In vitro evaluation of the antimicrobial properties of terpinen-4-ol on apical periodontitis-associated bacteria.

Manuka oil and tea tree oil are essential oils with known antibacterial properties that are believed to be caused by one main component: terpinen-4-ol. Terpinen-4-ol has potent antibacterial activity against caries-related microorganisms. However, few studies have investigated the antimicrobial effects of terpinen-4-ol on bacteria in apical periodontitis. Thus, the objective of the present study was to evaluate the antibacterial and antibiofilm potential of terpinen-4-ol against Enterococcus faecalis, Porphyromonas gingivalis, Prevotella intermedia, and Fusobacterium nucleatum, which have all

Endodontic Microsurgery of Mandibular Second Molars Using the Bony Lid Approach: A Case Series.

Endodontic microsurgery is important for preservation of natural teeth with symptomatic apical periodontitis when conventional endodontic treatment is neither feasible nor effective. The thick buccal cortex, poor visibility, and potential neurovascular complications can make microsurgical procedures of mandibular second molars infeasible. With the aid of cone-beam computed tomography, the bony lid approach was applied in 10 cases of endodontic microsurgery of mandibular second molars at the VIP Center, Hangzhou Stomatology Hospital between 2019 and 2020. In all cases, the clinical and radiogra

Exploiting Dentine Matrix Proteins in Cell-Free Approaches for Periradicular Tissue Engineering.

The recent discovery of mesenchymal stem cells within periapical lesions (PL-MSC) has presented novel opportunities for managing periradicular diseases in adult teeth by way of enhancing tissue regeneration. This discovery coincides with the current paradigm shift toward biologically driven treatment strategies in endodontics, which have typically been reserved for non-vital immature permanent teeth. One such approach that shows promise is utilizing local endogenous non-collagenous dentine extracellular matrix components (dECM) to recruit and upregulate the intrinsic regenerative capacity of P

Conservative Approach in the Management of Large Periapical Cyst-Like Lesions. A Report of Two Cases.

Background and Objectives: Periapical cystic lesions are a pathology frequently addressed to endodontic specialists. Although their therapy is still not standardized, the treatment should be as conservative as possible and by endodontic means, as they are lesions of endodontic origin. The present case report describes two cases of upper central incisors with large cyst-like periapical lesions, and their one-year follow up. Materials and Methods: Endodontic orthograde treatment was performed under copious irrigation with sodium hypochlorite, in association with calcium hydroxide as an intra-can

Influence of a High-fat Diet in the Progression of Apical Periodontitis.

INTRODUCTION: This study evaluated the effects of a high-fat diet (HFD) on the progression of apical periodontitis (AP) in rats. METHODS: Eight-week old Wistar rats (N = 24) were included in this study. Half of the animals received an HFD and the other half a control diet (CD). AP lesions were induced 8 weeks later by creating pulp exposure of the mandibular left first molars. The animal's body mass was verified on a weekly basis. After 21 and 40 days, the animals were sacrificed. Blood was collected for biochemical tests, and the liver was removed for hepatic triglycerides analysis. The left

Apical Periodontitis Is Associated with Elevated Concentrations of Inflammatory Mediators in Peripheral Blood: A Systematic Review and Meta-analysis.

INTRODUCTION: Apical periodontitis (AP), except for the local known consequences, may also be a systemic burden. Circulating inflammatory mediators that are released to sustain the AP lesion can in theory harm other bodily tissues. The aim of this systematic review was to summarize the existing evidence on the influence of AP on the peripheral blood levels of inflammatory mediators and markers of systemic stress. METHODS: A search of MEDLINE-PubMed, Embase, and Cochrane was conducted up to and including February 2019 to identify studies in 5 different languages. The Newcastle-Ottawa Scale was

Mesenchymal chondrosarcoma mimicking apical periodontitis.

This report describes the case of a patient with mesenchymal chondrosarcoma in the region of tooth #18 that mimicked apical periodontitis. Forty-five days after endodontic treatment, gingival swelling and intense pain were observed. The endodontist suspected a furcation lesion, and the treatment option that the patient chose was extraction of the tooth. Seven days after suture removal, a rapidly growing swelling was seen in the region. The patient was referred to an oral and maxillofacial surgeon, who recommended an incisional biopsy after evaluating imaging and clinical findings. Microscopic

Subattachment inflammatory root resorption: treatment strategies.

Attempts have been made to classify root resorption according to its etiology in order to develop more efficient strategies for its treatment. This condition has been identified as resulting from damage to the protective attachment layer (predentin or precementum) of the root combined with inflammation adjacent to the root surface. This article demonstrates a series of treatment strategies focused on the reversal of each manifestation of subattachment root resorption, specifically emphasizing defects that extend apically and localized defects.

[One-year control examination of therapy of chronic apical periodontitis treated by single visit method].

The results of one year control-examination of the single visit endodontic therapy of chronic apical lesions are presented. Clinical and radiographic control examinations were performed on 40 of 58 treated single and double root canal teeth from both jaws. Eighty percent of treated teeth was found to be successful. Endodontic failure was found in 15%, while 5% of treated teeth were extracted due to prosthodontic or some other reason. No relation between the incidence of early postoperative discomforts and endodontic failure was found. The incidence of therapy was higher in teeth with primarily

The Influence of Apical Radiolucency Size on Dentists' Decision-making Regarding Adequately Treated Teeth with Asymptomatic Apical Periodontitis using Digitally Simulated Images.

INTRODUCTION: Epidemiologic studies have reported a relatively high prevalence of asymptomatic apical periodontitis in endodontically treated teeth. Such teeth, when adequately restored and endodontically treated, present a clinical dilemma regarding whether to intervene or follow-up. The aim of this study was to examine dentists' clinical decision-making for well-treated endodontically and adequately restored teeth with asymptomatic apical periodontitis, presenting various sizes of simulated apical radiolucencies. MATERIALS AND METHODS: Twenty dentists (10 endodontic specialists and 10 prost

Is odontogenic keratocyst an endodontic enigma? A rare case report of management of odontogenic keratocyst in anterior mandible.

Odontogenic keratocyst (OKC), a rare, locally aggressive developmental cyst, is found incidentally on dental radiographs, most commonly in the posterior mandible. When it occurs in the periapical region in association with nonresponding teeth to pulp sensibility tests, it is often misdiagnosed as other endodontic lesions such as radicular cyst, lateral periodontal cyst and dentigerous cyst. This case report describes the rare occurrence of OKC in the anterior mandibular region managed successfully with endodontic treatment and re-interventional surgery in a conservative approach. This case rep
