Videourodynamic analysis of pathophysiology of men with both storage and voiding lower urinary tract symptoms.

OBJECTIVES: Lower urinary tract symptoms (LUTS) are highly prevalent among men; however, not all men with LUTS have urodynamically confirmed bladder outlet obstruction. Overactive bladder symptoms are often caused by bladder dysfunctions alone or in combination with bladder outlet obstruction. A detailed diagnosis to identify the causes of LUTS in men seems mandatory to correctly target different therapy to the various underlying pathologic features. METHODS: A total of 1407 male patients referred for investigation of LUTS were included in this study. All patients had both storage and voiding

[Achalasia--early diagnosis and treatment options].

Achalasia is a rare motility disorder of the distal esophagus, showing reduced or absent swallow induced relaxation of the lower esophageal sphincter region. The onset of the disease can be very slow and correct diagnosis might be delayed over years. Using exact medical history, upper GI endoscopy and barium swallow X-ray the achalasia can be suspected and can be confirmed by esophageal manometry. Other especially malignant diseases, which can imitate the clinical symptoms, have to be ruled out. Pneumatic balloon dilatation is therapy of first choice in achalasia. Especially in younger patient

Vagus nerve stimulation promotes gastric emptying by increasing pyloric opening measured with magnetic resonance imaging.

BACKGROUND: Vagus nerve stimulation (VNS) is an emerging electroceutical therapy for remedying gastric disorders that are poorly managed by pharmacological treatments and/or dietary changes. Such therapy seems promising as the vagovagal neurocircuitry modulates the enteric nervous system to influence gastric functions. METHODS: Here, the modulatory effects of left cervical VNS on gastric emptying in rats were quantified using a (i) feeding protocol in which the animal voluntarily consumed a postfast, gadolinium-labeled meal and (ii) a non-invasive imaging method to measure antral motility, py

Integrative Medicine Modalities for Adolescent Patients Undergoing Minimally Invasive Repair of Pectus Excavatum: A Scoping Review.

BACKGROUND: The minimally invasive repair of pectus excavatum (MIRPE), a surgical procedure done primarily in adolescence to correct pectus excavatum (PE), a congenital chest wall deformity, is associated with significant postoperative pain and opioid consumption. The use of an integrative medicine (IM) approach-combining complementary therapies with conventional treatments-can support postoperative pain management and potentially reduce opioid consumption in adolescent patients undergoing this procedure. This scoping review examines the published literature addressing the use of IM modalities

[Intrathecal baclofen therapy: a 30-year experience].

BACKGROUND: The first pump for intrathecal administration of baclofen was implanted in 1984. Over thirty years, intrathecal prolonged infusion of muscle relaxants has occupied a worthy niche among all methods for correction of non-focal drug-resistant disabling muscle spasticity. However, this method has not become routine despite high awareness of specialists in Russia and abroad, as well as undeniable advantages for restoring the daily activity, improving the walking pattern and providing care and quality of life in people with limited mobility. This is due to scrupulous analysis of adverse

Comparative Efficacy of Pharmacological Therapies for Low Back Pain: A Bayesian Network Analysis.

Low back pain (LBP) is a common problem, but the efficacy of pharmacological therapies remains controversial. Therefore, we aimed to comprehensively evaluate and quantitatively rank various pharmacological therapies for patients with low back pain. Two meta-analyses were performed: an initial pair-wise meta-analysis, followed by network meta-analysis using a random-effects Bayesian model. We included randomized controlled trials comparing placebos, non-steroidal anti-inflammatory drugs, opioids, skeletal muscular relaxants, pregabalin (or gabapentin), and some drug combinations. The primary an

Art therapy with cancer patients during chemotherapy sessions: an analysis of the patients' perception of helpfulness.

OBJECTIVE: Art therapy has been shown to be helpful to cancer patients at different stages in the course of their illness, especially during isolation for bone marrow transplantation, during radiotherapy treatment, and after treatment. The aim of this study is twofold: (1) to assess whether patients during chemotherapy sessions perceive art therapy as helpful and (2) to outline in which way art therapy is perceived as helpful. METHOD: 157 cancer patients attending an Oncology Day Hospital (Siena, Italy) met the art therapist during their chemotherapy sessions. The art therapist used the same

Effects of a Muscle Relaxation Technique on Catatonia Symptoms Associated With Schizophrenia: A Case Report.

