Rhabdomyolysis and anesthesia: a report of two cases and review of the literature.

Rhabdomyolysis occurred in two apparently healthy boys aged 9.5 and 5.5 years after general anesthesia with suxamethonium. Mild hyperkalemia and renal failure were observed in the first patient, who was subsequently diagnosed with Becker dystrophy. In the second patient, the clinical presentation was not classic for malignant hyperthermia and a muscle biopsy failed to disclose any pathological finding. A review of the literature revealed 66 pediatric cases (56 boys and 10 girls) of anesthesia-associated rhabdomyolysis. Forty-nine (74%) cases were caused by an underlying, mostly unrecognized co

Comparison of three quadratus lumborum block approaches for pediatric lower abdominal surgeries: a randomized controlled trial.

BACKGROUND: Lower abdominal surgeries in the pediatric population are associated with significant post-operative pain. Regional anesthesia techniques including ilioinguinal nerve block, Transversus Abdominis Plane (TAP) block, and Quadratus Lumborum (QL) block have been explored for lower abdominal surgeries. This study compares the analgesic effect of three different approaches to quadratus lumborum block in pediatric patients undergoing lower abdominal surgeries. METHODS: This randomized controlled trial included 120 pediatric patients aged between 1 and 7 years, scheduled for lower abdomin

Comparative evaluation of 3% plain mepivacaine and 2% lignocaine with 1:100,000 adrenaline in the pediatric population: A double-blind, randomized controlled trial.

CONTEXT: Effective local anesthesia is essential in pediatric dentistry, especially during procedures like pulpectomy. While lignocaine with adrenaline is widely used, mepivacaine without a vasoconstrictor offers potential benefits in pediatric patients. AIMS: This study aimed to compare the hemodynamic responses and pain perception associated with 3% mepivacaine (plain) versus 2% lignocaine with 1:100,000 adrenaline in children undergoing pulpectomy. SETTINGS AND DESIGN: A randomized, double-blind controlled trial was conducted at the Department of Pediatric and Preventive Dentistry of Dr.

Multi-institution assessment of the use and risk of cardiovascular computed tomography in pediatric patients with congenital heart disease.

BACKGROUND: Cardiac computed tomography (CT) is increasingly used in pediatric patients with congenital heart disease (CHD). Variability of practice and of comprehensive diagnostic risk across institutions is not known. METHODS: Four centers prospectively enrolled consecutive pediatric CHD patients <18 years of age undergoing cardiac CT from January 6, 2017 to 1/30/2020. Patient characteristics, cardiac CT data and comprehensive diagnostic risk were compared by age and institutions. Risk categories included sedation and anesthesia use, vascular access, contrast exposure, cardiovascular medica

Comparison of a combination of midazolam and diazepam and midazolam alone as oral premedication on preanesthetic and emergence condition in children.

BACKGROUND: Preanesthetic anxiety and emergence agitation are major challenges for anesthesiologists in pediatric anesthesia. Thus, midazolam has been used as premedication for children. However, midazolam alone is not effective for emergence agitation. The present study tested the effect of a combination of midazolam and diazepam on the preanesthetic condition and emergence behavior in children. METHODS: Forty-two children were allocated to one of three groups: the NoPre group received no premedication; the Mi group received midazolam 0.5 mg kg(-1) orally; and the Mi + Di group received mida…

Use of Serratus Plane Block for Repair of Coarctation of Aorta: A Report of 3 Cases.

OBJECTIVES: The practice of regional anesthesia techniques (thoracic, epidural, paravertebral) in pediatric cardiac surgery enhances perioperative outcomes such as improved perioperative analgesia, decreased stress response, early extubation, and shortened hospital stay. However, these blocks can be technically challenging and can be associated with unacceptable failure rate and complications in infants. For these reasons, regional anesthesia is sometimes avoided in pediatric cardiac surgery. We describe the simple and effective serratus plane block for thoracotomy analgesia in 2 neonates and

Pediatric Procedural Sedation.

Pediatric procedural sedation (PPS), formerly known as conscious sedation, is often used outside the operating room for various procedures. Twenty years ago, nearly all cases of PPS were performed by pediatric intensivists, dentists, emergency medicine physicians, and anesthesiologists, due to the urgent nature of procedures in their settings. However, with the emergence of pediatric hospital medicine as a board-certified subspecialty, many children's hospitals have created dedicated PPS teams. These teams, composed of highly trained physicians and ancillary staff, are well-suited for procedur

Association Between Extreme Weight-for-Age and Airway-Related Adverse Events During Anesthesia Induction: An Analysis Using the J-PEDIA Registry Data.

BACKGROUND: Children are particularly vulnerable to hypoxia during airway management, which can lead to life-threatening complications. However, pediatric evidence quantifying the association between body weight range and airway management-related adverse events (ARAEs) during anesthesia induction remains limited. This study aimed to evaluate the association between body weight and incidence of adverse events during general anesthesia induction. METHODS: This multicenter retrospective cohort study used data from the Japanese Pediatric Difficult Airway Registry from June 2022 to February 2025

Experience of General Anesthesia for Glossopexy in Infants With Robin Sequence.

