Interventricular cardiac resynchronization with right ventricular apical pacing in a patient with Ebstein's defect.

We present the case of a patient with Ebstein's defect surgically corrected, and a complete right bundle branch block (RBBB) documented on echocardiogram. After an episode of near syncope due to a high-grade atrioventricular (AV) block, the patient was assisted with a bicameral DDDR pacemaker implanted with traditional right ventricular apical pacing. After the DDDR, and after stimulation with an AV delay of 180 ms, a narrow QRS complex was observed. Meanwhile, the typical left bundle branch block morphology of the right ventricular pacing and the native RBBB morphology were missing. The QRS c

Non-right-handedness is associated with migraine and soft bipolarity in patients with mood disorders.

BACKGROUND: There is a substantial body of data showing differences in the functioning of the two hemispheres in unipolar depressive and bipolar disorders. Migraine is a frequent co-morbid disorder in these patients, and it has been proposed that migraine may be associated with left-handedness. It would therefore be interesting to study migraine and handedness in a population of patients with mood disorders. METHODS: A total of 201 patients with an index episode of either major depression or mania were interviewed with a semi-structured interview based partly on DSM-IV criteria and partly on

Mirizzi syndrome in a patient with anomalous connection of the cystic duct into the right hepatic duct.

In 1948, Mirizzi described a syndrome characterized as the obstruction of the common hepatic duct by a stone located in the gallbladder's neck or in the cystic duct. We present a rare case of Mirizzi syndrome resulting from a fistula involving the cystic duct, the neck of the gallbladder, and the right hepatic duct. This finding was possible because the patient had a rare biliary anatomic variation: the insertion of the cystic duct straight in the right hepatic duct. The diagnosis was suggested by abdominal ultrasonography and confirmed by endoscopic retrograde cholangiopancreatography. The su

Contrecoup haemorrhage in a patient with left pubic fracture but right obturator artery bleeding.

Contrecoup injury following head trauma is well known. It is caused by the acceleration-deceleration mechanism that can be fully explained by Newton's first law of motion. We report on a victim of a motor vehicle accident with non-displacement left pubis fracture but haemorrhage from the right obturator artery. Contrecoup haemorrhage should be excluded first in unstable patients without evidence of significant trauma but with a minor pelvic fracture.

Right ventricular pacing and the risk of heart failure in implantable cardioverter-defibrillator patients.

BACKGROUND: Right ventricular (RV) pacing in implantable cardioverter-defibrillator (ICD) patients may have detrimental effects on morbidity and mortality, in particular by inducing heart failure (HF). OBJECTIVE: We investigated whether RV pacing increases the risk of HF in an asymptomatic ICD population. METHODS: We evaluated all patients without symptomatic HF who received an ICD. The primary endpoint was the occurrence of HF, which was defined as new HF, hospitalization for HF, or death due to HF. The secondary endpoint was appropriate shocks. RESULTS: The study population consisted of 4

Percutaneous transluminal coronary angioplasty by right transradial approach in a patient with arteria lusoria.

Though the radial artery is an ideal site of access for coronary interventions, at times anatomical variations make this procedure complicated. We describe one such arterial aberrancy, the retroesophageal right subclavian artery or arteria lusoria, in which the right subclavian artery arises distal to the left subclavian artery. Therefore, approaching through the right radial artery, the catheter has to take a zigzag course through the right subclavian to the descending aorta, and then to the ascending aorta. This often makes it difficult to perform diagnostic angiography. Performing angioplas

[Coronary artery bypass grafting in a patient with right aortic arch; report of a case].

A 60-year-old man was admitted to another hospital because of chest oppression on effort. Chest X-ray showed radiographic evidence of a right aortic arch and double vessel coronary artery disease with 50% stenosis in the left main trunk was diagnosed by coronary angiography. He was transferred to our institute for surgical treatment of angina pectoris. Preoperatively, multi-detector row computed tomography (CT) was performed and it revealed a right aortic arch and an aberrant left subclavian artery with narrow left internal thoracic artery. A right internal thoracic artery was well demonstrate

Changes in anthropometric values of paired craniofacial measurements of patients with right coronal synostosis.

Quantitative changes of the 5 paired (right and left) projective linear measurements of the craniofacial complex were analyzed by anthropometric methods in 26 patients (9 males and 17 females) with right coronal synostosis, 16 in infancy (0-5 and 6-12 months) and 10 in early childhood (1-10 years). Measurements were taken from 3 regions: the supraorbital rim length of the head (t-g), the length (en-ex) and height (ps-pi) of the eye fissures in orbits, and the width (pra-pa) and length (sa-sba) of the ears. None of the patients had undergone corrective surgery.Anthropometric analysis of patient

Unusual presentation of a patient with arrhythmogenic right ventricular dysplasia treated with a Glenn shunt.

Clinically, arrhythmogenic right ventricular dysplasia (ARVD) usually presents with ventricular arrhythmias, and unusual presentations were reported as acute coronary syndrome, heart failure and electrical storm. Taking all this different presentations and treatments in to account, we report a case of ARVD presenting with central cyanosis and clubbing simulating congenital heart disease. Besides this unusual presentation, the patient underwent also an unusual operation for this kind of abnormality, which cured the cyanosis completely.

Right ventricular function after pulmonary valve replacement in patients with tetralogy of Fallot.

PURPOSE: To assess the time course of right ventricular (RV) function improvement after pulmonary valve replacement (PVR) in patients 25.2 years +/- 7.0 after repair of tetralogy of Fallot. MATERIALS AND METHODS: The medical ethics committee approved this study, and informed consent was obtained. Cardiac magnetic resonance (MR) imaging was performed before, 7 months after, and 19 months after PVR in 25 consecutive patients with tetralogy of Fallot with a 1.5-T MR imager. RV function was assessed with gradient-echo sequences in the short-axis plane. Pulmonary flow was assessed with a velocity-

Volume reduction of the right anterior limb of the internal capsule in patients with schizotypal disorder.

We have previously reported bilateral volume reductions in the anterior limb of the internal capsule (ALIC) in patients with schizophrenia. The purpose of this study was to extend the volumetric measurements of ALIC to subjects with schizotypal features to explore the neurobiology underlying schizophrenia-spectrum disorders in view of the fronto-thalamic connectivity. Three-dimensional magnetic resonance images were acquired from 24 patients with schizotypal disorder (ICD-10) and 47 healthy volunteers matched for age, gender, handedness, and parental education. Volumetric analyses of the ALIC

Electroanatomic mapping of right atrial activation in patients with and without paroxysmal atrial fibrillation.

Inter-atrial conduction delay in patients with atrial fibrillation (AF) has been reported. However, the area of this conduction delay has not been well identified. The activation time and conduction velocity over the right atrial endocardium were evaluated during sinus rhythm using the CARTO mapping technique in 6 patients with paroxysmal AF (AF group) and 11 patients without history of AF (control group). No significant differences were observed between the 2 groups in the mean activation times and conduction velocities from the earliest activation site to the superior septum, His bundle area

Implantation of a biventricular pacing system in a patient with a coronary sinus not communicating with the right atrium.

Intubation of the coronary sinus failed during implantation of a biventricular pacing system. An angiogram of the left coronary artery showed in the venous phase a coronary sinus not communicating with the right atrium. The coronary sinus was draining into a persistent left superior vena cava communicating with the left subclavian vein. The coronary sinus lead was successfully implanted through the persistent left superior vena cava, whereas the atrial and ventricular leads were implanted through the right superior vena cava in a conventional way.
