In transition: current health challenges and priorities in Sudan.

A recent symposium and workshop in Khartoum, the capital of the Republic of Sudan, brought together broad expertise from three universities to address the current burden of communicable and non-communicable diseases facing the Sudanese healthcare system. These meetings identified common challenges that impact the burden of diseases in the country, most notably gaps in data and infrastructure which are essential to inform and deliver effective interventions. Non-communicable diseases, including obesity, type 2 diabetes, renal disease and cancer are increasing dramatically, contributing to multi

Antibacterial use in the Faroe Islands, Iceland, and Denmark 1999-2011.

BACKGROUND: The Faroe Islands, Iceland, and Denmark are neighbouring Nordic countries with great ethnic, cultural, and political similarities and are relatively homogeneous. Important information about prescribing practices can be obtained by comparing the antibacterial use in these countries. The objective was to describe, compare, and analyse the use of systemic antibacterial agents in these countries during the y 1999-2011. METHODS: Data were obtained from the Faroe Islands, Iceland, and Denmark on systemic antibacterial use and expressed in defined daily dosages (DDD). Prescription data w

Africa's unpaid outbreak frontline workers: The political economy of community health worker neglect.

Community health workers (CHWs) in Africa perform indispensable functions across the outbreak response cycle. These include early case detection, contact tracing, risk communication and zoonotic interface monitoring. They depend on community embeddedness, an asset the formal health system cannot replicate in emergencies. Yet despite a substantial evidence base, more than 80% of Africa's 1.4 million CHWs operate under volunteerism frameworks, without formal employment, salaries or enforceable protections. This is not a consequence of resource scarcity. It is a deliberate programme design choice

Rethinking Organisational Justice in Healthcare Through Nurses' Voices: Insights From Nancy Fraser's Theory of Justice.

Organisational justice has emerged as a key area of interest given the challenges of protecting the health and well-being of nurses in healthcare organisations. Existing research shows significant conceptual and methodological limitations that must be addressed to achieve a more comprehensive and ethically grounded understanding of nurses' needs in the healthcare system. This article explores how nurses experience their working conditions within public healthcare organisations through a theoretical lens of social justice to unveil the systemic injustices embedded within them. Adopting a critic

How does the Nigerian health system serve people living with disabilities? A socio-ecological analysis on people living with visual impairment.

People living with disabilities (PLWD) in Nigeria experience significant disparities in health outcomes and access to care. Among them, individuals living with visual impairment (VI) face compounded barriers due to structural inequities and social exclusion. This paper explores how the Nigerian health system serves PLWD through a socioecological lens, with a focus on those living with VI. We conducted a narrative review using databases such as Medline, Scopus, Embase and Google Scholar, along with policy repositories, to identify peer-reviewed and grey literature. Inclusion criteria focused on

A global public health convention for the 21st century.

As shown by COVID-19, infectious diseases with a pandemic potential present a grave threat to health and wellbeing. Although the International Health Regulations provide a framework of binding legal obligations for pandemic prevention, preparedness, and response, many countries do not comply with these regulations. There is a need for a renewed framework for global collective action that ensures conformity with international regulations and promotes effective prevention and response to pandemic infectious diseases. This Health Policy identifies the necessary characteristics for a new global pu

Resilience testing in action - piloting the health system resilience testing tool with a pandemic scenario in Finland.

BACKGROUND: System-wide approaches to measure, prepare for and manage the next acute shock are needed. We document the application of the health system resilience testing tool to a hypothetical pandemic scenario in Finland. METHODS: The resilience testing tool promoted pre-crisis identification of resilience gaps and was built on the Health Systems Performance Assessment Framework and the Shock Cycle Framework. It included guidance on building a shock scenario, conducting a semi-structured resilience testing dialogue with health system stakeholders, and evaluating resilience. A hypothetical s

South African children: a history of adversity, violence and trauma.

This paper examines the adverse situation that continues to face South African children. Children have until very recently, been regarded as one of the most neglected and overtly oppressed sectors of South African society. The installation of a democratic government ushered in a range of policies and measures designed to promote the wellbeing of children. Despite this new commitment and support, the position of children has remained one of considerable concern. In this paper we report on a range of social, educational, health and psychological indicators of the wellbeing of South African child

Strengthening Kenya's public health response to reproductive coercion and intimate partner violence in family planning clinics: applying the FRAME + IS approach.

BACKGROUND: Reproductive coercion (RC) and intimate partner violence (IPV) undermine reproductive autonomy and are prevalent among women seeking family planning (FP) services. In response, Kenya's Ministry of Health (MOH) selected ARCHES (Addressing Reproductive Coercion in Health Settings), an evidence-based intervention (EBI) integrating universal education, screening, and support on RC and IPV during routine FP counseling, for national adaptation and scale-up within a hybrid implementation-effectiveness trial. Institutionalizing such interventions within public health systems requires caref

Support usability, institutional trust, and psychological adaptation among international students from war-affected regions in Chinese universities: a mixed-methods study.

