Unusual recurrence of hydatid cysts of the heart: report of two cases and review of the clinical and surgical aspects of the disease.

In cardiac echinococcosis, a hydatid cyst most frequently forms either solely in the heart or in the pericardium, but there are several reports of cysts forming in the liver or lung or in both. In two cases reported here, both patients developed cysts in new sites after one or more previous surgeries for hydatid cyst removal. In Case 1, the patient first underwent spleenectomy and resection of multiple cysts with no evidence of a cyst in the heart; 3 years later, there was no sign of Echinococcus in the liver, but a large inframyocardial cyst had damaged the left ventricle. In Case 2, the pati…

[A case of thymic epidermoid carcinoma recurred in the pericardial cavity].

A 61-year-old woman with cough and sputum and right scapular pain was referred to our hospital with a suspected anterior mediastinal tumor by Chest X-rays and CT-scan. The tumor was diagnosed as thymic cancer cytologically by fine needle biopsy. At the the thoracotomy via median sternotomy, the tumor adhered to left brachial vein, superior vena cava and pericardium and it was dissected by maneuver. It was diagnosed as moderately differentiated epidermoid carcinoma by histology of the surgical specimen and involvement of the pericardium was histologically presumed. The patient received postoper…

[Emergency in neo-natal surgery. High risks in forty-one cases (author's transl)].

41 babies were operated upon in emergency in the cardiac surgery department. 11 of them were submitted to open heart surgery and 30 were ope … bed. The authors insist on the frequency of lethal cardiac fibrillation occurring at the opening of pericardium in case of coarctations of aorta complicated by pericardial effusion. Prevention of this accident is possible if at the beginning of anaesthesia the baby lies in a proclive position. On the opposite, bradycardia occurring at the opening of the peric …

Which type of duraplasty is best for Chiari type I malformation surgery?

OBJECTIVE: This study investigated whether the selection of different dural substitutes and distinct dural repair techniques correlates with … autologous duraplasty. The remaining 338 cases with heterologous duraplasty from equine and bovine pericardium were considered for the comparative statistical analysis. The mean follow-up duration ranged from 39 months in adults to 45 months in children. The CSF complication rate requiring revision was 6.5% in the total cohort, with a higher incidence in chi …

Transseptal access through synthetic materials: An in vitro comparative study of radiofrequency and needle-based techniques.

BACKGROUND: The transseptal approach for accessing left heart structures is commonly performed with metal needles and radiofrequency (RF) ne … n RF wire. Study materials included expanded polytetrafluoroethylene (ePTFE), polyester, and bovine pericardium; porcine atrial septum was the control material. The primary outcomes were (1) the number of 1-second RF pulses required to puncture for RF devices and (2) the peak force required to puncture for the metal needle. RESULTS: Excluding ePTFE, RF-based …

Prevention of glutaraldehyde-fixed bioprosthetic heart valve calcification by alcohol pretreatment: further mechanistic studies.

BACKGROUND AND AIM OF THE STUDY: Calcification is a major cause of failure of bioprosthetic heart valves derived from glutaraldehyde-crosslinked bovine pericardium or porcine aortic valve (PAV) cusps. Recently, we have shown that ethanol pretreatment of PAV cusps prevents calcification in animal models. METHODS AND RESULTS: In this study we showed that ethanol pretreatment was equally effective in preventing calcification of glutaraldehyde-crosslinked bovine pericardium (control Ca2+ = 121.16+/-7.49 microg/mg tissue; ethanol-pretreated Ca2+ = 2.95+/-0.78 microg/mg). Furthermore, other low-mol…

A novel technique for accessing the pericardial space.

BACKGROUND: Pericardial procedures are potentially dangerous in patients with minimal anterior pericardial effusion and cases with pericardium adhesion. OBJECTIVE: To enhance safety, we used an innovative wire puncture technique for pericardiocentesis. METHODS: The hard end of the percutaneous transluminal coronary angioplasty guidewire was used to puncture the parietal pericardium. To improve the puncture ability, a flat-tipped metal tube was used to support the guidewire; furthermore, a microcatheter was used to exchange the soft end of the guidewire. Before the clinical trial, the novel t…

Successful treatment with positive airway pressure ventilation for tension pneumopericardium after pericardiocentesis in a neonate: a case report.

