Neuropathic complications after 157 procedures of continuous popliteal nerve block for hallux valgus surgery. A retrospective study.

BACKGROUND: Continuous peripheral nerve block (CPNB), in particular at the popliteal fossa, is widely used in orthopedic surgery, allowing good postoperative analgesia. Possible neuropathic complications, however, remain poorly known. OBJECTIVE: To review the characteristics of peripheral neuropathy (PN) after sciatic CPNB at the popliteal fossa, estimating prevalence, severity, evolution and possible risk factors, especially those relating to the procedure. METHODS: Retrospective study of PN associated with popliteal fossa CPNB for hallux valgus surgery, between November 1st, 2005 and Novem

Low-level laser therapy alleviates neuropathic pain and promotes function recovery in rats with chronic constriction injury: possible involvements in hypoxia-inducible factor 1α (HIF-1α).

Nerve inflammation plays an important role in the development and progression of neuropathic pain after chronic constrictive injury (CCI). Recent studies have indicated that hypoxia-inducible factor 1α (HIF-1α) is crucial in inflammation. Low-level laser therapy has been used in treating musculoskeletal pain, but rare data directly support its use for neuropathic pain. We investigated the effects of low-level laser on the accumulation of HIF-1α, tumor necrosis factor-α (TNF-α), and interleukin-1β (IL-1β) in controlling neuropathic pain, as well as on the activation of vascular endothelial grow

Pulse pressure is independently associated with sensorimotor peripheral neuropathy in patients with type 2 diabetes.

OBJECTIVES: The mechanisms responsible for the onset and progression of sensorimotor peripheral neuropathy (SMPN) in type 2 diabetes remain largely unknown. Although a link between hypertension and SMPN has been observed, it is not clear which blood pressure (BP) component (i.e. systolic, SBP; diastolic, DBP; or pulse pressure, PP) is primarily involved. We sought to determine the relationship between BP components and parameters of nerve function in type 2 diabetes. DESIGN: Cross-sectional study. SETTING: Academic medical centre. SUBJECTS: A total of 55 consecutive ambulatory patients with

(Cost-)effectiveness of lower extremity nerve decompression surgery in subjects with diabetes: the DeCompression (DECO) trial-study protocol for a randomised controlled trial.

INTRODUCTION: The peripheral nerves of patients with diabetes are often pathologically swollen, which results in entrapment at places of anatomical narrowing. This results in nerve dysfunction. Surgical treatment of compression neuropathies in the lower extremities (lower extremity nerve decompression (LEND)) results in relief of symptoms and gain in peripheral nerve function, which may lead to less sensory loss (short term) and less associated detrimental effects including foot ulceration and amputations, and lower costs (long term). The aim of the DeCompression trial is to evaluate the effec

TDRKH is a candidate gene for an autosomal dominant distal hereditary motor neuropathy.

Distal hereditary motor neuropathies (dHMNs) comprise a group of clinically and genetically heterogeneous inherited lower motor neuron syndromes mainly characterized by a distal-predominant pattern of progressive muscle atrophy, weakness and hyporeflexia, without sensory dysfunction. Although at least 21 causative genes for dHMN have been reported, mutational scanning of these genes often fails to identify the causative variants in dHMN cohorts, suggesting that additional causative genes remain to be identified. We studied a four-generation pedigree of a Japanese family with autosomal dominant

Nerve function in workers with long term exposure to trichloroethene.

Certain functions of the nervous system were examined in 31 printing workers (mean age 44) exposed to trichloroethene (mean duration 16 years) and 28 controls (mean age 45). In the sural nerve the conduction velocity (SNCV), response amplitude, and refractory period (SRP) were measured. The latencies of the masseter and the blink reflex were determined to test the trigeminal nerve. In the peroneal nerve the conduction velocity of fast and slow nerve fibres, the response amplitude, and the refractory period were determined. As a measure of autonomic nerve function the response of the heart rate

A TRPV4 mutation caused Charcot-Marie-Tooth disease type 2C with scapuloperoneal muscular atrophy overlap syndrome and scapuloperoneal spinal muscular atrophy in one family: a case report and literature review.

