The survival gap between young and older patients after surgical resection for colorectal cancer remains largely based on early mortality: A EURECCA comparison of four European countries.

BACKGROUND: A decade ago, it was demonstrated that the difference in survival between older patients and younger patients with colorectal cancer (CRC) was mainly due to mortality in the first postoperative year. Over the last few years, improvements - especially in perioperative care - have increased survival. The current research investigates whether a survival gap between younger and older patients with CRC still exists on a national level in four European countries. METHODS: Population-based data from Belgium, the Netherlands, Norway, and Sweden were collected from patients that underwent

[Heat probe: its use in the palliation of stenosing and bleeding tumors of the digestive tract. Preliminary report].

We present our experience with the use of Heater Probe (H.P.) of the management of malignant stenoses of Gastrointestinal (G.I.) tract. We treated 6 patients: 4 with esophageal cancer, 1 cancer of rectum and 1 malignant infiltration of sigmoid colon. The symptoms were: dysphagia: 4 cases, rectal bleeding: 1 case and bleeding plus obstruction: 1 case. The symptoms were relieved in 5 of the 6 cases. In 1 case with dysphagia there were not response. The H.P. can represent a valid alternative in the palliation of malignant obstruction.

Ornithine decarboxylase activity in hepatic and extrahepatic metastases from colorectal cancer.

The values of ornithine decarboxylase (ODC) activity in hepatic and extrahepatic metastases from primary colorectal cancer were studied. Adjacent noninvolved tissue was used as a control. Liver metastases had significant elevated ODC levels over surrounding liver (0.271 vs. 0.065, respectively, P less than .008). Results similar to those found in liver metastases were noted in extrahepatic metastases (median value, 0.271). This study discusses the possible reasons for these elevations and emphasizes that these differences may have potential roles in the areas of diagnosis, staging, monitoring

The McKittrick-Wheelock syndrome: a rare cause of curable diabetes.

UNLABELLED: McKittrick-Wheelock syndrome (MWS) is a rare consequence of severe dehydration and electrolyte depletion due to mucinous diarrhoea secondary to a rectosigmoid villous adenoma. Reported cases of MWS commonly describe hypersecretion of mucinous diarrhoea in association with dehydration, hypokalaemia, hyponatraemia, hypochloraemia and pre-renal azotemia. Hyperglycaemia and diabetes are rarely reported manifestations of MWS. Herein we describe the case of a 59-year-old woman who presented with new-onset diabetes and severe electrolyte derangement due to a giant rectal villous adenoma.

Well-differentiated papillary mesothelioma, possibly giving rise to diffuse malignant mesothelioma: a case report.

Well-differentiated papillary mesothelioma (WDPM) is a distinct subtype of mesothelial tumor from diffuse malignant mesothelioma (DMM), with an uncertain malignant potential. The relationship between WDPM and DMM, with regard to the ability of the former to develop into the latter, is also unknown. A 58-year-old woman, diagnosed with a rectal carcinoid tumor, underwent removal of the lymph nodes via the abdomen in 2004. A large number of white miliary nodules were identified on the mesentery and peritoneum, which were histologically diagnosed as WDPM. No further therapy was administered, but t

Organ and Function Preservation in Gastrointestinal Malignancies.

Radiation plays an important role in organ preservation for gastrointestinal malignancies, with a watch and wait strategy enabling surgery to be avoided in patients who are not suitable or who are refusing surgery. Brachytherapy boost allows the radiation dose to be escalated, which plays a pivotal role in the successful outcome of achieving organ preservation. Here we describe the role of brachytherapy in two common gastrointestinal malignancies (oesophagus and rectum). Their indications and how the brachytherapy procedures are carried out, together with the dose and fractionation commonly us

Machine learning based prediction of recurrence after curative resection for rectal cancer.

PURPOSE: Patients with rectal cancer without distant metastases are typically treated with radical surgery. Post curative resection, several factors can affect tumor recurrence. This study aimed to analyze factors related to rectal cancer recurrence after curative resection using different machine learning techniques. METHODS: Consecutive patients who underwent curative surgery for rectal cancer between 2004 and 2018 at Gil Medical Center were included. Patients with stage IV disease, colon cancer, anal cancer, other recurrent cancer, emergency surgery, or hereditary malignancies were exclude

Predictors and Outcomes of Upstaging in Rectal Cancer Patients Who Did Not Receive Preoperative Therapy.

