Technical success and adverse event rates after endoscopic retrograde cholangiopancreatography using deep sedation with propofol.

BACKGROUND: With the increasing complexity and prolonged duration of endoscopic retrograde cholangiopancreatography (ERCP) procedures, sedation shifted from conscious sedation with benzodiazepines to deep sedation with propofol. We assessed the technical success and adverse event rates of ERCP with deep versus conscious sedation. METHODS: Consecutive patients treated with ERCP in the University Medical Center Utrecht over a 7-year period (2010-2016) were screened for eligibility. Gastroenterologist-administered conscious sedation with midazolam was used from 2010-2013, whilst anesthesiology-a

Safety aspects of deep sedation during catheter ablation of atrial fibrillation.

BACKGROUND: The combination of intravenous propofol and midazolam is frequently used to provide unconscious sedation during catheter ablation of atrial fibrillation (AF), but only a very few reports are available on the influence of prolonged propofol infusion on arterial blood gas, blood pressure, and anesthesia-associated complications during ablation of AF. The purpose of this study was to assess tolerance and safety of unconscious sedation with intravenous propofol and midazolam during catheter ablation of AF. METHODS: A total of 316 consecutive patients (age 59 ± 10 years, 68% men) prese

Colonoscopy without premedication versus barium enema: a comparison of patient discomfort.

BACKGROUND: Colonoscopy is considered a painful procedure requiring routine intravenous sedation. We investigated whether unsedated colonoscopy causes more discomfort than barium enema. METHODS: Procedure-related discomfort was determined in 100 consecutive patients undergoing colonoscopy without premedication and in an equal number of patients referred for sigmoidoscopy and barium enema. All patients underwent such examinations for the first time and had no history of previous bowel surgery. During colonoscopy, sedation was offered if significant pain or discomfort occurred. RESULTS: In pat

The Nature, Frequency, and Timing of Pediatric Sedation Adverse Events.

OBJECTIVES: The nature and frequency of pediatric sedation adverse events (AEs) have been well described. However, the timing of specific AEs in induction, procedure, and recovery phase of sedation remains unknown. The objective was to describe the nature, frequency, and timing of AEs. We hypothesized that most AEs would start at the induction phase. METHODS: We examined prospectively collected data of sedation encounters of children 3 months to 18 years of age, characterized by at least 1 AE, from January 1, 2013 to December 31, 2020. Patient characteristics, primary diagnosis, procedure typ

Effect of intravenous lidocaine on outcomes in patients receiving propofol for gastrointestinal endoscopic procedures: an updated systematic review and meta-analysis.

BACKGROUND: Gastrointestinal endoscopic procedures (GEPs) are frequently employed for the diagnosis and treatment of various gastrointestinal ailments. While propofol sedation is widely used during these procedures, there is a concern regarding its potential negative effects. Intravenous (IV) lidocaine has been suggested as an add-on to propofol sedation for GEPs, but current evidence on its efficiency and safety is limited. This systematic review and meta-analysis aimed to assess the impact of IV lidocaine on outcomes in patients receiving propofol during GEPs. METHODS: Electronic databases

Anesthesia outcomes in lysosomal disorders: CLN3 and GM1 gangliosidosis.

Natural history studies of pediatric rare neurometabolic diseases are important to understand disease pathophysiology and to inform clinical trial outcome measures. Some data collections require sedation given participants' age and neurocognitive impairment. To evaluate the safety of sedation for research procedures, we reviewed medical records between April 2017 and October 2019 from a natural history study for CLN3 (NCT03307304) and one for GM1 gangliosidosis (NCT00029965). Twenty-two CLN3 individuals underwent 28 anesthetic events (age median 11.0, IQR 8.4-15.3 years). Fifteen GM1 individua

Continuous intravenous versus bolus parenteral midazolam: a safe technique for conscious sedation in plastic surgery.

Conscious intravenous sedation is a safe alternative method to general anaesthesia. We have used a technique of continuously titrated, as opposed to incremental boluses of, intravenous or intramuscular midazolam for conscious sedation, with tumescent adrenaline-lignocaine solution for local anaesthesia, routinely in 421 plastic surgical procedures between 1997 and 2000. All patients were American Society of Anesthesiologists (ASA) class I or II. Conscious sedation was administered through our protocol of continuously titrated doses of midazolam in dextrose saline. The operative field was injec

Inhaled Methoxyflurane in Australian Emergency Departments-A Cost-Minimisation Analysis.

BACKGROUND: Around 60% of emergency department (ED) patients present with pain. With growing ED-demand, there is need for analgesic options that reduce resource use and cost. We estimate the potential cost-savings from broader use of inhaled methoxyflurane in Australian EDs compared with standard-of-care (SOC) analgesics or procedural sedation. The analysis focused on non-admitted patient presentations requiring analgesia or procedural sedation to treat dislocations, burns, fractures and other injuries. METHODS: We conducted a cost-minimisation analysis to assess differences in resource use a…

New Trends in Airway Management During Endoscopic Retrograde Cholangiopancreatography: A Narrative Review.

Over time, endoscopic retrograde cholangiopancreatography (ERCP) evolved into the preferred method for both diagnosing and treating diseases of the biliary, pancreatic, and ampullary systems. Traditionally performed under "conscious" sedation, anesthesiological management during ERCP increasingly involves the use of general anesthesia (GA) due to the complexity of procedures and patient comorbidities. This narrative review aims to underscore the current absence of definitive evidence supporting a single airway management strategy during ERCP. In each section, we examine the strengths and limit
