of life measure in immune (idiopathic) thrombocytopenic purpura (ITP) and to determine its validity, reliability, and responsiveness to change. STUDY DESIGN: The initial phase involved cognitive debriefing of 12 families, on the basis of which the measure was modified and then named Kids' ITP Tools (KIT). The measure was administered on 2 occasions with the Pediatric Quality of Life Inventory (PedsQL) to 41 patients with acute ITP and 49 patients with chronic ITP, 2 to 18 years old, and their parents (proxy-respondents) at 6 Nor

The management of epiglottitis in a small paediatric intensive care unit.

The aim of this study was to examine the outcome of epiglottitis in a small paediatric institution. Over the 5 year period October 1986 to September 1991, 44 children (of whom 15 were retrieved) with epiglottitis were admitted to the only paediatric intensive care facility in the Hunter Region. The so-called classical features were often absent. Forty of the 44 children were intubated. The mean length of intubation was 22 h and the mean length of hospital stay was only 2 1/2 days. One child had a respiratory arrest prior to intubation but made a complete recovery; one child had a coexistent su

The influence of maternal long term health conditions including multimorbidity on child oral health: A scoping review and evidence gap map protocol.

OBJECTIVE: This scoping review will map the extent and type of evidence in relation to the association between maternal long term health conditions (LTCs), including multimorbidity, and child oral health. INTRODUCTION: Newer theories are emerging that detail the many factors that can influence child oral health at child, family and community levels. More recently, the association between maternal general health and child oral health has been explored, with preliminary evidence suggesting a link between shared environmental factors and diet/substance use during pregnancy causing childhood cari

Who Gets Protection? A National Study of Multiple Victimization and Child Protection Among Taiwanese Children.

This study aims to examine the prevalence of multiple types of child victimization and the effects of multiple types of victimization on children's mental health and behavior in Taiwan. The study also examines the child-protection rate and its correlates among children experiencing various types of victimization. This study collected data with a self-report questionnaire from a national proportionately stratified sample of 6,233 fourth-grade students covering every city and county in Taiwan in 2014. After calculating the 1-year prevalence of child victimization, the study found that bullying w

[Sex of rearing in 46,XY partial gonadal dysgenesis (case report)].

XY patients with gonadal dysgenesis present with a wide array of clinical pictures that is characterised by a variable incomplete virilisation of the external genital organs (female phenotype, posterior hypospadias, cryptorchidism) and the persistence of the internal müllerian duct structures. The birth of a child with this type of abnormality is a social phenomenon, which will probably have a physical and psychical impact. The choice of the sex depends on the organic anatomy, diagnosis age, risk of gonadal tumors and the development possibilities (mainly at puberty) of the child. The registra

Effect of peer administered consequences upon articulatory responses of speech-defective children.

Twenty second-grade lisping children were taught to articulate /s/ and /z/ correctly during three training sessions. Two weeks later, each child was asked to invite three classmates to a fourth session in which the classmates could earn tokens contingent upon the target child's correct articulation of /s/ and /z/ sounds. A response cost procedure was initiated with one group of 10 target children and the invited peers. In addition to tokens given for correct responses, tokens were removed following each of the target child's incorrect /s/ or /z/ responses. This condition resulted in the presen

Maternal metabolic syndrome in pregnancy and child development at age 5: exploring mediating mechanisms using cord blood markers.

BACKGROUND: There is limited evidence on how the classification of maternal metabolic syndrome during pregnancy affects children's developmental outcomes and the possible mediators of this association. This study uses a cohort sample of 12,644 to 13,832 mother-child pairs from the UK Born in Bradford Study to examine the associations between maternal metabolic syndrome classification (MetS) and child development outcomes at age 5, using cord blood markers as candidate mediators. METHODS: Maternal cardiometabolic markers included diabetes, obesity, triglycerides, high-density lipoprotein chole

Little Agreement on Treating Residual Bipolar Disorder Symptoms in a Child.

BACKGROUND: There is a paucity of studies on treatment of childhood-onset bipolar disorder and its associated comorbidities, which leads to a wide diversity of opinion on choice and sequencing of treatment options. METHODS: From December 2018 to January 2019, a graphic depiction of medications and weekly ratings of symptoms of mania, depression, anxiety, attention-deficit/hyperactivity disorder (ADHD), and oppositional behavior that parents had rated on their 9-year-old child over a period of several years was sent to experts in child and adult bipolar disorder.
