PURPOSE: To assess the changes in the contractility of the ciliary muscle in eyes with presbyopia before and after phacoemulsification and intracapsular lens implantation using ultrasound biomicroscopy (UBM).

If presbyopia is caused by hardening of the crystalline lens, replacing it with a material with mechanical properties similar to the young crystalline lens should restore accommodative ability. Such a silicone material has been developed. Refilling the capsular bag with this material results in 3 to 5 D of accommodation in primates in response to pilocarpine.

An attempt was made to explore the validity of the Hess-Gullstrand and Duane-Fincham models of presbyopia development, on the assumption that accommodative miosis could be used as an indicator of ciliary muscle effort. A bilateral compound hyperopic astigmat, with-the-rule, with incipient presbyopia and refractive amblyopia fitted with toric (custom) soft lenses in a monofit mode is described. Presbyopia, like many age related conditions, does not present an identical picture in all patients. Most, but not all, presbyopes can be cared for satisfactorily by the use of conventional optometric corrections. Some atypical presbyopic patients, however, may benefit from the unorthodox remedial procedures mentioned in this paper.

The results are described of a 10-year study of the longitudinal changes in the static response of an individual subject during the approach to absolute presbyopia. The amplitude of accommodation declines linearly with age. A new finding is that the magnitude of the slope of the linear portion of the response/stimulus curve also reduces with age, as does the level of tonic accommodation.

As a consequence of improvement in intraocular lens (IOL) technology and the wish for presbyopia correction, modern multifocal IOLs are being implanted increasingly more often. The apodized diffractive optic represents a new design among multifocal IOLs. A clear understanding of the anatomy of accommodative structures, the mechanism of accommodation, and the causes of presbyopia is necessary to understand whether accommodation may be restored to the presbyopic eye. A survey of the literature on the incidence of the age of onset of presbyopia in various parts of the world reveals a great deal of variation. At first sight this appears to correlate with geographical latitude, but statistical and epidemiological evidence rules this out in favour of ambient temperature. Indirect evidence suggests that the crystalline lens is located sufficiently near to the surface of the body for this to be a likely explanation.

New multifocal phase designs aiming at expanding depth of focus in the presbyopic eye are presented. The designs consist of multiple radial or angular zones of different powers or of combined low- and high-order aberrations. Prescribing vertical prisms not infrequently leads to unsightly spectacles at best, and computing them may lead to errors at worst. Prismatic compensation for a particular reading point is of little value when the patient uses other eccentric portions of the lens, and it is not necessary if the patient learns to use (or continues to use) the optical centers. Clinical evaluation of eye-head posture and vertical phorias may reveal tell-tale clues in managing vertical imbalances in general and the anisometropic presbyope in particular.
