[Cilioretinal artery occlusion and central retinal vein occlusion complicating hyperhomocysteinemia: a case report].

INTRODUCTION: Hyperhomocysteinemia is known to be a risk factor in both retinal artery and retinal vein occlusions. We report the case of a young patient with combined occlusion of the cilioretinal artery and the central retinal vein due to hyperhomocysteinemia. PATIENTS AND METHODS: A 23-year-old patient without significant medical history, presented for sudden, painless visual loss in the right eye. Ophthalmologic examination revealed best-corrected visual acuity of the right eye 8/10 P2, and 10/10 P2 on the left. Anterior segment exam was normal in both eyes, while the right fundus reveale…

A case of central retinal artery occlusion after chiropractic manipulation of the neck.

Here we report a case of central retinal artery occlusion after chiropractic manipulation on the neck. A 49-year old man presented at the hospital because of sudden visual loss in his right eye after chiropractic neck manipulation. He had received chiropractic manipulation of the neck by a chiropractor eight days prior. When he first visited us, his best corrected visual acuity in his right eye was hand motion. A full ophthalmic examination was performed. There was cherry-red spot in the macula in his right eye. We performed a fluorescein angiogram and cervical color Doppler. The arterio-venou…

Central retinal artery occlusion treated with oxygen: a literature review and treatment algorithm.

Central retinal artery occlusion (CRAO) is an uncommon eye disorder, but one that typically produces severe and irreversible vision loss in the affected eye. The retina has a dual blood supply, with the retinal circulation supplying the inner layers and the choroidal circulation supplying the outer layers. In CRAO, vision loss results from cell death in the inner retinal layers despite relative sparing of the outer layers. If supplemental oxygen is provided, however, oxygen from the choroidal circulation may diffuse in adequate quantity to the inner layers of the retina to maintain retinal fun…

ANCA-associated vasculitis with central retinal artery occlusion developing during treatment with methimazole.

A 63-year-old woman suddenly developed central retinal artery occlusion following a slight fever while being treated with methimazole (MMI) for hyperthyroidism. She was diagnosed to have anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) based on increased inflammatory reactions with positive myeloperoxidase-ANCA in the serum. Her visual acuity remained low despite immediate treatment with corticosteroids and cyclophosphamide after cessation of MMI, which may have played a role in the pathogenesis of AAV. Central retinal artery occlusion is a rare manifestation of AAV; how…

Eyelid arteriovenous malformation treated with embolization leading to a branch retinal artery occlusion.

BACKGROUND: Arteriovenous malformations (AVMs) are serious vascular abnormalities that rarely affect the eye. However, documented cases of eyelid AVMs have been reported to occur spontaneously or after trauma. Treatment for AVMs, which generally include embolization and resection, can also pose a risk to a patient's life and vision. CASE REPORT: This report describes a 21-year-old man who experienced multiple branch artery occlusions resulting in severe visual field defects after treatment for an eyelid AVM. CONCLUSION: Eyelid AVM is a rare but potentially serious condition that should be co

Central retinal artery occlusion and ophthalmoplegia following spinal surgery in the prone position.

A 14 year old boy underwent a 7 hour long spinal surgery for scoliosis in the prone position. In the immediate postoperative period, he developed right proptosis, periorbital swelling, chemosis and total ophthalmoplegia. The vision in his right eye was only counting fingers and the intraocular pressure was 68 mmHg. Fundus examination revealed occlusion of the right central retinal artery. A rare manifestation of both vein and artery occlusion was possible in this patient as a result of external ocular compression due to a prolonged period in the prone position. This report highlights the impor…

Molecular and histological changes following central retinal artery occlusion in a mouse model.

The aim of this study was to characterize the molecular and histological changes that occur in the retina following central retinal artery occlusion (CRAO) in a mouse model. CRAO was induced in 60 mice by laser photoactivation of intravenously injected rose bengal. Mice were sacrificed at 3, 6, 12, and 24h and 7 and 21 days after CRAO induction for molecular analysis (5-13 mice/time point) and histological and apoptosis studies (3-4 mice/time point). Fundus examination and fluorescein angiography were also performed at various points. Retinal mRNA was analyzed for expression of T-cell antigen …

Optic disc laceration with combined retinal artery and vein occlusion following penetrating injury.

