OBJECTIVES: In this study, we aimed to evaluate changes in frontal plane (anatomical axis) and sagittal plane (tibial slope, patellar height) parameters following closed-wedge high tibial osteotomy (HTO) and possible correlations between them.
PATIENTS AND METHODS: Between June 2003 and May 2007, 15 knees of the 13 female patients (mean age 52.6 years; range 45 to 64 years) who were followed on a regular basis in our outpatient clinic and underwent closed-wedge HTO were included. The mean follow-up was 49 months (range 29-75 months). Radiologic analysis was performed using pre- and postoperat

BACKGROUND: This retrospective study reported on the treatment of extension contracture of the knee after surgical treatment of patellar fractures.
METHODS: Twenty-eight patients who sustained extension contractures of the knee after surgical treatment of patellar fractures 8-14 months previously (mean, 12 months) were treated. A midline longitudinal approach was made. After implants were removed, adhesions between the vastus intermedius and the femur, and in the patellofemoral joint were released completely. The knee was flexed up to 110 degrees. Postoperatively, a continuous passive motion

Acute patellar dislocation (PD) is usually a problem of adolescents and young adults. In most cases, it is a sports-related injury. It is the result of an indirect force on the knee joint, which leads to valgus and external rotation of the tibia relative to the femur. PD is unlikely to occur on a knee with normal patellofemoral joint (PFJ) anatomy. Acute PD consists of an acute injury of the ligamentous medial patellar stabilizers in the background of factors predisposing to patellar instability. These factors are classified into three groups. The first group refers to the integrity of the lig

This is a case report of a 15-year-old male patient who presented with bilateral tibial tubercle avulsions and a unilateral complete patellar tendon stripping following a fall on both knees while playing football. The left knee was treated conservatively, while the right underwent open reduction internal fixation and patella tendon repair. The case was followed for one year postoperatively. Complete soft tissue and fracture healing were reported bilaterally. A modified Lysholm score of 84 compared to 95 pre-injury and a Tegner score of four compared to nine pre-injury were recorded. This case

BACKGROUND: Isolated chondral fractures of the knee are a rare and challenging problem, typically occurring with an acute traumatic event such as dislocation of the patella or ligamentous injury. Historically, repair of unstable chondral fragments without osseous attachment has not been recommended due to concerns about the limited healing potential of cartilage.
PURPOSE: To describe a technique for fixation of large isolated chondral fractures of the knee and present 3 cases where large chondral fragments without osseous attachment were fixed successfully with chondral darts and biologic adh

A new surgical technique to uniformly harvest the middle one-third of the patellar tendon for anterior cruciate ligament reconstruction is described. The technique uses a guide system of 8 templates of varying widths to guide the saw cuts and "collared," depth-controlled saw blades and drill bits to produce a standard cutting and drilling depth. This kind of template system lessens the risks of graft harvesting complications such as patella fracture, bone graft fragmentation, suture pull out, and judgment errors pertaining to graft depth, length, or width. The Patellar Tendon Graft Guide was u…

Fifty-nine fresh osteochondral allografts were consecutively transplanted into the knees of fifty-eight patients. The preoperative diagnoses were chondromalacia or degenerative arthritis of the patella, osteochondritis dissecans, a traumatic defect or osteonecrosis of the femoral condyle, a painful healed depressed fracture or traumatic defect of the tibial plateau, and unicompartmental traumatic arthritis of the knee. All of the patients had disabling pain after the failure of previous attempts to correct the problem surgically. Thirty-nine patients (forty knees) were available for follow-up

Purpose: Floating knee (FK) injuries are complex high-energy traumas associated with poor functional outcomes. This study aimed to identify … sor mechanism injury (OR 2.04), vascular injury (OR 4.89), intra-articular extension (OR 1.92), and patella fracture (OR 1.76) as independent predictors of poor functional outcome. The FKS-score, ranging from 0 to 15, demonstrated high predictive accuracy (AUC = 0.89). An optimal cut-off value of ≥9 points yielded a sensitivity of 78% and specificity of 85%. Concl …

INTRODUCTION: Chondral defects can be a particularly challenging clinical presentation. Because damaged cartilage does not heal itself, an evolution of multiple new surgical treatments designed specifically for cartilage repair and restoration have been introduced. Internal fixation has demonstrated good benefits in case reports and small qualitative studies. Our systematic review aimed to evaluate the effectiveness of internal fixation in the management of acute chondral fractures.
METHODS: A comprehensive search strategy was carried out using the NICE Healthcare Databases Advanced Search. T

OBJECTIVES: Chondral fractures are focal cartilage lesions without osseous attachment, most commonly seen in adolescent knees. They have limited capacity for intrinsic healing and traditional treatment has been removal of loose fragments. However, case reports of successful healing after fixation indicate that repair of the joint surface is possible. We wanted to evaluate the outcome in a cohort of patients who underwent fixation of acute chondral fractures in the knee.
DESIGN: Patients treated with fixation of a chondral fracture in the knee at our institution were invited to participate in

INTRODUCTION: Soft tissue swelling after acute fracture surgery is a challenge which may increase wound dehiscence, delay early range of motion, and increase infection rate postoperatively. This study investigates closed incision negative pressure therapy (ciNPT) using wide-range cover over the incision site and the peripheral swelling trauma zone to promote early active motion and to mitigate joint stiffness, bulla formation, and tendon adhesion.
METHODS: Twenty-nine patients were enrolled between January 2018 and December 2018. Patients with high-energy soft tissue trauma and comminuted fra

A modification of Maquet's procedure, longitudinal proximal tibial osteotomy, was performed on 36 adults under the age of 40. The tibial tubercle was elevated between 2.0 and 2.5 cm. Fourteen were operated on for significant osteochondral injury to the underside of the patella, including patellar fracture; 16 were operated on because of osteoarthrotic change in the patellofemoral joint secondary to recurrent subluxation. Six were operated on because of pain persisting after previous patellectomy. Follow-up ranged between 2 and 5 years, was at least 2 years, and averaged 3.53 years. A good resu

We present a rare occurrence of popliteal vascular injury due to blunt trauma. The patient had an isolated blunt lower extremity trauma. The patient subsequently experienced moderate tenderness and non-expanding hematoma at the popliteal fossa, reduced range of motion at the knee, and diminished distal pulses. X-rays showed a patella dislocation and tibial plateau non-displaced fracture but no knee dislocation. CT angiography showed an abrupt non-opacification of the distal portion of the popliteal artery with an overlying large hematoma. Surgical exploration was performed which revealed a con

The medial patellofemoral ligament is the primary static restraint to lateral patellar translation. It is injured in 96% to 100% of patellar dislocations that affect approximately 6 to 29 of 100,000 patients and is more common in patients younger than 20 years of age. Risk factors for patellar dislocation include patella alta, trochlear dysplasia, genu valgus, increased Q angle, and hyperlaxity. The treatment for patellar instability depends on the clinical and radiographic findings and can be nonoperative for first-time dislocations (bracing, proximal strengthening, and progressive return to

OBJECTIVES: The suprapatellar approach for medullary nailing of the tibia is increasing. This requires intra-articular passage of instruments, which theoretically places the knee at risk of postoperative sepsis in the setting of open fracture. We hypothesized that the risk of knee sepsis is similar after suprapatellar or infrapatellar nailing of open tibia fractures.
DESIGN: Retrospective, multicenter.
SETTING: Three urban level 1 trauma centers.
PATIENTS: All patients treated with medullary nailing for open diaphyseal tibia fractures (OTA 42) from 2009 to 2015. Patients younger than 18 yea