BACKGROUND: Pneumopericardium in neonates is often associated with respiratory diseases, of which positive pressure ventilation (PPV) is an exacerbating factor. Here, we present a neonate case of pneumopericardium after cardiac surgery which was resolved after applying PPV. CASE PRESENTATION: A 28-day-old neonate with left recurrent nerve palsy after aortic reconstruction for interrupted aortic arch developed pericardial effusion. Pericardiocentesis was performed under general anesthesia, and a drainage tube was left in the pericardium. After extubation, stridor gradually exacerbated, followi…

Post-transplant Lymphoproliferative Disorders: A Rare Case of Primary Pericardial Involvement Epstein-Barr Virus-Correlated in Heart Transplantation.

Post-transplant lymphoproliferative disorders (PTLDs) are considered a fatal consequence of immunosuppression. We report a case of a 52-year-old patient, who underwent a cardiac transplantation and presented undefined recurrent episodes of pleuropericardial effusions without lymphoadenomegaly at chest radiographs and computed tomography. Histopathological analysis of the bioptic pericardium showed a specific chronic inflammation. Monitoring endomyocardial biopsies (EMBs) showed only 1 episodes of greater than grade 2R acute cellular rejection requiring immunosuppressive treatment, mild vasculi…

[Coexistence of sarcoidosis and thymic carcinoid].

We report a case of coexistence of sarcoidosis and thymic carcinoid. A 57-year-old man was pointed out the anterior mediastinal tumor when his generator of pacemaker was exchanged. The tumor was diagnosed as atypical carcinoid by percutaneous needle biopsy. Chest computed tomography (CT) revealed the mediastinal and right hilar lymphadenopathy. Preoperative transbronchial aspiration cytology revealed no malignancy and extirpation of the anterior mediastinal tumor was carried out together with left diagraphmatic nerve and pericardium. The histopathological examination of the pretracheal lymph n…

Echocardiographic detection of pneumomediastinum and pneumopericardium: the air gap sign.

Six patients referred for echocardiographic evaluation in whom an unusual echocardiographic sign resulted from air within the mediastinum or pericardium are described. Three patients had a pneumomediastinum that occurred after chest trauma and three patients had a pneumopericardium induced during a therapeutic pericardiocentesis. Important features included a broad band of echoes (air) recorded during held respiration which obscured the normal cardiac structures and dropout (gap) of echoes posteriorly. Between the cyclic appearance of the "air gap" sign, intracardiac structures were normally v…

Pericardial ectopic thymoma presenting with cardiac tamponade: report of a case.

Ectopic thymoma arising from organs other than the thymus, such as the neck, trachea, thyroid, lung and pericardium, is rare. To date, there have been only seven other cases of pericardial thymoma reported in the English literature. We herein report a case of pericardial ectopic thymoma that presented with cardiac tamponade. A 72-year-old Japanese male noticed body weight gain and leg edema. Chest computed tomography (CT) revealed pericardial effusion and an irregularly shaped mass in the pericardial space compressing the right atrium. He was considered to have cardiac tamponade due to a parac…

[Sudden/unexpected death due to spontaneous aortic rupture: two unusual manifestation forms].

Sudden unexpected death from a natural (pathological) cause often occurs under suspicious circumstances suggesting foreign intervention, so that a forensic autopsy is deemed necessary. In adults, a significant share of these deaths is due to spontaneous ruptures of the aorta. In figures, dissections of the ascending aorta and the aortic arch (mostly associated with secondary perforation into the pericardium) account for a large percentage followed by arteriosclerotic aneurysms of the abdominal aorta (with retroperitoneal and sometimes intraperitoneal hemorrhage). The authors report on two fata…
