Temporal bone histopathology of Paget's disease with sensorineural hearing loss and narrowing of the internal auditory canal.

A temporal bone report is presented of a patient with progressive sensorineural hearing loss and advanced Paget's disease involving the skull. The histopathology reveals Pagetic bone invading the internal auditory canal and compressing the cochlear division of the VIIIth cranial nerve resulting in severe neural degeneration. There are no other histopathologic abnormalities present in the cochlea to explain the hearing loss. A low-frequency conductive hearing loss was also present, but no histopathologic correlate could be identified.

Clinical use of immunosuppressive drugs: part II.

In haematological diseases, insufficient data has been accumulated to evaluate the efficacy of immunosuppressive drug treatment in patients with erythroid aplasia or sideroblastic anaemia. Cyclophosphamide may be efficacious in inhibiting circulating anticoagulants in patients who need continued replacement of clotting factors. Azathioprine, 6-mercaptopurine, cyclophosphamide and vincristine have been used successfully in treating patients with idiopathic thrombocytopenic purpura, and some patients with auto-immune haemolytic anaemia may benefit from the addition of purine analogues. However,

HEAT STABILITY OF HUMAN PLACENTAL ALKALINE PHOSPHATASE.

Alkaline phosphatase prepared from human placentae shows greater resistance to heat inactivation than any other known alkaline phosphatase of human origin. In the presence of magnesium this enzyme may be heated at 70 degrees C. for 30 minutes without loss of activity whereas other human alkaline phosphatases lose most of their activity on being heated at 56 degrees C. for this period of time. This heat stability is seen in freshly prepared enzyme, in alcohol-fractionated and freeze-dried material, and in the sera of individuals into whom placental alkaline phosphatase has been infused. The cli

The role of autophagy in bone metabolism and clinical significance.

The skeletal system is the basis of the vertebral body composition, which affords stabilization sites for muscle attachment, protects vital organs, stores mineral ions, supplies places to the hematopoietic system, and participates in complex endocrine and immune system. Not surprisingly, bones are constantly reabsorbed, formed, and remodeled under physiological conditions. Once bone metabolic homeostasis is interrupted (including inflammation, tumors, fractures, and bone metabolic diseases), the body rapidly initiates bone regeneration to maintain bone tissue structure and quality. From bone alterations to tumours: Genetic drivers linking Paget's disease of bone to cancer.

Paget's disease of bone (PDB) is a metabolic disorder characterized by abnormal bone remodelling and a strong genetic component. Several genes are involved, with distinct genetic backgrounds correlating with specific clinical manifestations. Mutations in SQSTM1 are most commonly associated with classical PDB, while alterations in ZNF687 and PFN1 define rarer, more aggressive variants that may progress to bone tumours such as giant cell tumour and osteosarcoma. Furthermore, mutations in VCP and TNFRSF11A are linked to syndromic forms where PDB coexists with other clinical features. This review

Treatment of Paget's disease of bone.

Paget's disease of bone is the paradigm of bone focal distortion with accelerated bone turnover. Over the years, a number of different drugs have been used to control its activity but, since biphosphonates were introduced for the treatment of the disease, they have become the preferred treatment. This review will update the therapeutic indications, available drugs and therapeutic response monitoring.

A tale of three diseases of the bone.

Paget's disease is a relatively rare disorder of the bone with only a few reports and case series observations from India. Hypocalcaemia is rare in Paget's disease, usually occurring as a consequence of therapy with bisphosphonates. We report a 65-year-old woman with Paget's disease who had hypocalcaemia secondary to vitamin D deficiency. On further evaluation, she also had severe osteoporosis. How vitamin D deficiency affects the diagnosis and monitoring of Paget's disease and the relationship between the three diseases are discussed. This case illustrates an interesting situation with abnorm

Placement and restoration of dental implants in a patient with Paget's disease in remission: literature review and clinical report.

