Patient safety in the rehabilitation of children with spinal cord injuries, spina bifida, neuromuscular disorders, and amputations.

Pediatric patient safety continues to challenge both pediatricians and pediatric physiatrists. While there is a trend toward developing general patient safety initiatives, there is little research on pediatric patient safety. This article identifies major areas of general safety risk, with a focus on timely diagnosis and care coordination to prevent secondary complications that compromise health, function, and quality of life in pediatric neuromuscular disease, spinal cord disorders, and amputation.

Early patient experience with an electro-anatomic navigation system dedicated to device lead implantation: feasibility and safety.

INTRODUCTION: Fluoroscopy-guided pacing lead placement has well-recognized limitations and risks. We studied the safety and feasibility of using a novel electromagnetic navigation system specifically designed to guide pacemaker and implantable cardioverter defibrillator lead placement. METHODS: Twenty-four patients (mean age of 54±34 years) underwent the study protocol; 16 before electrophysiology study and eight before device implantation. The navigational deflectable sheath assembly was introduced via the subclavian vein and advanced to seven prespecified targets within the right heart cham

Safety of mixture of morphine with ketamine for postoperative patient-controlled analgesia: an audit with 1026 patients.

BACKGROUND: Adding ketamine to morphine for patient-controlled analgesia (PCA) may be useful. However, data on this drug combination have been collected on small sample sizes. In order to evaluate the safety of the combination morphine- ketamine, we conducted a prospective study on a large patient population. METHODS: Patient-controlled analgesia was performed with 1026 patients using morphine and ketamine in a dose ratio of 1:1. All patients were treated in the ward. Prospectively collected data included incidence of complications and side-effects, verbal pain scores at rest and during mobil

Violence in health care: the contribution of the Australian Patient Safety Foundation to incident monitoring and analysis.

Because of growing concern about violence in health care in Australia, we reviewed the relevant data on incidents involving violence collected using the Australian Incident Monitoring System (AIMS). Among 42 338 incidents reported from 1 July 2000 to 30 June 2002, 3621 (9% of all incidents) involved patients and physical violence or violent verbal exchange; staff injury was reported in 5% of cases. The proportion was higher in emergency departments (16%, with frequent involvement of mental health problems or alcohol or drug intoxication) and mental health units (28%). Contributing factors incl

Safety and toxicity of catheter gene delivery to the pulmonary vasculature in a patient with metastatic melanoma.

One approach to gene therapy for human cancer is transcatheter injection of DNA liposomes into tumor masses. To determine the feasibility of selective delivery of recombinant genes by a catheter to the pulmonary vasculature in humans, a patient with melanoma received two treatments of HLA-B7 plasmid DNA complexed to cationic liposomes into a right posterior basal pulmonary artery associated with a mass lesion. The treatments were well tolerated. No adverse respiratory, cardiac, immunologic, or other organ toxicities were detected. The delivery of recombinant genes by catheter may be a useful m

Pleural procedural safety in the UK: is everyone's house in order? Reflections from the BTS National Pleural Service Organisational Audit and a national review of patient safety incidents.

INTRODUCTION: The 2022 British Thoracic Society Pleural Services Organisational Audit highlighted evidence of ongoing risk of harm from pleural procedures. To better understand the underlying causes of these safety concerns we undertook a review of patient safety incidents from the National Reporting and Learning System (NRLS). METHODS: Incident-level patient safety data from NRLS were requested from any level 3, 4 and 5 incidents describing harm resulting from pleural intervention, specifically chest drain insertion or pleural aspiration for pleural effusions (fluid), submitted between 1 Apr…

Safety and Blinding of Transcranial Direct Current Stimulation in Schizophrenia Patients.

INTRODUCTION: Transcranial direct current stimulation (tDCS) is a method of noninvasive brain stimulation. tDCS clinical efficacy is debatable, but it was proved to be effective at the neurophysiological level. tDCS has a very good safety profile in human subjects. Because the method has been more frequently studied in patients with schizophrenia, we wanted to analyze the safety and tolerability of tDCS treatment in this population. We also evaluated blinding quality of tDCS stimulations. MATERIAL AND METHODS: All stimulations were performed using the DC-Stimulator PLUS (neuroCare, Germany) w

Safety and Efficacy of Therapeutic Agents in Viral Pneumonia in Immunosuppressed Patients.

INTRODUCTION/OBJECTIVE: Immunosuppressed Patients (ISP) - defined by congenital or acquired deficiencies in systemic defense mechanisms - are disproportionately susceptible to viral respiratory infections. Agents such as SARS-CoV-2, Respiratory Syncytial Virus (RSV), influenza A and B, and Cytomegalovirus (CMV) cause significant morbidity in both immunocompetent and immunosuppressed individuals. Clinical manifestations range from mild influenza-like illness to severe pneumonia, respiratory failure, multiorgan dysfunction, and concurrent bacterial infections. METHODS: A narrative synthesis of

Safety of Subcutaneous Heparin in Patients Requiring External Ventricular Drains.

BACKGROUND AND OBJECTIVES: External ventricular drain (EVD) placement and removal to manage intracranial pressure carry an inherent risk of hemorrhage, which complicates chemoprophylactic management of venous thromboembolism (VTE). VTE chemoprophylaxis with subcutaneous (SQ) heparin is effective in preventing deep venous thrombosis; however, administration to patients with EVDs may exacerbate hemorrhage risk. We assessed the risk of hemorrhage with SQ heparin administration in patients who received EVDs. METHODS: One hundred twenty patients who underwent EVD placement received chemoprophylact

Drug safety in patients with mitochondrial disease: an observational cohort study.

BACKGROUND: Mitochondrial diseases are common inherited neurometabolic disorders and frequently involve the nervous system, yet their multisystem nature often necessitates complex pharmacological management. Many commonly prescribed medications have off-target effects on mitochondrial function, and patients with mitochondrial disease may be particularly vulnerable to such effects due to impaired energy metabolism. However, systematic data on medication safety in this patient group remain scarce. METHODS: In this retrospective, single-centre, cohort-based study at Turku University Hospital (Tu
