A patient with an autosomal dominant nail dystrophy characterized by periodic, asymptomatic shedding of the nails followed by regrowth is described herein. This highly penetrant disorder is similar to two earlier cases found in the dermatologic literature.

A case of thyrotoxic periodic paralysis based on clinical grounds, laboratory data and therapeutic response as well is reported. The authors comment on the differential diagnosis with the most frequent types of periodic paralysis. The importance of a correct diagnosis and treatment as early as possible is stressed as to prevent further development of permanent paralysis due to irreversible degenerative myofibril changes as stated in literature.

The electrocardiograms (ECGs) of 30 patients with hypokalaemic thyrotoxic periodic paralysis during and after paralysis were studied. During paralysis, typical features of hypokalaemia were seen in all patients with serum potassium levels of 2.8 mmol/l or less; above this level, the ECGs varied from non-diagnostic to those showing typical features of hypokalaemia. It was not possible to accurately predict the serum potassium level from the ECG except when either sinus arrest or heart block was present. Although extrasystoles have been reported to be common in hypokalaemia, none of the patients

An electrophysiologic study of peripheral nerve excitability was performed in patients with hyperkalemic periodic paralysis (HPP) and in normal controls. It was found that a marked degree of neural hyperexcitability existed in all patients with HPP who showed cold sensitivity. The results suggest involvement of the nerve cell membrane in the proximal segments of peripheral nerve. This testing method may be of value in differentiating the cold-sensitive variant of HPP.

Although sickle cell anemia rarely presents with clinical manifestations in the newborn period, it should be considered a possible cause of unexplained early jaundice in black newborn infants. If sickle cell disease is suspected, sickle cell preparation and hemoglobin electrophoresis of the infant and of both parents should be carried out.

Patterns of breathing at night were recorded in 4 patients with heart-failure. 2 had periodic breathing while awake and in 2 it developed after they fell asleep. In all 4 the phase of hyperventilation disturbed sleep. These cases also illustrate other problems caused by periodic respiration in heart-failure, which range from tiredness during the day to an inability to sleep for more than a few minutes. Nocturnal waking in the hyperventilation phase of Cheyne-Stokes breathing should be differentiated from paroxysmal nocturnal dyspnoea caused by episodes of pulmonary oedema at night.

The critical period in which human visual acuity may be lost after visual deprivation can be deduced by studying children with amblyopia which follows uncomplicated unilateral cataract and the subsequent aphakic blur. The visual acuity at the first contact lens fitting increases linearly on a log scale as a function of the age at onset between 3 and 10 years. Correction before 4 months of age produces less visual loss but we cannot specify the function within this period. If the eye is deprived between 6 and 30 months of age finger counting is the best visual acuity achieved. Deprivation comme

Removal of N-acetylneuraminic acid from the platelet surface causes rapid removal of platelets from the circulation but causes little change in other platelet functions. We have now investigated the effects of sodium periodate which is thought to oxidize the sialic acid of glycoproteins on cell surfaces and has been shown to affect the functions of other cells. NaIO4 (1 to 10 mm) caused aggregation of stirred suspensions of washed platelets from rabbits. Calcium was required in the suspending medium for NaIO4-induced aggregation. Aggregation was not accompanied by the release of amine storage

1) Etiology of convulsions starting prior to two years of age was discussed in 418 cases. Neonatal seizures before 30 days old appeared in 86 cases (53 boys and 33 girls). Three hundred and thirty-two patients (172 boys and 160 girls) had convulsions in infancy. Twelve patients (9 boys and 3 girls) suffered from convulsions both in neonatal and infantile period. 2)Etiology of convulsions was prenatal in 67 cases (16%), natal in 49 cases (12%), postnatal in 158 cases (38%) and unknown in 144 cases (34%). Prenatal factors consisted of cerebral malformation (23 cases, 6%), associated physical min

Polygraphic recordings of 20 neonates with brain malformations and/or chromosomal anomalies were compared with those obtained in 29 normal fullterm neonates. Seep assessment was made with different methods described in the literature. These methods of sleep scoring gave similar results in the normal newborn babies but discrepant results in many pathological neonates. Thus sleep was also studied by other approaches; assessment of cycling of the 5 parameters (EEG, REM, EMG, Motility, Respiration) and study of the discrepant parameters during a sustained period of Active sleep (AS) or Quiet Sleep

A classical definition of Pickwickian syndrome associates alveolar hypoventilation, obesity and hypersomnia with periodic breathing. Obesity in itself is enough to explain the alveolar hypoventilation and some of the sleep disorders, but in fact all three elements of this syndrome are intricated. Obesity, whether associated with a Pickwickian syndrome or not, affects ventilatory mechanics similarly. With the cause of sleep disorders are associated central ventilatory pauses, appearing periodically followed by phases of apnoea secondary to buccopharyngial hypotony. The alveolar hypoventilation