Paget's disease is a disorder of bone that results in the replacement of normal bone with highly vascularized, low density bone. The diminished bone quality associated with Paget's disease is a relative contraindication to the use of dental implants, as it interferes with achievement of osseointegration. There is no current literature on the success of dental implants in patients with Paget's disease. Furthermore, there is no current literature on dental implant success in patients with remissive Paget's disease or in bone that appears to be unaffected by the disease in patients with Paget's.

Osteoclast diseases.

Osteoclasts are the only cells capable of resorbing mineralised bone, dentine and cartilage. Osteoclasts act in close concert with bone forming osteoblasts to model the skeleton during embryogenesis and to remodel it during later life. A number of inherited human conditions are known that are primarily caused by a defect in osteoclasts. Most of these are rare monogenic disorders, but others, such as the more common Paget's disease, are complex diseases, where genetic and environmental factors combine to result in the abnormal osteoclast phenotype. Where the genetic defect gives rise to ineffec

Treatment of bone diseases with bisphosphonates, excluding osteoporosis.

The main biologic action of bisphosphonates consists of the inhibition of osteoclastic bone resorption, and, at least, for the drugs introduced after etidronate, without any significant inhibition of bone mineralization. Bisphosphonates therefore play a major role in conditions that are characterized, at least partly, by an increased bone resorption. Primary and secondary osteoporosis by far constitute the most widespread indications for bisphosphonates, mostly because recent published trials have demonstrated their ability to prevent fractures. Potentially crippling conditions such as symptom

Decreased beta-isomerization of the C-terminal telopeptide of type I collagen alpha 1 chain in Paget's disease of bone.

In Paget's disease of bone, the normal lamellar bone is replaced by a woven structure with an irregular arrangement of collagen fibers. In this study, we investigated whether the degree of beta-isomerization within C-telopeptide of alpha 1 chain of type I collagen was altered in Paget's disease compared with other bone diseases with no alteration of bone structure. In Paget's disease (n = 26), but not in patients with primary hyperparathyroidism (n = 6) or hyperthyroidism (n = 17), the urinary excretion of nonisomerized (alpha) fragments derived from degradation of type I collagen C-telopeptid

Pamidronate disodium and possible ocular adverse drug reactions.

Ciba-Geigy Central Epidemiology and Drug Safety Center has received 23 reports of suspected ocular adverse drug reactions associated with the use of intravenous pamidronate disodium, an inhibitor of bone resorption that is used primarily in the management of tumor-induced hypercalcemia and Paget's disease of the bone. Anterior uveitis, bilateral in six of seven patients, occurred within 24 to 48 hours after the drug was administered. The anterior uveitis recurred in four of the five patients who were rechallenged. Three reports involved unilateral episcleritis or scleritis occurring within one

Clinical significance of free plasma hydroxyproline measurement in metabolic bone disease.

Free hydroxyproline was measured in plasma of 67 normal subjects and in 70 patients with bone disease including primary hyperparathyroidism (n = 19), osteoporosis (n = 18), Paget's disease (n = 14), cancer involving bone (n = 8), chronic renal failure (n = 6), and osteomalacia (n = 6), and osteomalacia (n = 5). A good correlation was found between plasma and urinary values of the amino acid in normal subjects (r = 0.66; p less than 0.001). In patients with skeletal disorders a highly significant direct correlation was observed between free plasma hydroxyproline on the one hand and urinary hydr

Hydrocephalus-dementia-complex in Paget's disease.

The clinical triad of gait disturbances, memory impairment and urinary incontinence is associated with a communicating hydrocephalus in the 'normal pressure hydrocephalus'-syndrome. The authors present a case of H.D.C. (hydrocephalus-dementia-complex) in Paget's disease with an identical syndrome, but with obstructive hydrocephalus, causing a triventricular dilatation. Today, each case of mental deterioration in Paget's disease, should be immediately observed and neurosurgical intervention kept in view. An X-ray of the skull, a CT-scan of the brain and a cisternography are performed as routine
