[Management of primary angle-closure glaucoma by phacoemulsification with foldable posterior chamber intraocular lens implantation].

OBJECTIVE: To evaluate the efficacy and safety of phacoemulsification with foldable posterior chamber intraocular lens(PC-IOL) implantation in the management of acute or chronic primary angle-closure glaucoma(APACG or CPACG) with cataract. METHODS: Twenty-six cases(28 eyes) with simple cataract and 60 cases(70 eyes)of PACG with cataract, including 14 eyes of preclinical APACG, 29 eyes of paroxysmal APACG, 27 eyes of CPACG. Visual acuity distributed from hand movement to 0.8. Phacoemulsification and intraocular lens implantation were performed and patients were followed up for(8.5+/-4.5) month…

Combined fluocinolone acetonide intraocular delivery system insertion, phacoemulsification, and intraocular lens implantation for severe uveitis.

PURPOSE: To determine whether a three-year fluocinolone acetonide sustained drug delivery system can be implanted safely at the same time that phacoemulsification and intraocular lens (IOL) implantation are performed for a visually significant cataract in eyes with uveitis. DESIGN: Retrospective, single-center case series. METHODS: All consecutive patients treated from April 1998 through September 2006 at an academic clinical practice with intermediate uveitis, posterior uveitis, or panuveitis requiring immunosuppressive therapy, periocular corticosteroid injections, or both. Phacoemulsification time (seconds) and average phacoemulsification power in foot position 3 (%) were recorded. RESULTS: Cataracts were moderate to high density, nuclear color = 3.96, and nuclear opalescence = 3.91. Endothelial cell loss = 196 cells/square millimeter ( …

Long-term effect of cataract surgery on intraocular pressure after trabeculectomy: extracapsular extraction versus phacoemulsification.

PURPOSE: To compare the effect of phacoemulsification with intraocular lens (IOL) implantation on long-term intraocular pressure (IOP) control in glaucoma patients who had previous trabeculectomy with the effect on IOP control in similar patients after extracapsular cataract extraction (ECCE) with IOL implantation. SETTING: Oxford Eye Hospital, Oxford, England. METHODS: Twenty-eight consecutive patients who had phacoemulsification with IOL implantation (phaco group) at least 3 months after trabeculectomy were identified from hospital records, and 28 patients who had ECCE with IOL implantatio…

Endothelial cell loss during the transition from nucleus expression to posterior chamber-iris plane phacoemulsification.

Corneal endothelial cell loss and other complications were analyzed in a series of 344 consecutive extracapsular cataract extraction patients operated on during one year. The cases analyzed represent the first phacoemulsification extractions performed by this surgeon and were compared with those of an already familiar technique, nucleus expression. No significant difference in overall endothelial cell loss (10.7% cell loss for nucleus expression and 10.0% cell loss for phacoemulsification) was found. Analysis of all other complications revealed no significant difference between these two group…

Intraocular Pressure Following Phacoemulsification with Anterior Vitrectomy in the IRIS® Registry (Intelligent Research in Sight).

PURPOSE: To evaluate intraocular pressure (IOP) changes following phacoemulsification with concurrent anterior vitrectomy (AV) using the IRIS® Registry (Intelligent Research in Sight). DESIGN: Retrospective clinical cohort study SUBJECTS: Eyes in the IRIS Registry that underwent stand-alone phacoemulsification with concurrent AV from 1/1/2013-9/30/2019. METHODS: Daily mean IOP was compared to baseline IOP through postoperative day 90. An IOP spike was defined as IOP >30 mm Hg and an increase of >10 mm Hg from baseline IOP within the first postoperative week. A linear mixed-effects model was …

Comparison of the incidence of postoperative ocular hypertension after pars plana vitrectomy and phacoemulsification and aspiration in dogs.

OBJECTIVE: To retrospectively evaluate whether postoperative ocular hypertension (POH) occurs after pars plana vitrectomy (PPV) for rhegmatogenous retinal detachment, as POH does after phacoemulsification and aspiration (PEA). METHODS: Medical records of dogs that underwent PPV or PEA between January 2018 and December 2021 were reviewed retrospectively. Cases were divided into 2 groups: the PPV group and the PEA group. The PPV group was further divided into 2 groups: the PPV-only group and the PEA + PPV group, which included cases that underwent PEA and PPV simultaneously. Intraocular pressur…

Comparing Outcomes of Femtosecond Laser-Assisted Cataract Surgery and Conventional Phacoemulsification in a Randomized Controlled Trial.

This protocol compares femtosecond laser-assisted cataract surgery (FLACS) with conventional phacoemulsification cataract surgery (CPCS) for age-related cataract using a standardized randomized trial workflow. A total of 113 patients were allocated 1:1 to FLACS (n = 56) or CPCS (n = 57), stratified by nuclear hardness, and assessed at Day 1, Week 1, Month 1, and Month 3. The protocol specifies eligibility screening, concealed allocation, harmonized perioperative care, predefined intraoperative settings, and masked outcome assessment. Primary and secondary outcomes included corrected and uncorr…

Automated Analysis of the Foveal Avascular Zone in Optical Coherence Tomography Angiography Before and After Phacoemulsification.

Objective: This study aimed to compare two methods for measuring the foveal avascular zone (FAZ) before and after phacoemulsification: a script-based semiautomated algorithm and a machine learning (ML)-based semiautomated algorithm. Methods: Optical coherence tomography angiography (OCTA) images were obtained with a Spectralis OCTA system (Heidelberg Engineering, Germany) preoperatively and in three postoperative visits. The FAZ was measured using both methods. Results: The study analyzed 708 OCTA scans from 59 eyes. Correlation analyses showed strong agreement between the semiautomated script…

The effects of phacoemulsification and scleral-fixated intraocular lens implantation on the choroidal vascular index: A comparative study.

This study evaluates and compares choroidal vascular changes in patients undergoing phacoemulsification and scleral-fixated intraocular lens implantation. The research focused on assessing the choroidal vascular index (CVI) and choroidal thickness as indicators of vascular and structural integrity, using normal eyes as controls. This comparative study involved 135 eyes divided into 3 groups, phacoemulsification, scleral-fixated intraocular lens implantation, and control. Choroidal thickness and CVI were evaluated using optical coherence tomography within the first 3 months after surgery. Image…

The complete effect of phacoemulsification is masked by topical intraocular pressure-lowering medications. PURPOSE: To assess the impact of phacoemulsification and intraocular lens implantation on intraocular pressure (IOP) level in individuals with primary open angle glaucoma (POAG), exfoliative glaucoma (XFG), exfoliation syndrome (XFS), normal tension glaucoma (NTG), ocular hypertension (OH), or healthy controls 12 months after the su…
