Barriers to the Equitable Implementation of Risk-Based Cervical Cancer Management Guidelines.

BACKGROUND: The 2019 American Society for Colposcopy and Cervical Pathology provides risk-based management guidelines for abnormal cervical cancer screening test result management. It is not known whether populations facing inequities in primary care delivery have the prior screening history needed to take advantage of risk-based management of abnormal cervical cancer screening results. OBJECTIVE: Determine the prevalence and associations of unknown screening history for people with an abnormal cervical cancer screening result. DESIGN: Retrospective multi-center population-based study of thr

Algorithmic platform management and risk-taking behavior among Chinese food delivery riders.

Food delivery riders in China face widespread risk-taking behavior. This behavior is tied to algorithmic platform management. It is become more pressing as the digital platform economy expands. Drawing on Cognitive Appraisal Theory and Persistent Cognition Theory, this study develops a dual-pathway model integrating online immediate and offline sustained cognition, using two-wave questionnaire data from 320 riders in Changsha. Results show algorithmic platform management promotes risk-taking behavior via perceived algorithmic control and emotional rumination, while inhibiting it through proble

ACG Clinical Guideline: Perioperative Risk Assessment and Management in Patients With Cirrhosis.

This guideline presents a comprehensive approach to perioperative risk assessment and management in patients with cirrhosis. Recognizing the unique surgical risks in this population, the guideline emphasizes a multidisciplinary approach to preoperative evaluation, perioperative care, and postoperative follow-up. Key considerations include the severity of liver disease, nonhepatic comorbidities, and surgery-specific factors, with an emphasis on the use of validated cirrhosis-specific risk calculators, such as the VOCAL-Penn Score, for individualized risk stratification. Recommendations highligh

Adolescent Soccer Overuse Injuries: A Review of Epidemiology, Risk Factors, and Management.

INTRODUCTION: Overuse injuries are a growing concern among adolescent soccer players, with the repetitive nature of the sport placing significant physical demands on young athletes. These injuries can have long-term implications for physical development, performance, and overall well-being. This narrative synthesis aimed to evaluate the existing literature on the epidemiology, risk factors, and management strategies for overuse injuries in adolescent soccer players. METHODS: A comprehensive literature search was conducted using PubMed and Embase. A total of 123 articles were identified, 27 of

Endometrial preparation protocols for frozen embryo transfer: risk assessment and individualized management.

Frozen embryo transfer (FET) cycles have increased exponentially in the last decade. For many years, the most widely accepted protocol for endometrial preparation for FET cycles has been the artificial cycle (AC-FET), mainly due to the ease of coordinating the timing of embryo transfer with the operational needs of the IVF lab, the medical team and the patient. Accumulating data support that, due to the presence of corpus luteum, natural cycle frozen embryo transfer (NC-FET) is associated with better maternal and perinatal outcomes, especially lower preeclampsia risk, as compared with AC-FET.

Rare complications after esophagectomy: incidence, clinical features, risk factors, management, and prevention.

In recent years, common postoperative complications after esophagectomy have received increasing attention. However, the attention paid to rare complications, which often lead to serious consequences if they are not diagnosed in a timely manner, has not been sufficient. In this article, we present both the clinical and imaging features of rare complications following esophagectomy and strategies for their prevention and management. These rare complications are classified into four groups: esophageal substitute-related complications, thoracic duct-related complications, hernia-related complicati

A Review of Early Childhood Caries: Risk Factors, Management, and Policy Recommendations.

Initially, the American Academy of Pedodontics and the American Academy of Pediatrics (AAP) jointly addressed severe tooth decay related to bottle use by issuing a statement on Nursing Bottle Caries. However, over the next two decades, the American Academy of Pediatric Dentistry (AAPD) expanded its understanding, recognising that early childhood caries (ECC) resulted from various inadequate feeding practices, leading to the adoption of the broader term "ECC" to better reflect its complex causes. This review discusses ECC: its prevalence, risk factors, effects, prevention, early detection, and

CMV management of patients with leukopenia after CMV high-risk kidney transplantation.

BACKGROUND: For CMV high-risk constellations, guidelines recommend 3-6 months of prophylaxis with valganciclovir (VGCV). Management in preventing CMV primary infection in patients developing VGCV-associated leukopenia remains challenging. METHODS: We retrospectively analyzed the development of leukopenia during VGCV prophylaxis in 57 seronegative kidney recipients of a CMV-seropositive donor between 2008 and 2021. We analyzed CMV risk and development of CMV-specific T cells in the first post-transplant year depending on leukopenia during VGCV prophylaxis and management with CMV-IVIg. RESULTS

Assessing the evidence for effective biosafety risk management in Coxiella burnetii research.

Coxiella burnetii, the causative agent of Q fever, is a highly infectious zoonotic bacterium presenting significant biosafety challenges, particularly in laboratory environments. Due to its low infectious dose, environmental resilience, and potential for aerosol transmission, C. burnetii has been classified as a Risk Group 3 pathogen and Category B biothreat agent. This review evaluates the biosafety practices required for safe handling, including adherence to Biosafety Level 3 (BSL-3) containment for propagative work and BSL-2 standards for diagnostic activities. Strategies such as vaccinatio

Management of Cardiovascular Risk in the Non-alcoholic Fatty Liver Disease Setting.

Non-alcoholic fatty liver disease (NAFLD) is an overlooked and undetected pathology, which affects more than 32% of adults worldwide. NAFLD is becoming more common in Western industrialised countries, particularly in patients with central obesity, type 2 diabetes, dyslipidaemia and metabolic syndrome. Although NAFLD has traditionally been interpreted as a liver disease with a high risk of liver-related complications, NAFLD is an underappreciated and independent risk factor for atherosclerotic cardiovascular disease, which is the principal cause of death in patients with NAFLD. Treatment option

Surgical Management and Considerations for Patients with Localized High-Risk Prostate Cancer.

Localized high-risk (HR) prostate cancer (PCa) is a heterogenous disease state with a wide range of presentations and outcomes. Historically, non-surgical management with radiotherapy and androgen deprivation therapy was the treatment option of choice. However, surgical resection with radical prostatectomy (RP) and pelvic lymph node dissection (PLND) is increasingly utilized as a primary treatment modality for patients with HRPCa. Recent studies have demonstrated that surgery is an equivalent treatment option in select patients with the potential to avoid the side effects from androgen depriva

Obesity and cardiovascular disease: Risk assessment, physical activity, and management of complications.

The patient with obesity is at risk of developing cardiovascular disease and risk factors. Obesity negatively impacts prognosis and increases cardiovascular morbidity and mortality. Therefore, a comprehensive risk assessment is needed to define the cardiovascular risk of the patient and, thus, a tailored management and treatment. Chronic and successful management of these patients involves the evaluation of the various therapeutic strategies available (comprehensive lifestyle intervention, weight-loss medications, and bariatric surgery) and the diagnosis and treatment of cardiovascular complic

Aggressive T-cell lymphomas: 2024: Updates on diagnosis, risk stratification, and management.

INTRODUCTION: Aggressive T-cell lymphomas continue to have a poor prognosis. There are over 30 different subtypes of peripheral T-cell lymphoma (PTCL), and we are now beginning to understand the differences between the various subtypes beyond histologic variations. MOLECULAR PATHOGENESIS OF VARIOUS SUBTYPES OF PTCL: Gene expression profiling and other molecular techniques have enabled deeper understanding of differences in various subtypes as reflected in the latest 5th WHO classification of PTCL. It is becoming increasingly clear that therapeutic approaches that target specific cellular path
