Statistical analysis of visual evoked potentials in optic neuritis and ischemic optic neuropathy subjects.

An effective and clinically relevant parameter for differentiating ischemic optic neuropathy (ION) and optic neuritis (ON) patients using the Visual Evoked Potentials (VEPs) has been identified. In this study, statistical analysis has been carried out on time domain parameters of the VEP waveforms recorded for ION (17 patients) and ON (35 patients) groups. The ratio of the amplitude of P100 component of affected eye to fellow eye showed no significant difference between the two groups. However, the mean P100 absolute latency component of ION patients was found to be significantly (p<0.001) sho…

Comparative studies on pattern VECP between patients with ischemic optic neuropathy and optic neuritis.

PURPOSE: To evaluate whether and how VECPs are useful in differentiating ION from ON. METHODS: We retrospectively studied 27 patients with non-arteritic ION and 69 patients with ON using pattern visually evoked cortical potentials (PVECPs) to 3 rev/sec stimulation (transient) and 12 rev/sec stimulation (steady-state). RESULTS: In patients with ION, non-recordable responses were found in 25% of those with transient VECP and in 74% of those with steady-state VECP. In ON patients, non-recordable responses were found in 34% of those with transient VECP and in 64% of those with steady-state VECP.

Long time echo stir sequence magnetic resonance imaging of optic nerves in optic neuritis.

Magnetic resonance imaging of optic nerves was obtained in 13 patients with acute optic neuritis and in 13 patients with a previous history of optic neuritis (ON), assessed by clinical, visual fields and visual evoked potentials evaluations. Results of the conventional short tau inversion recovery (STIR) sequence obtained with short time echo (STE-STIR: 22 ms) were compared with long time echo (LTE-STIR: 80 ms) sequence. The conventional STE-STIR sequence revealed lesions in 78.5% of acute ON and in 58.8% of optic nerves affected by previous ON. The LTE-STIR sequence was diagnostic in 92.8% of…

Detection of optic nerve lesions in optic neuritis using frequency-selective fat-saturation sequences.

MRI was performed on seven patients with acute optic neuritis, using two sequences which suppress the signal from orbital fat: frequency-selective fat-saturation and inversion recovery with a short inversion time. Lesions were seen on both sequences in all the symptomatic optic nerves studied.

Myelinotoxic activity on tadpole optic nerve of cerebrospinal fluid from patients with optic neuritis.

The myelinotoxic activity of unconcentrated cerebrospinal fluid (CSF) from eight optic neuritis (ON) and five multiple sclerosis (MS) patients with oligoclonal IgG, and from five ON patients without oligoclonal IgG, was tested in the tadpole optic nerve system. CSF from ON or MS patients with oligoclonal CSF IgG gave a significantly greater number of myelinotoxic lesions than did CSF from ON patients without oligoclonal CSF IgG, CSF from control patients, or physiologic saline. Induction of myelinotoxic lesions may be coupled with the presence of oligoclonal IgG. The findings support the hypot…

Aberrant static and dynamic brain functional topological organization in the differentiation of myelin oligodendrocyte glycoprotein antibody-seropositive optic neuritis from seronegative optic neuritis.

OBJECTIVE: An early and accurate diagnosis of myelin oligodendrocyte glycoprotein antibody seropositive optic neuritis (MOG-ON) versus seronegative-ON is critical for optimal management. We aimed to explore alterations in static and dynamic functional networks for differentiation by resting-state functional magnetic resonance imaging (RS-fMRI) with the graph theory method. METHODS: RS-fMRI was performed on 53 patients (23 with MOG-ON and 30 with seronegative-ON) and 26 healthy controls (HCs). Graph theory analysis was used to investigate the topological properties of the functional networks. …

The Acute Optic Neuritis Network (ACON): Study protocol of a non-interventional prospective multicenter study on diagnosis and treatment of acute optic neuritis.

UNLABELLED: Optic neuritis (ON) often occurs at the presentation of multiple sclerosis (MS), neuromyelitis optica spectrum disorders (NMOSD), and myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD). The recommended treatment of high-dose corticosteroids for ON is based on a North American study population, which did not address treatment timing or antibody serostatus. The Acute Optic Neuritis Network (ACON) presents a global, prospective, observational study protocol primarily designed to investigate the effect of time to high-dose corticosteroid treatment on 6-month …

Management of optic neuritis in Ireland: a survey comparing the management practices of acute demyelinating optic neuritis amongst ophthalmologists and neurologists in Ireland.

BACKGROUND: Acute optic neuritis (ON) is often the first manifestation of multiple sclerosis which is particularly common in Ireland. Despite the specific clinical details regarding investigations and management of ON provided by the Optic Neuritis Treatment Trial (ONTT), international surveys have shown that there are still notable differences in the management of ON between neurologists and ophthalmologists. AIM: To compare the investigation and treatment of acute optic neuritis between ophthalmologists and neurologists in Ireland METHOD: A survey consisting of a case scenario and questions…

Updates and Controversies in the Management of Acute Optic Neuritis.

Optic neuritis remains a common diagnosis with controversial management. Although typical optic neuritis is often associated with "good" recovery of visual acuity, patients are often left with persistent impairments of contrast sensitivity, color vision, and visual field. These permanent visual deficits correlate with structural injury to the anterior visual pathway and are closely linked to visual quality of life. High dose corticosteroids are commonly used for patients with acute optic neuritis. However, even several decades after the initial clinical trials, there remains significant contro…

A Comparison of Optic Neuritis in Asian and in Western Countries.

Acute demyelinating optic neuritis is a common optic neuropathy in young adults. There is usually satisfactory visual recovery. However, some patients convert to multiple sclerosis (MS) with potential sequelae of neurological disability. The Optic Neuritis Treatment Trial in the United States was conducted prospectively for 15 years and provided valuable data about clinical course, efficacy of steroid treatment, and risk of conversion to MS. Compared to the many studies in Western countries, research concerning optic neuritis in Asia has so far not been extensive. However, cumulative evidence …

Optic neuritis associated with adalimumab in the treatment of uveitis.

BACKGROUND: Tumor necrosis factor (TNF) inhibitors such as adalimumab are increasingly used in the treatment of ocular inflammatory disease refractory to standard therapies. There is evidence that TNF inhibitors are associated with demyelinating neurologic events. OBJECTIVE: The authors report a case of optic neuritis and multiple sclerosis developing in association with the use of adalimumab for the treatment of bilateral chronic granulomatous iridocyclitis and multifocal choroiditis. They also review the available literature on cases of optic neuritis occurring in the setting of TNF inhibit…

[Bartonella henselae as a cause of optical nerve neuritis].

INTRODUCTION: Bartonella henselae is included into the group of gram-negative bacteria that can cause not so rare disease known as cat-scratch disease (CSD). This disease is characterized by the specific general symptoms, and the complications in the eyes can be manifested in the form of neuroretinitis, follicular conjunctivitis and focal chorioretinitis. In this paper clinical and epidemiological characteristics of a patient with ophthalmologic complications caused by Bartonella henselae are described. We indicate diagnostic possibilities and the criteria for making the diagnosis. CASE REPOR

[Thermography in the diagnosis of vascular neuritis of the optic nerve].

A new method for diagnosing vascular neuritis of optic nerve by means of thermography is proposed. Among 47 patients with diagnosis of optic nerve neuritis of unclear etiology, in 37 of them vascular genesis of the disease was diagnosed. The thermograms have shown appearance of dark "cold" spots and a fall of temperature by 1.0 degrees C and lower in the supraorbital and orbital areas of the affected eye. The method is highly precise (90.27%, sensitive (94.9%) and doesn't require long time.
