Role of Stent Versus Thrombolysis in Management of Cocaine-Induced ST-Elevation Myocardial Infarction.

Cocaine has been used increasingly nowadays because of its abundant availability and recreational effects. Along with that, we have been experiencing many cases presenting with cocaine intoxication and withdrawal effects, including hypertension and chest pain syndrome. In the modern era, many medical advances related to myocardial infarction treatment have been made, including not only medical therapy but also urgent or emergent reperfusion, and revascularization therapies. In percutaneous coronary revascularization therapy, a second-generation drug-eluting stent (DES) with dual antiplatelet t

Identification of Unique Metabolites of the Designer Opioid Furanyl Fentanyl.

The illicit drug market has seen an increase in designer opioids, including fentanyl and methadone analogs, and other structurally unrelated opioid agonists. The designer opioid, furanyl fentanyl, is one of many fentanyl analogs clandestinely synthesized for recreational use and contributing to the fentanyl and opioid crisis. A method has been developed and validated for the analysis of furanyl fentanyl and furanyl norfentanyl in urine specimens from pain management programs. Approximately 10% of samples from a set of 500 presumptive heroin-positive urine specimens were found to contain furany

Interactions between bupropion and 3,4-methylenedioxymethamphetamine in healthy subjects.

3,4-Methylenedioxymethamphetamine (MDMA; "ecstasy") is a popular recreational drug. The aim of the present study was to explore the role of dopamine in the psychotropic effects of MDMA using bupropion to inhibit the dopamine and norepinephrine transporters through which MDMA releases dopamine and norepinephrine by investigating. The pharmacodynamic and pharmacokinetic interactions between bupropion and MDMA in 16 healthy subjects were investigated using a double-blind, placebo-controlled, crossover design. Bupropion reduced the MDMA-induced elevations in plasma norepinephrine concentrations an

[Ecstasy poisoning in a 10-month-old infant].

UNLABELLED: We describe an accidental ecstasy poisoning in a 10-month-old girl. CASE REPORT: A 10-month-old infant suddenly exhibited behavior disorders with restlessness, hypertonia, and repetitive movements. She was immediately taken to the Emergency Department. Cardiovascular (tachycardia and hypertension) and neurological disorders (major agitation, mydriasis, and nystagmus) were the predominating clinical features. The most common differential diagnoses including febrile convulsion, cranial trauma, or encephalitis were progressively ruled out. The urinary toxicological screen was positiv

Cerebral activation in abstinent ecstasy (MDMA) users during a working memory task: a functional magnetic resonance imaging (fMRI) study.

The popular recreational drug ecstasy (3,4-methylenedioxymethamphetamine=MDMA and related congeners) is neurotoxic upon central serotonergic systems in animal studies. So far, the most convincing evidence for neurotoxicity-related functional deficits in humans derives from neurocognitive studies demonstrating dose-related long-term learning and memory problems in ecstasy users. In our study we used functional magnetic resonance imaging (fMRI) and a working memory task to investigate cerebral activation in eleven heavy, but currently abstinent MDMA users and two equally sized groups of moderate

The Dark Factor of Personality and Risk-Taking.

Aversive personality traits have been linked to risk-taking across various domains. Herein, we investigated whether the common core of aversive traits, the Dark Factor of Personality (D), is related to risk-taking. Whereas the conceptualizations of D (common core of aversive traits) and risk-taking (not inherently socially and/or ethically aversive) do not necessarily imply an association, several theoretical considerations do suggest a positive relation between the constructs. In three studies (overall n = 689), we linked D to various self-report measures of risk-taking (Studies 1 and 2), as

Cocaine decreases relative cerebral blood volume in humans: a dynamic susceptibility contrast magnetic resonance imaging study.

Cocaine has substantial effects on cerebral hemodynamics which may partly underlie both its euphorigenic and toxic effects. Dynamic susceptibility contrast magnetic resonance imaging (DSC-MRI) was used to determine whether a dose-effect relationship could be detected between cocaine administration and cerebral blood volume reduction in human brain. Twenty-three healthy and neurologically normal adult males with a history of recreational cocaine use (3-40 lifetime exposures) participated. Subjects underwent DSC-MRI measurements of relative cerebral blood volume (rCBV) at baseline and 10 min aft

Latent classes of substance use patterns and cannabis use disorder Risk in U.S. Adults during recreational Legalization expansion (2015-2023).