Conflict-driven mobility is reshaping international higher education and raising new questions about how universities support students whose learning trajectories have been disrupted by war, political violence, or prolonged instability. Although international student adaptation has been widely examined through the lenses of cross-cultural adjustment, academic integration, and social support, less is known about how students from war-affected regions psychologically experience formal university support as visible, usable, trustworthy, and safe. This gap matters because support services may form

Enhancing malaria elimination in high-transmission settings: the synergy of concurrent vector control and chemotherapy.

BACKGROUND: Malaria elimination, defined as interrupting local transmission and reducing cases to zero, is a critical public health goal. While a dual parasite-vector approach is essential, the path to elimination is complex and marked by both progress and setbacks. Despite renewed commitment and initiatives like the "High Burden High Impact" approach, challenges persist, particularly in sub-Saharan Africa. These include shifting epidemiological profiles, weak health systems, drug and insecticide resistance, and emerging global issues. Effective elimination, therefore, requires a multi-pronged

Systemic inequalities in indoor air pollution exposure in London, UK.

UNLABELLED: Deprived communities in many cities are exposed to higher levels of outdoor air pollution, and there is increasing evidence of similar disparities for indoor air pollution exposure. There is a need to understand the drivers for this exposure disparity in order to develop effective interventions aimed at improving population health and reducing health inequities. With a focus on London, UK, this paper assembles evidence to examine why indoor exposure to PM2.5, NOx and CO may disproportionately impact low-income groups. In particular, five factors are explored, namely: housing locati

Overview of the epidemiological conditions of HIV among key populations in Africa.

INTRODUCTION: Despite extraordinary progress in HIV treatment coverage and expanding access to HIV prevention services and that multiple African countries are on track in their efforts to reach 90-90-90 goals, the epidemic continues to persist, with prevalence and incidence rates too high in some parts of the continent to achieve epidemic control. While data sources are improving, and research studies on key populations in specific contexts have improved, work on understanding the HIV burdens and barriers to services for these populations remains sparse, uneven and absent altogether in multipl

Data mining and analysis of scientific research data records on Covid-19 mortality, immunity, and vaccine development - In the first wave of the Covid-19 pandemic.

BACKGROUND AND AIMS: Covid-19 is a global pandemic that requires a global and integrated response of all national medical and healthcare systems. Covid-19 exposed the need for timely response and data sharing on fast spreading global pandemics. In this study, we investigate the scientific research response from the early stages of the pandemic, and we review key findings on how the early warning systems developed in previous epidemics responded to contain the virus. METHODS: We conducted data mining of scientific literature records from the Web of Science Core Collection, using the topics Cov

Strengthening the health systems at national level for malaria elimination in the Greater Mekong Subregion countries: a qualitative study.

BACKGROUND: Countries in the Greater Mekong Subregion (GMS) have committed to eliminating malaria by 2030. The success of a national malaria programme's transition from malaria control to elimination is dependent on the readiness of the health system to implement malaria elimination strategies. Understanding the readiness of health systems and what needs to be adapted is key to identifying barriers in achieving malaria elimination goals. This study aims to assess health system needs for malaria elimination, identify national-level barriers to effective implementation, and provide recommendatio

Reimagining Paediatric HIV Care: Centering Infants and Young Children in Optimized, Integrated Models of Care.

INTRODUCTION: Despite substantial gains in the global HIV response, infants and young children continue to experience poorer outcomes across the HIV testing and care cascade. Rates of early infant diagnosis, timely antiretroviral therapy (ART) initiation, retention in care and viral suppression remain unacceptably low in paediatric populations. These disparities are compounded by systemic challenges, including fragmented service delivery, limited availability of child-friendly ART and other drug formulations, and health systems that fail to integrate HIV services within broader maternal and ch

Integrating the prevention of mother-to-child transmission of HIV into primary healthcare services after AIDS denialism in South Africa: perspectives of experts and health care workers - a qualitative study.

BACKGROUND: Integrating Prevention of Mother-to-Child Transmission (PMTCT) programmes into routine health services under complex socio-political and health system conditions is a priority and a challenge. The successful rollout of PMTCT in sub-Saharan Africa has decreased Human Immunodeficiency Virus (HIV), reduced child mortality and improved maternal health. In South Africa, PMTCT is now integrated into existing primary health care (PHC) services and this experience could serve as a relevant example for integrating other programmes into comprehensive primary care. This study explored the per

Countdown to 2015 country case studies: what have we learned about processes and progress towards MDGs 4 and 5?

BACKGROUND: Countdown to 2015 was a multi-institution consortium tracking progress towards Millennium Development Goals (MDGs) 4 and 5. Case studies to explore factors contributing to progress (or lack of progress) in reproductive, maternal, newborn and child health (RMNCH) were undertaken in: Afghanistan, Bangladesh, China, Ethiopia, Kenya, Malawi, Niger, Pakistan, Peru, and Tanzania. This paper aims to identify cross-cutting themes on how and why these countries achieved or did not achieve MDG progress. METHODS: Applying a standard evaluation framework, analyses of impact, coverage and equi
