Molding helmet therapy in the treatment of brachycephaly and plagiocephaly.

The purpose of this study was to compare the use of molding helmet therapy in the treatment of positional brachycephaly and posterior positional plagiocephaly. Four hundred twenty-eight children with positional brachycephaly or plagiocephaly were included in this study. In this group of patients, 132 (32%) were treated with positioning alone. Of the 292 (68%) patients who were treated with molding therapy, 64 (21.9%) were treated for positional brachycephaly and 248 (78.1%) were treated for posterior positional plagiocephaly. All children were evaluated by a craniofacial surgeon and a pediatri

Use of an ankle-foot orthosis in a dog with traumatic sciatic neuropathy.

Traumatic unilateral sciatic neuropathy secondary to a gunshot wound was diagnosed in a seven-month-old, intact male golden retriever. Findings on neurological assessment, performed after cardiovascular stabilisation, were incompatible with a return to normal limb function. An ankle-foot orthosis was used to correct excessive flexion of the digits and tarsus. This device greatly improved the dog's willingness to ambulate and could serve as an alternative to amputation in companion animals with severe sciatic axonotmesis or neurotmesis. Complications associated with chronic tibiotarsal hyperfle

Biomechanical evaluation of the efficacy of external stabilizers in the conservative treatment of acquired flatfoot deformity.

This study quantified and compared the efficacy of in-shoe orthoses and ankle braces in stabilizing the hindfoot and medial longitudinal arch in a cadaveric model of acquired flexible flatfoot deformity. This was addressed by combining measurement of hindfoot and arch kinematics with plantar pressure distribution, produced in response to axial loads simulating quiet standing. Experiments were conducted on six fresh-frozen cadaveric lower limbs. Three conditions were tested: intact-unbraced; flatfoot-unbraced; and flatfoot-braced. Flatfoot deformity was created by sectioning the main support st

Development of computer-based environment for simulating the voluntary upper-limb movements of persons with disability.

Upper-limb orthotic systems have been designed for restoring the upper-limb functions of individuals with disabilities resulting from spinal cord injury (SCI), stroke and muscular dystrophy. These systems employ either functional electrical stimulation or external power. It is proposed that, instead of time-consuming and complicated monitoring using sensors and motion analysis, a software simulator with both angular displacement and acceleration parameters can facilitate the design of a control strategy for an orthosis. Reaching movements of three cervical SCI subjects are used to verify the s

Examination and management of a patient with tarsal coalition.

The purpose of this case report is to illustrate how the literature can be used to guide clinical decisions related to a relatively uncommon pathological condition of the foot. This case report describes the approach used to examine and treat a 14-year-old boy referred by a physician for physical therapy with a diagnosis of peroneal spastic flatfoot (PSFF). Peroneal spastic flatfoot is a syndrome typically characterized by limited tarsal joint motion, a clonus response of the evertors, and a pes planus deformity. The patient reported having a limp for several years, but he said he was pain-fre

Comparison of a stiff and a spring-type ankle-foot orthosis to improve gait in spastic hemiplegic children.

The effect of two different types of functional ankle-foot orthosis on the gait of patients with spastic hemiplegia was studied. A gap was cut into the conventional stiff orthosis to allow a limited dorsiflexion of 10-15 degrees, while plantarflexion was blocked (spring type). This gap was bridged by an aluminum bar to stiffen the orthosis for the experiment. Fourteen patients of different ages (6.5-20.1 years) walked barefoot and with the orthosis, once springy and once stiff. Kinetics and kinematics were analyzed. Gait was significantly improved into normal pattern by using any type of ankle

Multistep measurement of plantar pressure alterations using metatarsal pads.

Metatarsal pads are frequently prescribed for nonoperative management of metatarsalgia due to various etiologies. When appropriately placed, they are effective in reducing pressures under the metatarsal heads on the plantar surface of the foot. Despite the positive clinical reports that have been cited, there are no quantitative studies documenting the load redistribution effects of these pads during multiple step usage within the shoe environment. The objective of this study was to assess changes in plantar pressure metrics resulting from pad use. Ten normal adult male subjects were tested du

Randomized, controlled trial of compression gloves in rheumatoid arthritis.