Ocular trauma is a major cause of unilateral blindness and male adults in the working age group have been found to have higher rates of ocular injury. A case of a work-related penetrating ocular trauma with an intraocular foreign body causing an optic disc laceration and consequently a combined retinal arterial and venous occlusion is presented. The patient did not recover useful vision despite early surgical intervention. This case highlights an unusual sequelae following penetrating ocular trauma as well as the importance of safety eyewear for individuals in high-risk occupations.

[Inferior branch retinal artery occlusion in a young adult revealing rheumatismal valvular disease].

INTRODUCTION: Inferior branch retinal artery occlusion is uncommon in young adults. Through a case report and data from the literature, we describe the clinical, fluorescein angiography, and progressive details of this disease. CASE REPORT: A 26-year-old man suddenly presented superior scotoma in the left eye. He underwent a full ophthalmologic examination, which showed visual acuity limited to 5/10 P4 and ischemia of the inferior retina at the fundus exam. Fluorescein angiography of the left eye revealed delayed filling of the inferior branch retinal artery. The ophthalmologic exam of the ri…

Central retinal artery occlusion caused by expansion of intraocular gas at high altitude.

PURPOSE: To report a case of central retinal artery occlusion caused by expansion of intraocular gas during mountain travel at high altitude. DESIGN: Interventional case report. METHODS: A 46-year-old man underwent vitrectomy with intraocular gas tamponade for rhegmatogenous retinal detachment, right eye. During his trip to Mountain Ali with an altitude of 6,210 feet, he felt a sensation of fullness and noted loss of vision in his right eye. When he came down to sea level, an intraocular pressure of 54 mm Hg was present in the right eye. He visited our clinics 2 weeks after the episode, and …

Catastrophic antiphospholipid syndrome presenting as dilated cardiomyopathy with bilateral branch retinal artery thrombosis.

Cardiac manifestation of antiphospholipid syndrome (APS) is mainly in the form of left-sided valvular insufficiency, intra-cardiac thrombi or coronary artery occlusion. Dilated cardiomyopathy is a rare but important cardiac manifestation of APS, and responds well to adequate anticoagulation and steroids. We describe a case in which APS presented with dilated cardiomyopathy and bilateral retinal artery thrombosis.

Color Doppler image of central retinal artery of eyes with an intraconal mass.

PURPOSE: Retinal ischemia secondary to hypoperfusion of the central retinal artery is recognized as one factor that may contribute to the development of loss of vision in eyes with intraorbital tumors. We study intraorbital tumors which produce motility disturbances and visual problems by color Doppler imaging to evaluate this factor. METHODS: We examined the central retinal artery velocities of 3 patients with disc edema caused by intraconal masses (2 cavernous hemangiomas and 1 presumed optic nerve glioma) via color Doppler imaging. RESULTS: The time-velocity waveform demonstrated abnormal…

Cytomegalovirus infection in a healthy adult associated with recurrent branch retinal artery occlusion.

The authors report a 16-year-old healthy male who developed branch retinal artery occlusions sequentially in both eyes. A few vitreous cells and retinal vasculitis were associated findings. A complete work-up of the patient for an etiology for the occlusions failed to show any possible embolic source. Spinal fluid studies showed increased protein content and lymphocytic pleocytosis. Serologic studies showed a tenfold rise in the cytomegalovirus (CMV) titer. Other viral titers did not rise. CMV could not be cultured from the patient's body. The case represents a possible cause of the so-called …

Central retinal artery occlusion combined with posterior ciliary artery occlusion after preoperative embolization of the middle meningeal artery for intracranial meningioma: A case report.

RATIONALE: Preoperative embolization of brain tumors has been widely used to minimize hemorrhaging during surgery, but anastomosis between vessels is sometimes overlooked and complications can occur. Herein we describe a case of rare complications of central retinal artery occlusion (CRAO) and posterior ciliary artery occlusion after embolization of the middle meningeal artery. PATIENT CONCERNS: A 48-year-old woman experienced acute, painless loss of vision in her left eye during embolization of the middle meningeal artery for meningioma. DIAGNOSIS: The patient was diagnosed with CRAO and po…
