Tropical spastic paraparesis (TSP) or human T-lymphotropic virus type 1 (HTLV-1) myelopathy is a chronic inflammatory disease of the spinal cord caused by HTLV-1. It usually presents as a disease of the lower extremities, characterized by paralysis and sensory loss. HTLV-1 is common. However, only a small fraction of those infected actually experience clinical symptoms. Respiratory complications are rare; this case discusses an uncommon presentation of acute respiratory failure in a patient with TSP. A bedridden 73-year-old female with HTLV-1-associated TSP, presenting with progressive deterio

High cervical spinal cord injuries lead to permanent respiratory deficits. One preclinical model of respiratory insufficiency in adult rats is the C2 partial injury which causes unilateral diaphragm paralysis. This model allows the investigation of a particular population of respiratory bulbospinal axons which cross the midline at C3-C6 spinal segment, namely the crossed phrenic pathway. Transcranial magnetic stimulation (TMS) is a non-invasive technique that can be used to study supraspinal descending respiratory pathways in the rat. Interestingly, a lateral C2 injury does not affect the ampl

A man in his 70s with Parkinson's disease underwent urgent below-elbow amputation for gangrenous sepsis. A right supraclavicular brachial plexus catheter was placed for postoperative analgesia. Shortly after tracheal extubation, he developed acute hypercapnic respiratory failure due to ipsilateral hemidiaphragmatic paresis, requiring urgent tracheal re-intubation and a brief period of mechanical ventilation. Following cessation of the local anaesthetic infusion, diaphragmatic function returned and he was successfully re-extubated. A reduced-dose supraclavicular infusion was later reintroduced

Vocal cord dysfunction (VCD) is an important differential diagnosis in acute dyspnoea. VCD is caused by adduction of the vocal folds during respiration typically leading to attacks of dyspnoea, stridor and coughing. Here we present an extraordinary case of an untypical paradoxical vocal fold adduction in a 15-year-old girl with stridor persisting over several weeks.

The finding of hemidiaphragm elevation on a chest radiograph, in absence of an ipsilateral lung disease, is assumed to indicate severe hemidiaphragm dysfunction. To test this hypothesis we retrospectively reviewed chest radiograph findings and corresponding twitch transdiaphragmatic pressure (T(W)P(DI)) results from 42 (17 female, age range 22-79 years) consecutive patients who underwent phrenic nerve stimulation studies. Chest radiographs were independently reviewed in a blind manner by two radiologists. The interobserver agreement was moderate, the kappa value ranging from 0.48 (left hemidia

A 56-year-woman with type 2 respiratory failure due to diaphragmatic dysfunction in Charcot-Marie-Tooth disease (CMT) is reported. The patient, who had a 50-year history of CMT, was referred to our hospital because of nocturnal dyspnea. Arterial blood gas analysis on admission showed marked hypoxia with hypercapnia, and physical examination revealed thoracoabdominal paradoxus in the supine position. Chest radiography revealed elevation of both sides of the diaphragm. The vital capacity and arterial blood gas pressure in the sitting position were markedly higher than those in the supine positio

A 33-year-old man suffered chronic ventilatory insufficiency with tetraplegia after an operation for atlantoaxial dislocation. He was alert and his tetraplegia gradually improved. However, continuous mechanical ventilation was necessary for him. Thirteen months after the operation, a diaphragm pacer (Avery Laboratory Inc.) was implanted on the right phrenic nerve in the cervical region. Nineteen days after the implantation of the right side pacer, a left side diaphragm pacer was also implanted. Diaphragm pacing was started two weeks after the second implantation. The pacing period was graduall

Autosomal recessive spinal muscular atrophy with respiratory distress type 1 (SMARD1) is the second anterior horn cell disease in infants in which the genetic defect has been defined. SMARD1 results from mutations in the gene encoding the immunoglobulin micro-binding protein 2 (IGHMBP2) on chromosome 11q13. Our aim was to review the clinical features of 29 infants affected with SMARD1 and report on 26 novel IGHMBP2 mutations. Intrauterine growth retardation, weak cry, and foot deformities were the earliest symptoms of SMARD1. Most patients presented at the age of 1 to 6 months with respiratory

Diaphragmatic paralysis (DP) is a rare cause of respiratory distress in young children. In the first years of life, the main cause is phrenic nerve injury after cardiothoracic surgery or obstetrical trauma. DP usually presents as respiratory distress. Asymmetrical thorax elevation, difficulty weaning from mechanical ventilation, pulmonary atelectasis, and repeated pulmonary infections are other suggestive signs or complications. DP is usually suspected on chest X-ray showing abnormal hemidiaphragm elevation. Although fluoroscopy was considered the gold standard for DP confirmation, it has grad

A best evidence topic in thoracic surgery was written according to a structured protocol. The question addressed was as follows: In adults with unilateral diaphragmatic paralysis, does diaphragmatic plication offer functional improvement in dyspnoea, better pulmonary function tests (PFTs) and return to activity? A total of 126 papers were found using the reported search, of which 13 represented the best evidence to answer the clinical question. The authors, date and country of publication, patient group studied, surgical approach, study type, relevant outcomes and results of these articles are

The purpose of this study was to determine whether there is a difference in respiratory mechanics and gas exchange between polio survivors and healthy, age-matched controls during wakefulness and sleep. Polio survivors were divided into four groups. The first group included those who had evidence of respiratory muscle involvement originally (PRM) and the second group included those who had bulbar muscle involvement originally (PBM). The third and fourth groups had only limb involvement originally but were separated by absence (PSL) or presence of a scoliosis (PSS) at the time of their evaluati

BACKGROUND: Vocal cord dysfunction resulting from recurrent laryngeal nerve palsy (RLNP) is a known complication following anterior cervical discectomy and fusion. RLNP occurs typically secondary to neurapraxia caused by intraoperative compression or traction on the nerve and less commonly from direct nerve injury intraoperatively in the setting of anterior cervical spine surgery. Patients with RLNP typically present with hoarseness immediately after surgery owing to unilateral vocal cord paralysis. In rare cases, there is late-onset, progressive development of RLNP that may potentially lead t

BACKGROUND: The only hereditary neurologic disorder described so far in American Staffordshire Terriers is adult-onset cerebellar degeneration secondary to ceroid lipofuscinosis. We have seen several dogs with a newly recognized neurological disease characterized by locomotor weakness with or without respiratory signs and juvenile onset consistent with degenerative polyneuropathy of genetic origin.
OBJECTIVES: To characterize a novel polyneuropathy in juvenile American Staffordshire Terriers.
ANIMALS: Fourteen American Staffordshire Terriers presented with clinical signs consistent with juve