A randomized, controlled trial was conducted to determine efficacy of the Isotoner and the Futuro compression gloves in 39 patients with hand synovitis secondary to rheumatoid arthritis. After 7 days of nighttime treatment, both brands of gloves were found to decrease subjective symptoms of pain and stiffness (p < 0.05). In addition, swelling of the proximal interphalangeal joints decreased, while range of motion, rate of finger motion, and grip strength all increased (p < 0.05). Compression gloves should be used with caution in patients with bilateral carpal tunnel syndrome, because 12 of 13

[Long-term results of congenital dislocation of the hip treated with Pavlik harness].

We have studied congenital dislocation and subluxation of the hip reduced by Pavlik harness with long-term roentgenographic results and prognostic factors. Eighty dislocations and thirty-five subluxations were assessed by X-ray. Average age at final follow-up was 17 years (14 to 23 years). Anatomical healing (Group I of Severin's classification) was 61.2% in dislocation and 71.5% in subluxation. Avascular necrosis of the femoral head was 7.0% (8 hips) and deformity was mild. Acetabular dysplasia appeared to be an important factor in determining the prognosis. In cases with both satisfactory an

[Three-dimensional CT study of the carpus under pronation-supination constraints].

By studying 3D imaging of the wrist under pronation-supination strain, we found that the simple comparison of a series of two corresponding cuts may provide a great deal of useful information on how the carpus transmits the longitudinal torque from the forearm to the hand. A special wooden trestle was made to fix the subject in the CT scanner in a permanent effort of pronation or supination. In the first group of scans, this effort was said to be "free" because the hand was simply maintained in a fixed window without any muscular contraction, except pronation or supination muscles. In the seco

Prevention of ankle sprains.

Two different methods for the prevention of ankle joint injuries in soccer were tested. Coordination training on an ankle disk improves functional stability and postural control, whereas an orthosis provides mechanical support. Both techniques reduce the frequency of ankle sprains in soccer players with previous ankle problems. The orthosis is an alternative to taping, and can be used during the rehabilitation period after injury or when playing on uneven ground. Coordination training on an ankle disk ought to be included in the rehabilitation of ankle injuries to prevent functional instabilit

[Legg-Perthes-Calvé disease. Plea for an original ambulatory orthosis].

Legg-Perthes-Calvé disease is a local disease which should be treated by non-iatrogenic means. The authors have designed a new ambulatory orthosis allowing containment in abduction, flexion and medial rotation, with relief of weight-bearing on the femoral head. It was tested in thirty six hips. In only eight of them a surgical procedure had to be performed during the evolution of the disease (femoral or pelvic osteotomy) because of secondary exenteration of the hip.

Basic considerations for shoes, shoe modifications, and orthoses in foot care.

It is important to remember that appropriate padding and materials for the foot should be selected on the basis of indications and functional needs of the patient. It is important to remember that foot wear, as protection and foot covering, needs to be compatible with the foot and the patient's functional requirements. The foot, orthosis, and shoe must function as a unit and as part of the patient's ambulatory system. I have attempted to identify some of the basic considerations involved in shoe selection, shoe modification, and orthotic selection for patient's rehabilitation. This article is

Hypermobile flatfoot in children. Comprehension, evaluation, and treatment.

Comprehension of the hypermobile flatfoot deformity depends on an understanding of the three structural deformities of the foot associated with the condition: (1) abduction of the forefoot relative to the weight-bearing line; (2) supination of the forefoot; and (3) heel valgus. Evaluation of the hypermobile flatfoot deformity is by clinical examination to determine the position of the foot with the talonavicular joint in the congruous position: position and motion of these components are determined from this starting point. A standing lateral talometatarsal angle is used to evaluate the deform

Congenital absence of the radius: the occupational therapist and a new orthosis.

Congenital absence of the radius (radial club hand) is a relatively common and disabling limb deficiency characterized by dislocation of the hand and carpus from the single forearm bore, the ulna. An understanding of the clinical-pathologic anatomy and consequent disruption of hand mechanics enables the orthopedic surgeon and the occupational therapist to formulate a team approach to this birth defect that can result in maximum functional capability for the child. The role of the occupational therapist and a specific therapy program for early stimulation and achievement of hand function are de