BACKGROUND: Preoperative chemoradiation is indicated for clinical stage II and III rectal cancers; however, the accuracy of clinical staging with preoperative imaging is imperfect. OBJECTIVE: The study aimed to better characterize the incidence and management of clinical and pathologic stage discordances in patients who did not receive preoperative chemoradiation. DESIGN: This was a retrospective cohort analysis. SETTINGS: The source of data was the National Cancer Database from 2006 to 2015. PATIENTS: We identified patients who underwent resection with curative intent for clinical stage I

Pictorial essay on MRI local staging of rectal carcinoma: An easy approach.

Rectal cancer is common and accounts for more than one-third of colorectal tumors. It is associated with significant morbidity and mortality. Previously computed tomography scan is the key imaging modality in preoperative assessment to detect local invasion and distant metastasis. However, the advent of magnetic resonance imaging (MRI) has aided in local staging and prognosticates the outcome of rectal tumor. Here, the author briefly explains why rectal MRI has a comprehensive role and provides a simple and easy way in reporting an MRI rectal carcinoma, even for a non-radiologist.

Living in limbo: Meanings of living with fecal incontinence as narrated by women after treatment for pelvic cancer.

PURPOSE: After pelvic radiotherapy, individuals suffer from loose stools and defecation urgency, often resulting in fecal incontinence (FI). Women who have been treated for pelvic cancer report FI as one of the most troubling symptoms, yet they avoid seeking healthcare due to shame and stigmatization. There is a lack of knowledge concerning women's lived experiences of FI in daily life after pelvic radiotherapy. The objective was to illuminate meanings of living with FI among women previously treated with radiotherapy for gynecological or rectal cancer. METHOD: Interviews were performed with

Staple-transection of the dorsal venous complex and urethra in cooperative laparoscopic and transperineal endoscopic total pelvic exenteration for pelvic malignancies.

INTRODUCTION: Herein, we describe our novel technique for transecting the dorsal venous complex (DVC) and urethra using a linear stapler inserted through a perineal port during endoscopic pelvic exenteration for pelvic malignancies. MATERIALS AND SURGICAL TECHNIQUE: First, a laparoscopic approach is made using a conventional 5-trocar pneumoperitoneum technique. When the Retzius and paravesical spaces are dissected to expose the tendinous arch of the levator ani muscle, the transperineal approach is synchronously begun. After the levator ani muscle and bilateral puboprostatic ligaments are dis

Nerve plane: An optimal surgical plane for laparoscopic rectal cancer surgery?

Radical resection for rectal cancer with total mesorectal excision has been widely recognized in mid-low rectal cancer. Although such surgery reduced the tumor recurrence rate and improved the survival rate of patients, the rate of urinary and sexual dysfunction was high after rectal cancer surgery, which might be attributed to pelvic autonomic nerve injury. The present study found that the pelvic autonomic nerves never exist alone. These are always surrounded by tiny capillaries and adipose tissue and covered by a thin layer of membranous tissue, leading to a continuous plane that should be p

[A Case of Laparoscopic Resection for Rectal Cancer with Persistent Descending Mesocolon].

A 66-year-old man presenting with melena was diagnosed with rectosigmoid cancer and underwent laparoscopic high anterior resection. Intraoperative findings showed that the descending colon did not adhere to the retroperitoneum and was largely displaced inward, and the descending and sigmoid colon extensively and strongly adhered to the small intestinal mesentery, as predicted prior to surgery. The patient was diagnosed with persistent descending mesocolon(PDM). The first sigmoid artery diverged from the left colonic artery; however, ligation of supplying arteries under laparoscopy was perceive

Management of locally advanced synchronous colorectal and prostate cancers: A case report.

INTRODUCTION: Synchronous colorectal and prostate malignancies are uncommon, with standard treatment guidelines not yet established. Chemoradiation therapy is involved in both colorectal and prostate cancers. However, differing dosage regimens and effects of irradiation field on anatomical planes for surgery makes management of the synchronous cancers challenging. We report the first case of synchronous prostate and rectal cancer being treated with a combination of treatment modalities with a unique addition of high dose rate prostate brachytherapy boost. PATIENT CONCERNS: The patient, a 69-y