BACKGROUND: Traditional surveillance may not capture heterogeneous patterns of cannabis use disorder (CUD) risk during policy transitions. We used latent class analysis (LCA) to identify substance use patterns associated with varying CUD risk during U.S. recreational cannabis legalization expansion (2015-2023). METHODS: We analyzed 326,193 U.S. adults aged 18 + from the National Survey on Drug Use and Health (2015-2023, excluding 2021). LCA identified classes based on eight indicators: past-year alcohol and cannabis use disorders, lifetime use of alcohol, cannabis, cocaine, and heroin, and ar

Cocaine use in university students: relationships with demographics, mental health, risky sexual practices, and trait impulsivity.

BACKGROUND: Cocaine is increasingly used on a recreational basis by the general population with potential implications for mental health. The aim of this study was to assess how common cocaine use is, and its mental health associations, in a large sample of university students. METHODS: Approximately 10 000 university students were invited to take part in an online survey, which assessed the use of cocaine (ever or past year), alcohol and drug use, mental health issues, and impulsive and compulsive tendencies. Group differences in demographic and clinical characteristics were characterized.

Cannabinoid Positivity in Traffic Fatalities Before and After Cannabinoid Legalization: A Retrospective Analysis in Connecticut (2018-2024).

Cannabinoid positivity in motor vehicle collision (MVC) fatalities is a topic of public interest, particularly as cannabis legalization expands. In Connecticut, recreational cannabis legalization in 2021 raised questions about impairment in fatal motor vehicle crashes. A retrospective study using Connecticut Office of the Chief Medical Examiner (CT OCME) data investigated MVC fatalities between 2018 and 2024, providing 3.5 years of data before and after legalization. We examined 1168 driver, 380 pedestrian, and 52 suicide fatalities. For the drivers, cannabinoid positivity increased from 32.8%

[The cognitive effects of ecstasy].

The recreational drug ecstasy is widely used among dance clubbers for its acute euphoric and entactogenic effects. Ecstasy exerts its acute effects by increasing the extracellular concentration of monoamines in the brain by reversing the functions of reuptake mechanisms. These elevations in extracellular monoamine concentrations result in wake promoting effects, body hyperthermia and reductions in local cerebral blood flow. However, on the long-run, ecstasy reduces serotonin concentration and density of serotonergic markers in several brain areas. Functional deficits, like sleep disturbances,

Synthetic opioids: a review and clinical update.

The term 'opioids' refers to both the natural compounds ('opiates') which are extracted from the opium poppy plant (Papaver somniferum) and their semi-synthetic and synthetic derivatives. They all possess relatively similar biochemical profiles and interact with the opioid receptors within the human body to produce a wide range of physiological effects. They have historically been used for medicinal purposes, their analgesic and sedative effects, and in the management of chronic and severe pain. They have also been used for non-medicinal and recreational purposes to produce feelings of relaxat

Cocaine-Induced Pituitary and Subdural Brain Abscesses and the Treatment Challenges.

Cocaine is a well-known recreational drug with stimulant effects associated with relevant social, economic, and clinical implications. The most common route of abuse is via snorting. It has high addictive potential. Furthermore, one of the most well-known symptoms of a chronic user is chronic rhinitis. In the medical journals, there are numerous reports of complications, including lesions affecting the nasal septum, nasal sinuses, and even brain abscesses. We came across a 41-year-old male patient with severe manifestations of chronic cocaine use involving nasal, paranasal, and visual symptoms

Pyoderma gangrenosum (PG) is an inflammatory, ulcerative condition that is characterized by painful ulcers that commonly present on the lower extremities. Up to half of PG cases are associated with underlying systemic disease, including inflammatory bowel disease, various autoimmune conditions, and malignancy. Another well-known association is the manifestation of PG with recreational cocaine use, especially cocaine contaminated with the adulterant agent levamisole. Once utilized for its immunomodulatory capabilities, levamisole was withdrawn from the market in 2002. It has since been repurpos