BACKGROUND: Charcot-Marie-Tooth disease 2C (CMT2C) and scapuloperoneal spinal muscular atrophy (SPSMA) are different clinical phenotypes of TRPV4 mutation. The mutation of p.R316C has been reported to cause CMT2C and SPSMA separately. CASE PRESENTATION: Here, we reported a Chinese family harboring the same p.R316C variant, but with an overlap syndrome and different clinical manifestations. A 58-year-old man presented with severe scapula muscle atrophy, resulting in sloping shoulders. He also exhibited distinct muscle atrophy in his four limbs, particularly in the lower limbs. The sural nerve

Proximal fibulectomy in the management of proximal fibular aneurysmal bone cyst: technical considerations and outcome; a retrospective case series.

BACKGROUND: Aneurysmal bone cyst (ABC) is a benign but locally aggressive bone tumor. Involvement of the proximal fibula is uncommon and represents a unique surgical challenge due to the proximity of the common peroneal nerve (CPN) and the lateral stabilizing structures of the knee. The optimal surgical management of ABC in this location remains controversial. METHODS: A retrospective case series was conducted including patients with histologically confirmed ABC of the proximal fibula treated with proximal fibulectomy between October 2018 and November 2023. All patients had a minimum follow-up

[Surgical Results of Lumbar Degenerative Disease with Foot Drop].

Drop foot is associated with various diseases, such as peripheral nerve disease, lumbar degenerative disease, metabolic disease, and endocrine disease. Among lumbar degenerative diseases, lumbar disc herniation and lumbar canal stenosis are most commonly reported. We report the surgical outcomes of patients with drop foot caused by lumbar degenerative diseases. Between 2008 and 2018, we encountered 24 cases of drop foot in patients who underwent surgery of the lumbar lesion. The mean age was 69.5 years(49-88 years). Fourteen patients were male and ten were female. The mean time interval betwee

Multiple locations of nerve compression: an unusual cause of persistent lower limb paresthesia.

A paucity of appreciation exists that the "double crush" phenomenon can account for persistent leg symptoms even after spinal neural decompression surgery. We present an unusual case of multiple locations of nerve compression causing persistent lower limb paresthesia in a 40-year old male patient. The patient's lower limb paresthesia was persistent after an initial spinal surgery to treat spinal lateral recess stenosis thought to be responsible for the symptoms. It was later discovered that he had peroneal muscle herniations that had caused superficial peroneal nerve entrapments at 2 separate

Common peroneal nerve palsy from an untreated popliteal pseudoaneurysm after penetrating injury.

A 17-year-old boy presented with drop foot and loss of extension in his left knee after penetrating injury to the popliteal region. Color duplex ultrasonography and angiography revealed a popliteal artery pseudoaneurysm in the popliteal fossa. Electrodiagnostic tests demonstrated nearly total common peroneal nerve injury. The delay in diagnosis from the time of injury was 1 month. The pseudoaneurysm was repaired with primary suture through a medial incision. Epineurolysis of the common peroneal nerve was performed a week later through a posterolateral incision. Pain and knee contracture resolv

Common peroneal nerve palsy following knee arthroscopy.

We report the case of a 43-year-old woman who underwent knee arthroscopy. Postoperatively, she developed a lesion of the common peroneal nerve, which was confirmed by neurophysiological studies. Exploration showed the nerve to be in continuity and externally undamaged. At review 17 months later, there was incomplete recovery. We believe this lesion was caused by a traction injury related to patient positioning, which has not been reported previously.

[Experimental study on the repair of peripheral nerve lesions--subacute compression neuropathy and neurolysis].

Neurolysis has been a widely accepted treatment for entrapment neuropathy and other nerve injuries in continuity. The purpose of this experimental study is to clarify the effects of neurolysis on the recovery from subacute compression neuropathy. The sciatic nerves of 49 mongrel dogs were used. In order to make the model of subacute compression neuropathy, the apparatus designed by Horiuchi to compress the nerve with constant force was applied to each nerve for three weeks. The pressure of compression was 55 mmHg when the thickness of the nerve was 2 mm. According to the degree of injury of th
