Retroperitoneal fibrosis (RF) often presents with characteristic clinical and urographic findings that suggest the diagnosis. However, malignant adenopathy may mimic this appearance. CT examinations of 10 patients with medial deviation of the ureters suggesting retroperitoneal fibrosis were reviewed. Six had idiopathic RF, two had perianeurysmal fibrosis, and two had malignant retroperitoneal adenopathy. Five cases are presented in which retroperitoneal fibrosis had entrapped the inferior vena cava and/or aorta and caused occlusive vascular disease. The diagnosis was known preoperatively in two cases and was made at laparotomy in the others. The fibrosis in one case was caused by abdominal tuberculosis. Eighteen patients who received surgical treatment for histologically proven idiopathic retroperitoneal fibrosis were reviewed retrospectively. In 6 patients, obstruction of one ureter recurred. In 3, histological evidence of invasion of the wall of the ureters by the fibrous plaque was found.

In one patient with pulmonary sarcoidosis, hypertension occurred during the course of the disease. Aortography showed extensive narrowing of the right renal artery. Surgical exploration disclosed extensive periaortic and perirenal fibrosis. This fibrosis encircled the right renal artery and caused extrinsic compression.