We present a case of an infant patient with Robin sequence (Pierre Robin sequence; PRS) who underwent general anesthesia for a glossopexy procedure. Pediatric patients with PRS are prone to upper airway obstruction during general anesthesia induction and intubation difficulties due to micrognathia and glossoptosis. In this case, we facilitated mask ventilation by inserting a nasopharyngeal airway before induction and successfully intubated the patient using a 2-person technique that combined the use of a video laryngoscope and a flexible fiber-optic scope. This experience suggests that the use

Pulseless electrical activity in a pediatric patient: a case report and review of causative factors and treatment.

Pulseless electrical activity, an arrhythmia that leads to cardiac arrest, is defined as the presence of organized electrical activity without a palpable pulse or arterial blood pressure. When this arrhythmia presents during anesthesia, it has become routine practice to initiate advanced cardiac life support according to the American Heart Association guidelines. This arrhythmia is usually associated with a poor prognosis unless a reversible cause is investigated and treated immediately. The purpose of this article is to summarize the causative factors of pulseless electrical activity and its

Acute unilateral enlargement of the parotid gland immediately post craniotomy in a pediatric patient: a case report.

A temporary acute unilateral enlargement of the parotid gland or "anesthesia mumps" has been described in both surgical and anesthesia literature. It has been described in elderly, dehydrated, poorly nourished, and post-operative patients. We present a 5-year-old patient who underwent a left temporal craniotomy for seizure focus resection and quadriceps muscle biopsy. Immediately post procedure, he was noted to have an acute unilateral enlargement of the right parotid gland. We report acute unilateral parotitis as a possible, but uncommon, complication of positioning in the pediatric populatio

[Isolated postoperative myoglobinuria as the only sign of malignant hyperthermia. Value of spinal anesthesia with bupivacaine in a posterior surgical intervention].

The Authors report a case of biopsy-proven malignant hyperthermia in a pediatric patient who underwent general anesthesia with halothane and succinylcholine for foot surgery, in whom the presenting symptom was isolated postoperative myoglobinuria. The above syndrome, in the absence of a positive family history, may present itself with a set of minor and atypical symptoms, thus being often underestimate. The need for further investigating all the cases of postoperative myoglobinuria is stressed by the Authors, which also consider muscular biopsy for inclusion among routine investigations. In th

Pediatric flexible fiberoptic bronchoscopy--a preliminary report.

The introduction of flexible fiberoptic bronchoscopy into clinical use in 1968 had led a veritable explosion in the application of pulmonary endoscopy. In 1978, Wood and his colleagues, using a 3.5 mm Olympus 3C4 pediatric bronchoscope, expanded this technique to pediatric field. Up-to-now, they have more than 2000 bronchoscopies in pediatric patients, however, there has been no report in this field in Taiwan. I, using a new Olympus pediatric flexible fiberoptic bronchoscope 3C20, sedation and topic anesthesia, performed 24 airway examinations in 22 patients in the past 7-month period. The you

Meta-analysis study on anesthetic sedation recovery and onset times in pediatric and elderly patients undergoing CT and MRI.

Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) are crucial diagnostic modalities that require patients to remain immobile for extended periods, with anesthesia sometimes used for comfort and image quality enhancement. The study compares dexmedetomidine and propofol in reducing recovery time and sedation onset in pediatric and elderly patients undergoing CT and MRI procedures. A meta-analysis of fifteen studies assessing recovery time, sedation onset, and failed sedation between dexmedetomidine and propofol in pediatric and elderly patients during CT and MRI was conducted. Intraoperative management of pulmonary arterial hypertension in infants and children--corrected and republished article.

PURPOSE OF REVIEW: Pediatric pulmonary arterial hypertension (PAH) continues to be a considerable problem to the pediatric anesthesiologist even if its management has seen remarkable advances in recent years. It is important that anesthesiologists caring for children with PAH be aware of the increased risk, understand the pathophysiology of PAH, form an appropriate anesthetic management plan. A review of some of the latest medical advances will provide the reader with a better understanding of the most current anesthetic management options. RECENT FINDINGS: The literature reviewed demonstrate

Continuous central and perineural infusions for postoperative pain control in children.

PURPOSE OF REVIEW: Continuous infusion remains the technique of choice when there is a prolonged operation or intense postoperative pain is anticipated. Supplementing a general anesthesia with a nerve block can result in a pain-free awakening and postoperative analgesia without the potentially deleterious effects associated with parenteral opioids. The literature confirms the very low rate of complications and adverse effects of regional anesthesia in children. RECENT FINDINGS: Clinicians need to be aware of the key points for performing a block and placing a catheter in children: good knowle

[Intraoperative infiltration of 0.5% bupivacaine in pediatric tonsillectomy].

Twenty-eight healthy pediatric patients scheduled for bilateral tonsillectomy under general anesthesia were allocated randomly into two groups. The same anesthetic technique was used in both groups. After oral intubation, tonsils were infiltrated with either 0.5% bupivacaine (group B) or isotonic saline solution (group F). During surgery, hemodynamic stability, blood loss, and need for additional anesthesia were evaluated. After surgery, hemodynamic stability, pain, general behavior, need for analgesics, oral intake, and complications were evaluated. The bupivacaine group had significantly les