Catatonia is characterized by the loss of voluntary control over the workings of the mind and body. It disrupts daily life by manifesting as idle posture, heightened muscle tone, and repetitive purposeless movements. However, specific physiotherapy methods addressing these symptoms are yet to be established. This case report describes a 63-year-old man hospitalized for schizophrenia who was then diagnosed with stuporous catatonia based on the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV-TR) criteria, characterized by catalepsy, mutism, and diffic

Early alcoholism treatment: the Emmanuel Movement and Richard Peabody.

The history of alcoholism treatment in the early twentieth century is outlined. The methods of the Emmanuel Movement and of Richard Peabody … s, including alcoholism, had physical, mental and spiritual components. His principal techniques of relaxation therapy and suggestion (including autosuggestion) were used to reach the unconscious. Worcester felt that alcoholics could be helped by redirecting their attention away from their problems to a life of service and spirituality. Prayer, group support and sel …

A biopsychological approach to temporomandibular joint pain and other chronic facial pain. Part II: Broadening of spectrum of treatments.

The multidimensional nature of chronic pain often requires a variety of therapies, ranging from reassurance and relaxation training to invasive treatment. The high spontaneous rate of recovery in temporomandibular dysfunction can lead to overtreatment of a benign disorder. However, a tendency to undertreat chronic facial pain and malignant pain conditions also exists. In therapy-resistant chronic pain cases, alternative therapies merit consideration, even though they may generally be rejected by Western medicine as unscientific and no better than placebo. Working with chronic pain patients is

Bronchorelaxant and anti-inflammatory profile of ensifentrine alone and combined with inhaled medications in lipopolysaccharide-challenged human airways from chronic obstructive pulmonary disease donors.

The ENHANCE trials demonstrated that ensifentrine (CAS Number 1884461-72-6), an inhaled dual phosphodiesterase (PDE) 3/4 inhibitor, offers additional benefits in chronic obstructive pulmonary disease (COPD). Its bifunctional activity warrants pharmacological investigation to elucidate mechanisms behind lung function improvements and exacerbation reduction. Human bronchial tissues from COPD donors were examined after lipopolysaccharide challenge. The effects of ensifentrine on bronchial relaxation were evaluated in response to carbachol and electrical field stimulation (EFS), also in combinatio

Treatment of segmental continuous hypertrophic myokymia of the limb with botulinum A toxin.

Myokymia is defined as fluctuating hyperexcitability of muscle fibers caused by repetitive spontaneous contraction of motor units. Myokymia is generally benign with self-resolution, although symptomatic treatment with benzodiazepines, anticonvulsants, and muscle relaxants can be used. Botulinum toxins can also be utilized, although they are mostly used for symptomatic facial myokymia. Here, we report two patients who developed continuous myokymia, resulting in secondary hypertrophy, stiffness, and discomfort in the affected muscles. The first patient had a history of a tethered spinal cord and

Drug and Nondrug Treatment in Tension-type Headache.

Tension-type headache (TTH) is a common primary headache with tremendous socioeconomic impact. Establishment of an accurate diagnosis is important before initiation of any treatment. Nondrug management is crucial. Information, reassurance and identification of trigger factors may be rewarding. Psychological treatments with scientific evidence for efficacy include relaxation training, EMG biofeedback and cognitive-behavioural therapy. Physical therapy and acupuncture are widely used, but the scientific evidence for efficacy is sparse. Simple analgesics are the mainstays for treatment of episodi

Specific phobias.

Exposure based treatments in which patients are systematically confronted with their feared objects of situations are highly effective in the treatment of specific phobias and produce stable improvement both in reported fear and behavioral avoidance. Exposure in reality is more effective in most cases than exposure in sensu. For situations that are difficult to realize, exposure in virtual environments has become increasingly valuable. Exposure in vivo is clearly superior to pharmacotherapy, although cognitive enhancers have been successfully used recently to increase the effect of exposure th

Stress management at the worksite for hypertension: compliance, cost-benefit, health care and hypertension-related variables.

The blood pressure-lowering effects of a group stress management program conducted with hypertensive employees at the worksite were assessed and replicated. Both systolic (SBP) and diastolic (DBP) blood pressure were significantly reduced from a baseline period to the end of a 10-week stress management training period (Group T1, N = 22). A control Group, T2 (N = 18), showed no significant SBP and DBP reduction over a comparable time period. When Group T2 was then given stress management training, the resulting SBP and DBP reductions were significant. When BP was recorded 3 years later, there w
